1 Family Practice Sensitive Conditions Full data definition sign-off completed. Name of Measure Name of Measure (short) Family Practice Sensitive Conditions: Percent of emergency department or urgent care centre visits for health conditions that may be appropriately managed at a family physician s office. Family Practice Sensitive Conditions Developed by the Health Quality Council of Alberta (HQCA). This indicator measures the Percentage/Proportion of ED/Urgent Care Visits for diseases/conditions sensitive to management at family physician offices. NOTE: Clinicians will be engaged over the coming year to refine and improve on the definition of this particular measure. Preferred Display Format 999% Total number of ED/Urgent Care Visits for diseases and conditions sensitive to management at family physician offices [FPSC Conditions] for given geography (ie AHS Zone, Alberta).
