1 2017 Portland/Seattle Nu Fair Class and Speaker Information A1 2 Hours (0.2 CEUs) / The Art and Science of Seating: The Mat Evaluation Sponsor: Numotion Speaker: Susan Johnson Taylor, OTR/L Course Description: Complex rehabilitation technology for seating and wheeled mobility is a unique branch of the assistive technology field. It is essential for the individuals in the evaluation team, clinicians and suppliers, to understand the evaluation processes involved in recommendation of seating and wheeled mobility technology. This course provides an overview of the important evaluation process with an emphasis on the physical mat assessment. This course will provide the participant with details of the physical mat assessment as well as how those details are used in equipment recommendations. During the lab section, the participants will be provided guided practice of components of the postural assessment. 1. The participant will list 2 possible goals of the evaluation. 2. The participant will name 2 bony prominences of the pelvis used to describe pelvic position. 3. The participant will list 2 outcomes from a mat evaluation. 4. The participants will 2 key angles from the mat evaluation. 5. The participant will name 3 key linear measurements from the mat evaluation. Speaker: Susan Johnson Taylor, OTR/L is an occupational therapist who has been practicing in the field of seating and wheeled mobility for 35 years primarily in the Chicago area at the Rehabilitation Institute of Chicago Wheelchair and Seating Center. While at RIC, she also created and participated in a variety of research studies at the Northwestern U Sensory Motor Perception Program. Susan has published and presented nationally and internationally and has consulted on product development for a variety of manufacturers. Susan is both a member and fellow with RESNA, holding such past positions as Publication Committee Chair and Instructional Program Chair for their annual conference as well as serving on the Board of Directors. She is currently a member of the RESNA /ANSI Wheelchair Standards Committee and the Clinician s Task Force. Susan joined the Numotion clinical education team in 2015 as the Manager of Training and Education speaking on a variety of topics in CRT. A2 2 hours (0.2 CEUs) / Is Your Client Stuck? Dynamic Seating Gets Things Moving! Sponsor: Seating Dynamics Speaker: Michelle Lange, OTR/L, ABDA, ATP/SMS Course Description: Dynamic seating has three primary functions to allow movement, to diffuse force and to protect the seating system and mounting hardware. Dynamic options continue to expand and improve. This course will take a look at the product options, discuss clinical indicators and contra indicators for dynamic components and present case studies to illustrate these points. 1. The participant will be able to list 3 clinical applications for dynamic wheelchair components vs. static options. 2. The participant will be able to list 3 clinical indicators for a dynamic movement of the pelvis. 3. The participant will be able to list 3 clinical indicators for dynamic movement of the lower extremities. 4. 2 Speaker: Michelle is an occupational therapist with 25 years of experience and former Clinical Director of The Assistive Technology Clinics of The Children s Hospital of Denver. She is a well respected lecturer, both nationally and internationally and has authored 4 book chapters and nearly 200 articles. She is the editor of Fundamentals in Assistive Technology, 4th ed. and Clinical Editor of NRRTS Directions magazine. Michelle is on the teaching faculty of RESNA. She is on the RERC on Wheeled Mobility Advisory Board. Michelle is a credentialed ATP, credentialed SMS and is a Senior Disability Analyst of the ABDA. A3 2 Hours (0.2 CEUs) / Custom Seating Solutions: Designing a Seat Support Sponsor: Comfort Company Speaker: Stacey Mullis, OTR/L, ATP Course Description: An off the shelf seat cushion is not always sufficient to address complex seating goals. A mold can be restrictive, rigid, and costly, with little room for change/growth. This course will provide an alternative to molds and instruct on how to build a cushion from the base up, incorporating growth. Participants will learn important foam properties and applications, and the clinical applications of cushion components that create the geometry of the cushion. 1. The participant will be able to define the 2 most relevant properties of foam. 2. The participant will be able to determine the most appropriate foams for each layer. 3. The participant will be able to describe one clinical presentation for each cushion base shape. 4. The participant will be able to list 2 styles of adductors and their clinical application. 5. The participant will be able to list at least 3 cushion building blocks and their clinical application. Speaker: Stacey is Director of Clinical Education for Comfort Company. She is a Canadian trained Occupational Therapist of 20 years. With experience in pediatrics, inpatient/outpatient rehabilitation, long term care, and home health, Stacey has faced the challenges of providing appropriate seating in various settings. This led her to pursue an apprenticeship in Asheville, NC to advance her skills and she now focuses her efforts on educating her colleagues on principles of seating and positioning. B1 2 Hours (0.2 CEUs) / Defensible Clinical Documentation for Seating & Wheeled Mobility Services Sponsor: Numotion Speaker: Dr. Laura Cohen, PhD, PT, ATP/SMS, RESNA Fellow Course Description: More rigorous requirements and greater scrutiny for claims for seating and wheeled mobility has made defensible documentation even more critical. Implementation of electronic health records (EHRs) in the clinics has resulted in inefficient and oftentimes redundant documentation processes. Even so, Medicare claim denial rates remain high due to insufficient medical documentation for SWM requests. More and more third party payers are scrutinizing the clinical documentation from clinicians to find evidence to support the need for the requested DME and CRT equipment. This course will provide a comprehensive look at the elements of appropriate and complete clinical documentation, facilitate critical thinking and assist the clinician in developing sound documentation practices. Participants will engage in interactive discussions about common mistakes, red flags, and recommended practices. Opportunity will be given to consider case examples from the perspective of a reviewer to identify improved ways to document information. Discussion will include strategies to utilize electronic health records (EHR) systems efficiently to meet the documentation needs for clinical services and DME requirements. 1. The participant will be able to explain 3 ways in which Medicare documentation standards have changed. 2. The participant will be able to identify at least 3 things that could be improved in sample documentation examples. 5. The participant will be able to describe 2 strategies to discuss with administrators related to EHR system to optimize efficiencies. Speaker: Laura Cohen is the Principal of Rehabilitation Technology Consultants, LLC. Since 2004 she has served as the Executive Director of the Clinician Task Force a national group of seating and mobility clinicians working to influence Medicare and Medicaid coding, coverage, and payment policies for complex rehabilitation technology devices and services. She has over 30 years of experience in rehabilitation including strategic planning, management, research, clinical practice, teaching, accreditation, and credentialing. She is a consultant to clients for rehabilitation and the complex rehabilitation technology marketplaces. Dr. Cohen advises clients on the legislative and regulatory environment, medical review, benefit integrity, policy development, and compliance. A recognized thought leader, Dr. Cohen has been appointed by Federal and State government agencies and contractors to serve on various panels and technical advisory boards tasked with recommending policy improvements for DME services. At the request of the U.S. Senate Committee on Finance, she provided testimony for a hearing on Medicare policy for power wheelchair provision. Presently she serves on the Steering Committee to establish a Medicare Separate Benefit Category for Complex Rehabilitation Technology. Her areas of policy expertise include post acute care, CRT, DMEPOS, electronic health records, and dual eligible populations. Her pursuit is to ensure individuals with complex mobility needs have access to quality rehabilitation and complex rehabilitation technology (CRT) services and devices from qualified professionals through responsible utilization management. B2 1 Hour (0.1 CEU) / Aging with a Disability: Research and Clinical Perspectives Sponsor: Numotion Speaker: Susan Johnson Taylor, OTR/L Course Description: A seating and mobility evaluation cannot begin without an understanding of the client s diagnoses and associated conditions. Anyone working in the rehabilitation field, including specialists in seating and wheeled mobility must understand the implications of and changes that occur in those clients who are aging with a disability. Before one can understand the issues surrounding aging with a disability, the natural history of surviving disabling conditions must be understood. In the last 25 years or so, there has been a great deal of research into the effects of aging with disabilities. That information has been slow to make its way to the clinical environment. Also, many of the clients that are seen with aging problems by clinicians went through rehabilitation at a time when philosophies differed markedly from today. The combination of all of these issues necessitate an understanding of all the factors involved, both the best clinical approaches as well as an awareness of research findings that can be incorporated in to their own clinical practices. 1. The participant will be able to identify 2 research studies that can be used clinically. 2. The participant will be able to list one factor of today s rehabilitation philosophy that is different from about 40 years ago. 3. The participant will be able to identify one factor of aging with a spinal cord injury to consider when interviewing the client. 4. The participant will be able to identify 2 factors relevant to aging with a disability to consider when interviewing the client. 5. The participant will be able to identify 2 considerations prior to making seating and mobility recommendations. B3 1 hour (0.1 CEU) / Pressure Injury Update: Reviewing Current Evidence and Terminology Sponsor: Permobil Speaker: Darren Hammond, MPT, CWS Course Description: Historically, pressure ulcer etiology has revolved around ischemic changes in the skin, and soft tissue. However, recent evidence has been introduced where tissue deformation of the skin and soft tissue has earlier implications in pressure ulcer development versus just ischemia alone. Currently there are various levels of clinical knowledge and experience in the allied healthcare community when it comes to understanding pressure ulcer etiology and appropriate management strategies. Unfortunately, most clinicians who are actively participating in wound management, do not have the full understanding of the pathophysiology of pressure ulcers. There is also a lack of clear understanding of the extrinsic and intrinsic risks, which need to be correctly managed for successful outcomes. This interactive session will review the current best evidence of pressure ulcer pathophysiology including ischemia, reperfusion tissue injury and tissue deformation. Discussions will revolve around the differences in the development of a superficial pressure ulcer versus a suspected deep tissue injury. In addition, the intrinsic and extrinsic risk factors commonly associated with pressure ulcers will also be discussed along with strategies to minimize the risks. 1. Participant will be able to define the pressure injury staging guidelines according to the NPUAP staging guidelines. 2. Participant will be able to list the four accepted pathological mechanisms causing a pressure injury. 3. Participant will be able to describe three intrinsic and the four extrinsic risk factors in developing pressure injuries. 4. Participant will be able to contrast three differences in the pathology of pressure injuries related to deformation. Speaker: W. Darren Hammond, MPT, CWS, has held clinical positions as a physical therapist and certified wound specialist in the United States for over twenty four years. Darren has also held faculty appointments at various Physical Therapy programs in the Philadelphia, PA region, including Drexel University, Temple University and Thomas Jefferson University of the Health Sciences. He is currently the West Regional Clinical Education Manager for Permobil. In addition to wound care, Darren s focus of treatment has been in the rehabilitation of individuals with spinal cord injuries. Throughout his career, Darren has had extensive experience with seating and positioning, chronic wound management, pressure mapping and cushion and wheelchair prescription. Mr. Hammond has achieved board certification as a Certified Wound Specialist by the American Board of Wound Management. 5 B4 2 hours (0.2 CEUs) / From the Mat to Paper: A Real World Approach to Documenting Seating Evaluations with Successful Mobility Outcomes Sponsor: Numotion Speaker: Andria Pritchett, Director of Clinical Education Course Description: This course is designed to provide an overview of the information necessary to capture during an evaluation for a Complex Rehabilitation Technology (CRT) seating and wheeled mobility from the point of view of what is necessary to obtain funding. Medicare funding rules will be used as a model, as they tend to be the gold standard in terms of justifying client functional need related to equipment choices. This will assist the clinician and ATP in collecting the appropriate information which will facilitate successfully matching the clinician s findings to the ATP s specific recommendations of CRT equipment. 1. Participant will be able to demonstrate objective documentation using Medicare s algorithmic approach. 2. Participant will be able to name 2 objective measures during the mat evaluation that could be used to help justify an ultralight weight manual wheelchair. 3. Participant will be able to list 2 very commonly overlooked details when prescribing custom molded or custom fabricated seating and how to keep those denials from happening again. 4. Participant will be able to describe 2 ethical strategies for documenting a typical day when the client has a diagnosis that changes within the hour, day, week, etc. Speaker: Andria Pritchett, Director of Clinical Education, has been with Numotion since Prior to beginning her career in complex rehab technology, she was a practice administrator for a large multi specialty therapy clinic. Upon shifting focus to complex rehab technology in 2010, Andria has used her extensive management, clinical and Medicare knowledge to develop and manage the traditional Medicare portion of many aspects of business at Numotion. She has managed Medicare Billing and Medicare Audits, ALJ Hearings, Funding and Order Processing and took a very active role in the Medicare education program within Numotion. As a current committee member for Medicare Jurisdiction D DME MAC Advisory Committee Rehab A Team, this vital role allows her to stay on top of the reimbursement environment and educate both internally and externally at the highest level. Andria currently develops a wide range of curriculum and educates Numotion leadership, ATPs and external clinicians nationally on a variety of topics. C1 3 Hours (0.3 CEUs) / Power Wheelchair Programming: Options to Maximize Safety and Independence Sponsor: Permobil Speaker: Darren Hammond, MPT, CWS Course Description: Have you ever driven a power wheelchair and thought it just doesn t feel right? This 3 hour class will explore all the basic programming options that can help a marginally safe PWC user become more confident and independent. Through a combination of lecture and lab based learning, participants will be able to adjust, re adjust and experience the most commonly adjusted power programmed drive parameters to better understand how to decrease frustrations for their patients and create safe, functional power wheelchair drivers. 1. The participant will be able to identify two differences between proportional drive and switch drive control inputs. 2. The participant will be able to list 2 common challenges in driving a power wheelchair. 3. The participant will be able to list to effects on the client between high and low speed, and fast and slow acceleration programming parameters. 4. 6 5. The participant will be able to list 4 common drive parameters to consider adjusting to ensure maximum patient safety and independence. Speaker: W. After this course, a LTC therapists will be better informed on basic seating and positioning principles to better choose a wheelchair, cushion and back support. 1. The participant will be able to name three common misconceptions that lead to positioning mistakes. 2. The participant will be able to name three reasons why a K0004 MWC is a better option in the LTC seating. 3. The participant will be able to describe three limitations of ELRs when used on a MWC. C3 2 Hours (0.2 CEUs) / The Effects of Manual Wheelchair Configuration on Function: Clinical and Research Aspects Sponsor: Numotion Speaker: Susan Johnson Taylor, OTR/L Course Description: The configuration of a manual wheelchair can have a profound effect on the user s function. The evaluation for and adjustment of the manual wheelchair requires attention to detail to match the adjustments to the user s lifestyle and physical function. This course will provide an overview of the evaluation process to assist in asking the right questions relative to functional and ADL skills, as well as lifestyle, and how and where the wheelchair is used. Review of the current literature regarding manual wheelchair set up and the incidence of shoulder pain, will be provided to assist in understanding of the evidence around this subject. The effect of these adjustments on user performance will be reviewed. Finally, the importance of structured education and training for manual wheelchair use will be reviewed. In the 2 hour version of this class, a onehour lab will provide guided breakout groups. The attendees are provided with real life case studies as well as a manual wheelchair that has been pre adjusted with certain problems. The attendees will be asked to verbalize and problems solve regarding how the wheelchair should be adjusted to address the case study problem. These cases are then presented to the whole group.
