7 Cold Therapy Supporting Documentation Title: Effects of Cold Therapy in the Postoperative Management of Pain in Patients Undergoing Anterior Cruciate Ligament Reconstruction Source: Amer J Sports Med 17 (3): , Author: Cohn BT, Draeger RI, Jackson DW Overview: In a prospective study done by Cohn, et al. 54 consecutive arthroscopically assisted ACL reconstructions were assessed for the amount of postoperative pain relief provided by cold therapy. Twenty-six patients using a cold therapy unit postoperatively were compared to 28 patients having the identical procedure, but without cold therapy. According to the results, the cold therapy patients were more helpful in self-assistance, were out of bed and ambulating in the halls more quickly, and did their range of motion exercises with greater ease. Cold therapy patients required less pain medication than the other group, and made the conversion from injectable to oral pain medication an average of 1.2 days sooner than the non-cold therapy patients. 2 Title: The Efficacy of Cryotherapy in the Postoperative Shoulder Source: J Shoulder Elbow Surg 5(1):62-8, Jan-Feb, Author: Speer KP, Warren RF, Horowitz L Overview: Speer, et al. evaluated the efficacy of cryotherapy in post-operative shoulder in pain, comfort, sleep, analgesic use and overall satisfaction. The authors found that pain on the night of surgery was less severe and occurred less often in the group receiving cryotherapy as compared to those that did not. The cryotherapy patients also slept better on the night of surgery and perceived the need to use pain medicines less often. The authors believe this study shows that cryotherapy for the post-operative shoulder is a useful adjunct in decreasing post-operative pain, discomfort, swelling and the need for narcotic analgesic medication. 10 Title: Continuous-Flow Cold Therapy for Outpatient Anterior Cruciate Ligament Reconstruction Source: Arthroscopy 14 (2): 130-5, March Author: Barber FA, McGuire DA, Click S Overview: In this prospective, randomized study, Barber evaluated continuous-flow cold therapy for postoperative pain in outpatient arthroscopic ACL reconstructions. The first group in the study used cold therapy constantly for 3 days, then as needed for days 4 through 7. The second group had no cold therapy. Assessments were made on pain, CPM usage, prone hangs, range of motion, and need for pain medication. Results showed that continuous-flow cold therapy lowered pain scores, reduced Vicodin use, and increased prone hangs, CPM, and knee flexion. Continuous-flow cold therapy was found to be safe and effective for outpatient ACL reconstruction reducing pain medication requirements.
