Focusing on developing a community based treatment facility to meet today s therapy needs under current insurance plan restrictions. Focus of care is patient education and home exercise programs, including a specialized program called post rehabilitation fitness. Focus of care is back and neck treatments from acute to chronic. Special emphasis is Spinal Rehabilitation and Post Op Spinal Fusion; Cervical, Thoracic, and Lumbar Rehabilitation; as well as Rehabilitation for Micro Discectomys. Special time scheduling and specific space in clinic is allocated for children on Tuesdays and Thursdays. Child typical injuries and disabilities currently treated: Neurological Dysfunctions Muscular Dystrophy Head Injury Orthopedic Rehabilitation Special Techniques with treatment with children include: Hand-on Developmental Techniques that incorporate movement and Cranial-Sacral method. This enhances movement and behavioral responses. Developed physical therapy programs for two locations. The focus of care was for spinal injuries and disabilities for adults and children. Majority of work was Post Op Fusion and Discetomy for lumbar and cervical areas. Treatment was for pain control, restoration of functional activities and appropriate strengthening and core training. Continued to see patients in a wide range of diagnosis from children with neurological needs to adults with work related injuries and elite athletes. Continued to work with adults and children with Scoliosis, Osteoarthritis, and Juvenile Rheumatoid Arthritis Dallmeyer Physical Therapy, Inc. Owner, Chief Physical Therapist Santa Barbara, CA Full service physical therapy facility with an adjacent rehabilitation and strength area. Also offering Occupational Medicine services as well as services by Doctor of Chiropractic. Support staff consisting of eight part-time massage therapists, exercise physiologists, and bilingual office staff. Built this practice up to 1,160 patient visits per month using a philosophy and emphasis on a team approach with the patient, to provide optimum benefit and rehabilitation. Primary emphasis on personal, intensive, supportive patient care. Emphasis on spine and neck care from acute to chronic; Discectomy Rehabilitation; and Post Op Fusion. This emphasis was the majority of our patient population. Effective core strengthening program established, utilizing flexibility and Palates type movements. Patient Education was a constant part of our environment. 3 Santa Barbara Fitness and Physical Therapy Center Sports and Rehabilitation Center Founder, CEO Opened this 4000 sq. ft. full-service physical therapy office and adjoining strength training facility in response to the needs of chronic injury patients and treatment to disabled children. The non-disease model, athletic setting provided an environment both challenging and comforting. Focus of care was back and neck care for acute to chronic injuries and post op fusion patients. The setting allowed grater variety for aggressive treatment for rehabilitation of children and adults. This setting was especially useful to children with Cerebral Palsy with Spasticity. This setting allowed appropriate separate space for children but also able to use added equipment for strengthening not found in many clinics. This setting allowed a wide variety of patient care from catastrophic patients to the elite athletes. 4 St. Francis HospitalSanta Barbara, CA Director of Physical Therapy Developed a team approach between hospital s occupational, respiratory and physical therapists and physicians. Developed a community support for children with developmental disabilities. Developed direct referrals from the acute setting to the rehabilitation institute for children and adults with disabilities. It included children with, Muscular Dystrophy, Head Injury, Brain Damage, Spinabifida, and Spastic Disorders. Started new programs and improved existing services for the 99 bed facility and outpatient clinic. Built the three-person service to a Physical Therapy Department of eleven, including five full-time, physical therapists. Developed and supervised program for Post Op Orthopedic Surgical patients for neck and back surgeries and fusions; Total Joint Replacements of the knees and hips; other surgical care for the ankle, elbow, and shoulder. Invented the Hot Pool exercise program for arthritic children and adult. The National YMCA currently offers this program in hundreds of YMCA clubs throughout the United States. Designed and implemented the Pulmonary Rehabilitation Program. Patients used supplementary oxygen supplies while exercising to regain self-sufficiency as a result of increased strength. Worked with the Lung Association Outreach Services motivating patients to continue their exercise programs at home. Reorganized the Stroke Rehabilitation Program. Coordinated Hospital Occupational Rehabilitation and Speech Therapy Departments with Physical Therapy. Conducted the program in conjunction with Memorial Rehabilitation Hospital. Founded the Chronic Pain Management Clinic, a prototype team approach. Modalities and therapy were selected to suite individual children and adult particular physical, emotional, social and spiritual needs. Guided teams involving patients, medical social workers, community volunteers, area physicians and psychotherapists. Reduced pain and increased quality of daily living for the thirty patients involved in the one year project. Initiated the Cardiac Rehabilitation Program with the support of area cardiologists. Provided follow-up care, education and exercise regimes for children and adults. Advanced the level of expertise in the treatment and rehabilitation of burn victims for children and adults.

5 Memorial Rehabilitation Hospital Santa Barbara, CA Staff Physical Therapist Evaluated and treated victims of catastrophic injury and illness: Paraplegics, Quadriplegics, Brain Damage Victims, Stroke Victims, Multiple Sclerosis, Guillain Barre, Bilateral Amputees, Post Op Rehabilitation for Spinal Fusion patients. Motivated angry and depressed patients. Kept them working hard to achieve their highest level of function. Constantly re-evaluated each patient, intensifying the therapy program to challenge and support changing physical abilities and emotional needs. Carried out long-term therapy programs which progressed from range of motion stretching in the acute post-injury stage, to muscle strengthening exercise leading to wheelchair transfer and driver training. Made home visits. Suggested ways to adapt living arrangement. Trained home attendants. Taught pressure-sore self awareness. Maintained follow-up evaluation and care. Evaluated and treated children of all ages from infants to teenagers with physical and mental disabilities. Treatment to children that included bracing, developmental-neuromuscular movements. Strengthening for functional changes and gait training. Teaching parents to continue care at home. Participating with team conferences that included medical doctors, occupational therapists, speech therapist and orthortist New England Memorial Hospital Stoneham, Massachusetts Physical Therapy Team Leader Sports Medicine Supervised a team of six physical therapists and assistants. Carried out the full range of physical therapists services required by this 500-bed facility and its outpatient clinic. Treated and provided rehabilitation for all orthopedic cases including surgeries for knees, hips, shoulders, and elbows; spinal surgeries including fusions and laminectomys. Specialized in the management and rehabilitation of burn patients that included children as well as adults. Treatment of children with, Muscular Dystrophy, Spinabifida, Amputees, Developmental Disorders and Head Injuries. Treatment included hands on care, bracing, use of assistive devices such as walkers, wheel chairs. Instructed families to assist in home care. A lifelong competitive athlete, currently coaching and competing in power lifting, Have provided specific weight training programs and strength coaching for athletes in the following sports: Power lifting Senior National Level Heptathlon World Class Level Track and Field World Class Level Rowing World Class Level Football Professional Level Western States 100 Mile Run National Event
