3 WHAT IS CORNEAL THERAPY? Non-surgical procedure that eliminates the need for spectacles or daytime contact lenses. It involves wearing special rigid gas permeable lenses while sleeping. These lenses reshape the corneas with the intension of providing acceptable vision for the duration of the following day.

4 PATIENTS ENQUIRY FOR CORNEAL THERAPY Staff members are dedicated to offer professional advice to patients who enquires about Corneal Therapy. Patients are asked the following questions by the staffs: - Where they heard about Corneal Therapy, if they are wearing glasses, soft contact lenses, or done any surgery. - They also ask, when and where they had last eye test, health conditions, and what is their prescription. - If patient are satisfied and willing to go ahead with the treatment they are booked in for a consultation.

5 PROCEDURE FOR CORNEAL THERAPY 1. Consultation fittings, discussion of therapy. 2. Order Therapy Lenses st Collection - Trained staff will teach patients how to put therapy lenses in and take them out. 4. Once the patient got their therapy lenses in, they are checked thoroughly, regarding their fitting and correction. 5. After Collection - Patients are booked within three days for their first aftercare. If everything is ok, patients are seen again in 3 months and again 6 months.

9 CORNEAL THERAPY - CONTRACT Corneal Therapy Centre Following the examination by: I have read and understood this document outlining the process of overnight vision correction. I understand the process involves wearing specially designed rigid gas permeable lenses while sleeping. These lenses will reshape my corneas with the intension of providing acceptable vision for the duration of the following day. Off-label lenses may have to be used in order to provide better correction, as this is customised procedure and this has been explained to me thoroughly. I understand that the process is temporary and reversible, and that my vision may fluctuate depending on the length of time I wear the lenses. I understand that if I do not wear the lenses as advised then the quality of my vision will change. It has been explained and I understand that the success of the procedure cannot be guaranteed, and if I do not find the results acceptable then process can be reversed over a period of time. I understand the importance of attending for regular after care examination and will do so as advised. I understand how to handle the lenses and how to clean them, and will do so daily as per instructions. Risks: While harmful health risks are not anticipated or intended by using overnight vision correction, there are potential risks common to all contact lens wear such as irritation, infection and corneal ulceration. If your eyes(s) become irritated/itchy and red or you have any concerns regarding the health of your eyes you should return for an examination and stop wearing your lenses immediately. Alternatives: Currently available altenatives to overnight vision correction are: spectacles, soft contact lenses, rigid gas permeable contact lenses and surgical correction. These alternatives have been offered and discussed. Benefits: Overnight vision correction lenses have been designed to provide good vision and high oxygen transmissibility to the eye. The design of the overnight vision correction lens should provide an improvement in an unaided distance vision. It is believed that the process is reversible and changes to the eye are temporary. Costs: Initial consultation Includes refraction, topography, external ocular assessment discussion of overnight vision correction process. (Stand alone cost of payable at time of exam). Provided the standing order has been continued. Discontinuation/Refund- If the process is deemed unsuccessfully by practitioner or patient within the treatment programme none of the initial payment will be refunded.
