PERCEPTIONS OF GENETIC TESTING AMONG HYPERTENSIVE BLACK MEN - HIMB GENX Markeya Menefee Cheyney University LDI SUMR Scholar Principal Investigator: Lisa Lewis, PhD, RN, FAAN
AGENDA 1. Hypertension related disparities in the Black community 2. An overview of HIMB GenX 3. HIMB GenX s preliminary findings 4. Lessons learned while working on HIMB GenX
SIGNIFICANCE: HYPERTENSION PREVALENCE IN THE U.S 75 million adults in America have hypertension. 32% of the population 1 in every 3 adults Has no warning signs or symptoms Source: Center for Disease Control High Blood Pressure Facts
SIGNIFICANCE: BURDEN OF HYPERTENSION IN U.S. BLACKS When compared to whites, blacks have a: % greater rate of nonfatal stroke % greater rate of fatal stroke Blacks Whites 420% greater rate of end-stage kidney disease Benjamin et al., 2017; Wong et. al., 2002
SIGNIFICANCE: BURDEN OF HYPERTENSION IN U.S. BLACK MEN Percentage of men with hypertension stratified by race Death rates for men due to HTN by race (percent-age) Black Men White Men 5 0 Black Men White Men CDC, 2010; Benjamin et al., 2017; Wong et. al., 2002
SIGNIFICANCE: POTENTIAL GENOMIC INFLUENCES ON MEDICATION ADHERENCE Rationale for HIMB GenX ØNeed a more comprehensive approach to medication adherence that considers genetic influences ØPrevious genomic studies are preliminary and few but promising ØVery little genomic work has been conducted with hypertensive black men ØNo knowledge about their interest to participate in genetic studies Axelssson et al, 2011; Seo et al., 2014; Homberg et al., 2012; Peprah et al., 2015; Rosenberg et al., 2010
PERCEPTIONS OF GENETIC TESTING AMONG HYPERTENSIVE BLACK MEN: HIMB GENX Primary objective To determine the feasibility and acceptability to consent and collect DNA samples from 100 hypertensive black men enrolled in an ongoing examination of medication adherence factors. Primary outcome measures Feasibility: assessed using scientific metrics of feasibility Acceptability: assessed using one item on a study questionnaire Was participating in the study difficult for you? (not hard at all; somewhat hard; or too hard)
PERCEPTIONS OF GENETIC TESTING AMONG HYPERTENSIVE BLACK MEN: HIMB GENX Secondary objective Assess attitudes towards genetic testing among hypertensive black men enrolled in an ongoing examination of medication adherence factors. Secondary outcome variable Attitudes toward genetic testing: assessed using the Attitudes Towards Genetics subscale of the Perceived Genetic Knowledge and Attitudes survey.
PERCEPTIONS OF GENETIC TESTING AMONG HYPERTENSIVE BLACK MEN Recruitment and Enrollment Participants from parent study (HIMB) were contacted. Data Collection Questionnaire about attitudes towards genetics was administered. DNA collection. Results Recruitment and enrollment rates.
HIMB GENX PARTICIPANT RECRUITMENT Parent Study Description Examines relationship between self efficacy, social networks, depression, physician trust, masculinity and medication adherence. Study population consisted of 200 hypertensive black men. Longitudinal study, followed participants for 12 months from baseline. *No longer enrolling participants but data is still being collected
-HIMB GenX Study Participants
DATA COLLECTION Potential participants were contacted by phone. Participants interested in the study were scheduled for a study visit. Measuring feasibility Documented All phone contact. Consent and enrollment rates. Refusal rates and reasons.
DATA COLLECTION Measuring Attitudes Attitudes Towards Genetics Questionnaire Survey consisted of statements that looked to further understand the men s attitudes towards genetics and genetic testing. 13 questions Likert Scale If I had a chronic disease, I would want to know if it was hereditary. A genetic discovery could change a person s health outcomes. I think the development of genetic research is a positive scientific advancement. The idea of genetic testing frightens me.
DATA COLLECTION: DNA COLLECTION Measuring acceptability of DNA Collection After the specimen was collected, participants were asked Participants were asked to provide a sample of DNA using their saliva. Was participating in the study difficult for you? Oragene-Discover OGR-500 User friendly, non-invasive Low maintenance
PRELIMINARY RESULTS Started contacting potential participants at the beginning of July. 53 calls were made in total. 41 Men were consented and enrolled into the study. 1 Only one person declined participation. 11 Unable to contact
LESSONS LEARNED Experience with qualitative research. Research is detail-oriented. Making research your own.
