It keeps evolving, and so Dr Lycka is talking with his friend Dr Charles Crutchfield, from Minneapolis, about some of the latest advances and news in cosmetic surgery here in June Welcome, Dr Crutchfield! It s great to be here today talking about some cool new stuff in aesthetic medicine! One of the coolest things, Charles, is something that we have been working on over the last couple of years. That is to make sure a patient gets a good, ethical doctor, and has a great aesthetic experience. Please tell our listeners about that. We started a group of physicians that are very responsible, who care about their patients, and we have a code of ethics. One of the codes is that any time you would like to have any type of aesthetic procedure, then the doctor will examine you, and make sure that you are appropriate for the procedure. The doctor will prescribe exactly what you need and oversee or personally do all the treatments. Also if you have any questions, concerns, or complications, then they will see you and take care of you. We think that this is SO important. Yes, this is so important and our audience needs to realize that there are centers out there that do not have doctors directly involved in the care of the patients undergoing treatments. Yes unfortunately Dr Crutchfield sees this problem all the time. There are people who lack ethics and so what they essentially do is rent out their medical license and will be affiliated with 5,10, sometimes 20 strip mall spas, who can order FDA regulated medicines under their license but that doctor is never there! They have unqualified people doing medical procedures and lasers and injections, and all kinds of things that can be very dangerous. When they have complications (which they often do) then there is no-one to help the patients. Then the patients come running to other ethical doctors offices, like Dr Lycka & Dr Crutchfield, asking for help! They say they had all sorts of procedures done, nothing is right, and when they call the office in the mall, no-one will return their calls, when they ask to see the doctor, they hang up on them. Many listeners will be aghast about that. Dr Lycka & Dr Crutchfield know it is a fact, but why don t the regulatory bodies step in and do something about this situation, Charles? Well we are not quite sure, but we have decided to take the bull by the horns and set up a society where people can find real physicians, from specialties trained in aesthetic medicine, such as dermatologists, plastic surgeons, oculoplastic surgeons, ophthalmologists. People who are trained in working on the face, so if you want someone who is qualified and cares, then you can come to the group and find physicians who will see you personally, and who will take full responsibility for your treatments. 2 Yes, that is the important thing. Everyone listening has to realize that the number one thing that is going to determine your cosmetic surgery goals and results is your choice of the facility & doctor that is going to do it. The onus is on YOU. You can t expect other people to protect your rights you have to do it yourself. The best way that you can do that is by finding a doctor who is moral and ethical, that cares enough about you that they are going to put their name on the line. We think that this is good for ethical physicians and think it is great for the patients! We are aiming at high quality treatments, high quality results, in a very safe and ethical manner. Yes, that is the key safe & ethical. Real doctors, real treatments, really safe because it is as safe as it can ever get. Once a doctor is putting their name and reputation to something, not just on paper overseeing loads of centers and not even being there, then that is when we get some real breakthroughs. Dr Crutchfield had a physician call him who was essentially renting his license out to a strip mall spa. A patient had a problem, and was demanding to see him. He asked Dr Crutchfield how he should handle the situation! At the end of the call, he then asked for direction to that center the center he was claiming to be the medical director of. He had never been there in his life Yes, there are some doctors, a small amount, that are terribly unethical. They only care about the money, and don t care about the consequences, and who are not at all good for the profession at all. Dr Crutchfield agrees 100% that the vast majority of doctors are fantastic, they work hard, they care about their patients but there is a small percentage, who don t. And it is this new group who are trying to make sure that patients avoid those doctors, they are not a wise or good choice at all. That is why an organization such as this is so important. It is why Dr Lycka has put his name to this organization because it is the right thing to do. And that is what differentiates doctors such as Dr Lycka & Dr Crutchfield they do the right thing and do it all the time. The name of the organization is Doctors For The Practice Of Safe & Ethical Aesthetic Medicine. We have a website where you can look up physicians and find out about our philosophy. It s a great thing for patients especially as aesthetic medicine is evolving and every year there are new procedures & treatments coming out, and even better results. Dr Lycka & Dr Crutchfield have known each other for over 20 years and have worked on many projects together. So please check out the DPSEAM group. Moving onto another area, this next item is still referred to as new although it has been out for 10 to 15 years because most people haven t heard about it. Yes Platelet Rich Plasma. It s one of those things that even though it has been around for a while, there are newer and newer uses for it and different results. It has been used in sports medicine probably for almost 15 years, on professional athletes and sports people with joint problems & pain. The US superstar basketball player Kobe Bryant swears by it. And Tony Parker, another pro basketball player, has also been using it. Even in Minnesota, the Governor Mark Dayton went to the Mayo clinic last summer and had PRP injections in his hips to improve his mobility and discomfort in his hips. However, we started using it aesthetically in skin and other things probably about 4 or 5 years ago. So it s been known about for a long time but we are discovering new treatments and applications for it. It is fascinating and unique because it is so natural. Everyone is looking for a natural treatment that works this is one of them. Our blood contains 2 parts. The red cells that are important when we get a blood transfusion. But there are also other items that sit in the blood. This is called the serum, or the plasma. We can separate that and take that serum off and then activate it. Then we put it back into a person s body and that is when the miraculous things happen. The activation and re-injection back into the body make such a difference. 3 The serum is yellow / straw colored but it is chock full of all kinds of different proteins, hormones, growth factors, cytokines, that do all kinds of miraculous things to cells, as far as controlling the way they grow, and their repair mechanisms, rejuvenation mechanisms, and we have got to the point now where we can actually concentrate these different cytokines and growth factors and separate them out, and them put them back into a spot where we need some relief. Examples are repairing a joint, such as repairing tennis elbow, helping a hurt knee, uses that we have known about for years. But now we are using it in the skin to help rejuvenate the face. It is rather embarrassing that it has taken us so long to get into this since sports medicine have been using it for so long. But it s great that we did. One of the main uses that Dr Lycka likes it for is treating the dark circles under the eyes. A darker skinned person, of middle eastern descent, or with a darker complexion, they frequently have those dark circles under their eyes. Using PRP is one of the few ways to treat this and gets great results. It will usually take between 1 and 3 treatments and the dark circles disappear in 95% of his patients. Dr Crutchfield has seen the same thing after learning the technique from Dr Lycka. He does the treatment 1 per month for 2 to 4 months, and the nice thing is that there is a new topical medicine that numbs the skin that is put on it 10 minutes before and when the tiny injections of PRP around the eyes are performed, those circles of periocular hyperpigmentation that come with wisdom & maturity, then the injections are absolutely painless. Patients don t even feel it. It is a great treatment, it s painless and it works. Patients see results very quickly from this treatment. But that is not all PRP can be used for. PRP can be used for the fine lines and cracks around the mouth area. We get wonderful results with it there. Patients are so happy with their results. We can also rejuvenate the neck and upper chin with PRP injections. Years of sun exposure makes the area look freckled and wrinkly, especially when wearing a low cut top / swim suit / shirt. We can completely turn that around, again with a treatment once per month for 2 to 4 treatments. And we also use PRP to rejuvenate the backs of hands. That is such a concern for many patients who have had some great aesthetic procedures on their face and are looking great but whose hands are giving things away. So we inject PRP into the backs of their hands and it completely rejuvenates them. It is not perfect for everyone, but wow in the vast majority of patients that do it, they get a really nice result. Dr Lycka also uses an instrument that causes micro-needling of the skin, from EndyMed, so the device makes lots of very tiny holes in the skin, then we take the PRP liquid and rub it into the holes. This creates faster healing and absolutely phenomenal results, with skin that just looks gorgeous. Dr Crutchfield uses a procedure very similar to that. And treating acne scars with PRP is one of the best treatments with the best results not for the acne scars that are real deep like holes, but for the ones that are shallow. We do a treatment then follow it up with injections of saline for a couple of months and we can make a huge difference for those people who have struggled for years with those horrible acne scars. It s not perfect but it does look so much nicer. Dr Crutchfield says to his patients that he doesn t want perfection to become the enemy of very good. And let s talk about another one that Dr Crutchfield has told Dr Lycka about. Many people of color get a skin disease called melasma where they get hyperpigmentation of the skin and it is very difficult. It occurs on the upper lip, the cheeks area. It is also known as the mask of pregnancy. And many women get it when they are on birth control pills. Or others get it from sunlight exposure, or from genetic causes. It is one of the hardest things for us to treat. We have tried so many things and have been frustrated in that we get some response but not really great results. What have you found to be a useful treatment for Melasma, Dr Crutchfield? Dr Crutchfield agrees that it s been traditionally difficult to help patients with this, getting results that are fair at best. Dr Crutchfield describes melasma to his patients as a very rapid, uneven suntan that is a lifelong problem. There are certain cells in the skin that produce color, and when they see sunlight they immediately produce color to give the rapid uneven suntan. The color is really deep in the skin, and the topical medicines that we put onto the skin can t get down deep enough to fade the color away. BUT, one of the things that is being seen now is that when Platelet Rich Plasma is put into the skin, it activates enzymes to break up the color and is creating some remarkable results in areas where there were no results in the past. So for treating melasma, platelet rich plasma is fast becoming the go to treatment. Inside Cosmetic Surgery Today interview Dr Barry Lycka and Dr Charles Crutchfield, 06jun16 3

4 Dr Lycka has not read a paper on this yet. Are you writing one now, Charles? Dr Crutchfield is getting a series of results, and there have been several case results published. It is starting to get out there now and Dr Crutchfield actually has about half a dozen patients doing well with this treatment. But he takes the plasma, and does injections twice a month for 4 months, and combines that with meticulous sun protection because even no matter how great the treatments are, if you do not block Ultra Violet A radiation, the melasma is going to come back again. As long as the sunlight is blocked, then the PRP treatment is creating really great results. Dr Crutchfield is so excited, because as Dr Lycka mentioned, this is a vexing problem, with mediocre results achieved at best in the past with other treatments. It is such a common problem for so many people. Yes, and a very frustrating problem for both doctors and patients. It has been very unrewarding to have to say to patients, well come back in a year or so when there might be a better treatment for you because right now we don t have anything very useful for you. And as a doctor its tough to have to say that, BUT we have to be honest with patients, and that is a hallmark of Doctors For The Practice Of Safe & Ethical Aesthetic Medicine being honest with patients. There are some doctors who will take advantage of patients by doing things that they know don t work. It is important to be honest and upfront with patients but it s great that we now have something for treating melasma. We are still learning about this platelet rich plasma, about how to use it and we still don t know everything about it. Over the next few years we will certainly know more about the aspects that make this treatment work, and be able to better target its use, but even now we are doing good, for something that we didn t have a few years ago. The other thing that it is working great for (and we can talk about in another show) and that is hair loss particularly androgenetic hair loss, female and male pattern hair loss. Patients do 4 to 6 treatments, over a few months, and their hair is growing back. It is absolutely miraculous. So right now there are early studies but they are very promising. This will be such an exciting treatment because hair loss is also a problem that has been very difficult to treat. But with treating male and female pattern hair loss with PRP, we are seeing some early but very promising results. Dr Lycka has also has some response with treating alopecia areata. That is an autoimmune disease, where people are losing hair in localized areas. As doctors we have tried things to suppress the immune system with things like steroids, but Dr Lycka has found that PRP in a large number of cases takes away the alopecia areata and really makes a significant difference. Dr Crutchfield couldn t agree more. His son suffers from it and Dr Crutchfield treated it with PRP and his hair is now almost 95% regrown. It is yet another exciting, promising treatment area. Dr Lycka & Dr Crutchfield talk often, and talk about combining therapies. They also talk about taking a currently existing therapy out of the box to try to use it in a different area / way. That is where the true breakthroughs happen. That is why PRP started to be used in dermatology, from a history of being used in sports medicine. That is what is so exciting at the moment, that doctors are getting better and better at trying things in different ways, or combining them in different ways, and getting better and better results. Absolutely! One of the nicest things about Platelet Rich Plasma is that it is completely natural and safe. It is using your own growth factors and cytokines that you make in your own body, that we concentrate, activate and re-introduce into your body. Dr Crutchfield tells his patients that it s you helping you. It takes a little time to prepare, as the blood has to be extracted, spun down, and the patient spends time, but in reality, it has been a major breakthrough. And for most patients, from check-in to leaving it s an hour. Charles, are there any parting words or advice that you would like to share with us today?
