Promises and pitfalls of using smartphone apps to change behaviour Susan Michie Professor of Health Psychology Director of Centre for Behaviour Change University College Rosetrees Trust, October, 2015

My roles at UCL Director of the Health Psychology Research Group - We develop methods for understanding behaviour and helping people to change their behaviour Prevention Managing illness Healthcare professional behaviour

Director, UCL s Centre for Behaviour Change Bring academic disciplines together Translate expertise to Policy makers Practitioners Commercial sector The public Forthcoming events

Why is behaviour important? Good health & wellbeing A productive economy A happy & well functioning society Behaviour is key to nearly everything! Satisfying relationships at work and home A safe environment A sustainable environment Being the kind of person we want to be

Why behaviour change?

Behaviour is in the moment; at any one moment, there are many choices Shall I lie here, watch TV, drink wine, eat popcorn? Battle of impulses and inhibitions in the moment Put the popcorn out of reach? Put the TV off? Do what I said I d do go to the gym?

My plan is not to drink in the week I know I drink more than is good for me I want to wake up in the morning feeling fresh Plans Beliefs about things being good or bad Wants and needs I intend to go to the gym tonight I feel better when I have done exercise I need to avoid putting on any more weight Inhibitions Behaviour Impulses PRIME theory, West & Brown, 2013; Primetheory.com

Effective interventions Intervene at many levels simultaneously & consistently NICE Guidance for Behaviour change at population, community and individual levels (2007) Community-level Population-level Individual-level Update for Behaviour change: individual level (2014)

Can smartphone apps help?

Smartphone apps and health Half used their phones to look for health advice >100,000 mhealth apps

1000 s apps supporting behaviour change what is their potential? Smoke Free 28 Drink Less

1. Behaviour is in the moment and connected to places Apps are with you 24/7 where and when you need them Information, reminders, encouragement, support

2. You are in control Determine how you want your app to work for you, e.g. Set your goals Tailor your own support

3. We are often not aware of what influences our behaviour Apps can monitor what s going on and detect patterns between our behaviour and physiology & movement ( wearables ) other people ( Bluetooth ) the environment ( sensors ) harness the inside and outside world to support change

4. Everyone is different Apps can use machine learning to build up a picture of you and how you react to circumstances find out what would be useful to you when and how

BUT.. Potential pitfalls People get easily bored How to make apps engaging & rewarding enough to sustain use? It s only an app People and the world around us are usually more rewarding than technology

So much data. And how will it be used? How to make sense of big data? To learn about behaviour and develop more effective interventions Ethical questions of privacy and surveillance

Early days.. We don t know much Very few smartphone apps are evaluated Lack information on What is in them behaviour change techniques Whether they work - effects How they work mechanisms of action Popular is different than effective Crane, Garnett, Brown & Michie, JMIR, 2015

Conclusions Smartphone apps have huge potential to support behaviour change However, this potential will only be realised if 1. informed by the science of behaviour change 2. knowledge is accumulated using experimental designs and cross-disciplinary collaboration
