2 Diabetes Awareness Week Hounslow CCG supported the national initiative of Diabetes UK, alongside Hounslow and Richmond Community Healthcare NHS Trust (HRCH) to create more awareness about diabetes management and prevention in the London Borough of Hounslow. We conducted outreach which meant a desk with various information literature about diabetes, eating patterns, physical activity levels and family history was available. Patients and service users were signposted to local services available within the Borough to enable them to make better health changes. For those with no diabetes diagnosis, HRCH undertook health checks height, weight, Body Mass Index (BMI), waist, blood pressure (BP) to calculate their Diabetes Risk Score (DRS). The score helps people to know the current status of their health and start thinking about making small lifestyle changes. Key People: Adeola Adeleke (Diabetes Engagement Officer, Hounslow CCG) Adeola is working with patients, community groups and the wider Hounslow population to increase awareness of diabetes, in particular the need to attend for regular checks (Adeola.Adeleke@nhs.net) Payal Halwai (Health Trainer Coordinator) Payal plans and manages the health Trainer service she organises outreaches and events to deliver the Hounslow Health Promotion programmes. She raises awareness about health issues, prevention and management by promoting local health and wellbeing services. (Payal.Halwai@HRCH.NHS.UK) Asha Bawa (Diabetes UK Volunteer) Asha is an active volunteer who holds information desks in amongst different locations within the borough and gives advice on diabetes literature. communities about diabetes. She is trained to carry out risk scores and she is also specifically trained to give talks about fasting during Ramadan within the Muslim community.

Achla Thakore (HRCH Health Trainer) Achla uses motivational health coaching to support people on a one-to-one basis to make small lifestyle and behaviour changes to improve their health. Kalpana Lamichhane (Health Champion) Kalpana is a trained Health Champion who is a project coordinator at CRI London Gurkha Settlement Service Why is diabetes a priority in Hounslow? We have approximately 15,000 residents in Hounslow diagnosed with diabetes. We estimate that nearly 5,000 people may have the condition but don t yet know it.

4 Fewer than half of our patients with diabetes receive all of their annual checks recommended by NICE (National Institute of Clinical Excellence). The statistics produced by the National Diabetes Audit on how well we deliver diabetes care show that Hounslow is in the bottom 25 per cent of CCGs in England. What have been the main challenges in addressing diabetes in Hounslow? Nearly 50 per cent of the Hounslow population is South Asian, Black African, Black Caribbean background, and these groups who have an increased risk of developing diabetes There are some language barriers with residents who speak English as a Second Language (ESOL) There are many people who don t know they have diabetes and often present secondary conditions or diabetic complications i.e. non diabetic conditions that are either the result of, further complicated by, undiagnosed diabetes. People are not offered annual checks, and many do not attend when they are requested to do so. We have a transient population. Many residents live three or four months of the year abroad. The patient was referred to the X-pert programme One patient was happy with the diabetes services but wanted information regarding local exercise programmes. The patient was signposted to the Healthy Walks programme in Hounslow. One patient spoke of their achievement in attending all essential Annual Checks. Explained it was important for them to attend every year to ensure that they were on target and were aware of any changes regarding their diabetes.

It was stunning to see what awareness people had in terms of healthy eating, physical activity, the services available to support them manage diabetes better. Our interactions resulted in a lot of learning for both us and the people. Empowering them with the available resources and local health and wellbeing services felt very rewarding. Adeola Adeleke - Diabetes Patient Engagement Officer It was really important that we raised awareness within the community. patients, community groups and carers are key to improving the level of diabetes care in Hounslow, and we wanted to encourage people to gather more information and to be made aware of all the services available to better manage the long term condition. We were pleased to be able to use this as a platform to gather patients feedback on the diabetes services in Hounslow as well as an opportunity to encourage

7 people to get involved in becoming a Diabetes Champion or joining the Diabetes Patient Reference Group to help spread the word, particularly in some of seldom-heard communities to encourage early diagnosis and promote the safe management of the condition. Kalpana Lamichhane - Health Champion As a Hounslow Health champion, one of the things I really wanted to work on with the Nepalese community was raising awareness of diabetes and the things one can do to prevent it or delay it. I got a wonderful opportunity to do just that by participating in a diabetes awareness raising session at Age UK Hounslow with the Diabetes Engagement Team. Amongst those who attended, we were able to assess more than 18 Nepalese people, who were either diabetic already or who were dangerously close to being diabetic, based on their family history and their current health and diet/lifestyle answers. It was a wonderful opportunity to engage with the local community and talk to them about the effects of diabetes and how it is rising, especially in Asian and Black ethnic groups. We were able to talk to them about the little changes they can make in their diet and/or daily exercise, which can prevent or delay diabetes. Many of those who attended were surprised at the recommended meal portion sizes and realised they were putting far too much on their plate! At the end of the session, I feel like we really made a difference as many pledged to increase their exercise and be more mindful of their portion size and the kind of food they re eating.
