4 DEFINITION A professional Geriatric Care Manager (GCM) is a health and human services specialist who helps families who are caring for older relatives. The GCM is trained and experienced in any of several fields related to care management, including nursing, gerontology, social work, or psychology. NAPGCM (2011)

13 Changes with Age Age-related problems are not necessarily limited to graying hairs. Baby Boomers are known to be less healthy than the previous generation. Often this creates unique needs for care for both generations.

17 Red Flag: Concealed Medications Threats Over the Counter Pain Medications Often considered harmless this group of medications can cause serious injury or even death. Products like Tylenol, contain acetaminophen, and can cause toxicity and liver damage especially when used while drinking alcohol-even as little as 3 drinks a day! Other common pain killers, like Advil, contain ibuprofen, these can also be toxic, causing bleeding ulcers and may also effect normal kidney function.

18 Red Flag: Diabetes While high blood sugar is a serious health issue, low blood sugar that results from too much medication or too little food can pose a serious problem that may lead to seizures, falls even coma and death.

19 Red Flag: Meds to Improve Behavior & Mood This group of medications can have serious toxic effects, especially when used together. They also are a major reason why persons over 65 years of age may fall. Falls lead to serious injuries, like fractured hips and even death.

20 Red Flag: Infections in Older Adults Physical changes in aging can affect how older adults respond to infection. Unusual or unexpected symptoms, like confusion, may delayed diagnosis and lead to complications. Knowing these symptoms and getting care early can improve outcomes.
