10 SCP in Canada Research settings First RCT testing SCP (Grunfeld et al - breast cancer) Results: Trial did not support the hypothesis that SCPs are beneficial for improving PROs Critique: Timing of intervention Population Usual care not standard in most jurisdictions (discharge visit with oncologist and discharge letter sent to GP) Pilots across the province: FV, CN, VIC

12 Survivorship Initiatives Vancouver Island Post treatment package for people with colorectal cancer Post treatment package/intervention received feedback from HCP and people with cancer At the end of treatment Nurse creates the treatment summary, conducts an exit meeting, follow up calls (W1+W3) How do the results connect to practice consistencies and surprises Challenges

13 Next steps: Sustainable SCPs (FVC): Treatment summary template is provided to patients at the beginning of trajectory HCP helps patient at transition points to complete the summary Patients needs/knowledge are assessed at transition points and patient is referred to the appropriate resources if necessary
