CONDITION: High triglycerides, low HDL, T2DM, secondary cardiovascular prevention DESCRIPTION: Placebo controlled trial of a potent fibrate, pemafibrate, to prevent MI, ischemic stroke, unstable angina requiring revascularization, and CV death in adults with T2DM. 14 Survival at 1,3,5 years: Mini 98,97,95% Conv 97,91,86% Neurologic events Risk of stoke 2.1 vs. 1.2 % (95% CI , RR 1.79) Subanalysis 3 fold increase without aortic occlusion Grossi et al Retrograde perfusion in elderly, PVD, and EF <30%, and no aortic occlusion all increased risk of stroke (each with P <0.05) Murphy et al CT screening in robot MVS 1.6% stroke 14 of 23
