As per approval monthly contribution for availing Medical Facilities will be deducted from the salary of the employees as per followings: Sl No Grade Pay Drawn by Employees Contribution (Rs. All Central Govt. s 2. All Govt. s of the State of Uttarakhand. 3. All Medical Colleges of the Govt. of Uttarakhand. 4. All such private s as approved by the Central Govt./State Govt. The application form for applying reimbursement for indoor and outdoor treatment and essentiality certificates are annexed. The applicability of scheme, treatment of Diseases, treatment in packages, processing of reimbursement, time limit for applying reimbursement is also annexed for reference and forwarding the bill to concern section. All the employees have been advised to submit the dependent list. University is in process of prepairing Health Card for employees. In the mean time, employees can apply for reimbursement for self, spouse and dependent children. In case both husband and wife are central Govt servants, the spouse drawing higher pay will contribute to the scheme. Parents of non-contributing employee will not be eligible for the scheme s benefits For reimbursement of clinical investigation bill, a copy of investigation report along with advice for investigation be submitted.
