To make an appointment with Dr. Marzouka-Losito, all (786) DIABETES: WIDESPREAD DISEASE IS OFTEN PREVENTABLE By St. Anthony Amofah, MD, MBA, CHCQM, FACP Diabetes is a growing disease that is largely preventable, but affets more than 25 million people in the United States and is the seventh leading ause of death. It an ause kidney failure, lower limb issues that lead to amputation, adult blindness, heart disease and stroke. It often runs in the family but many vitims fail to reognize symptoms. Roughly seven million diabetis are left untreated due to finanial reasons. Diabetes typially begins with high levels of blood sugar, or gluose, when the body is unable to produe insulin or use it properly to turn food into energy. Vitims often experiene extreme symptoms of fatigue, hunger and thirst. Some have unexplained weight loss and frequent urination. Type 1 diabetes typially is found in hildren and young adults, but an our at any age. The ause? The panreas produes too little or no insulin. The solution? Computerized pumps that send insulin into the body through a atheter inserted under the abdomen. Today s pumps are no larger than a small ell phone. Many forget they are using them. Type 2 diabetes most often ours in adults. The panreas produes insulin but the body doesn t use it effetively. It is more ommon in Afrian Amerians, Latinos, Native Amerians, Asian Amerians, Native Hawaiians and other Paifi Islanders. Adults over 45 are at greater risk, as are people who are overweight, or get little exerise, or have a family history of diabetes. Type 2 diabetis an manage the early stages by exerising, eating more low fat foods and pushing bak from the table. Some people exhibit pre-diabetes symptoms suh as blood sugar at levels just a little higher than normal. They stand a better hane of preventing Type 2 diabetes if they walk every day, eat sensibly and otherwise ahieve and maintain a healthy weight. Those are things we should do anyway. And these days, people have plenty of options to maintain that healthier lifestyle. 5 JOINT COMMISSION PCMH VISIT For several months now, Care Team Members of Community Health of South Florida have worked under the leadership of Dr. Anthony Amofah and Ms. Mae Goins to build and reate a patient entered medial home (PCMH) model of are. On Marh 12th surveyors from the Joint Commission arrived at CHI for a surprise two day visit. And THE DENTAL MINUTE Information ourtesy of the Aademy of General Dentistry Mouth Sores? What does this mean? How to minimize outbreaks? What are Cold Sores? Cold sores, also known as fever blisters, are tiny, lear, fluid-filled blisters that form around the mouth and are aused by the herpes simplex virus (usually type 1, or HSV-1) living inside your nerve tissue. Cold sores usually do not last longer than two weeks. However, the sores are highly ontagious and tend to reur when the virus is reativated by a trigger suh as stress, sunlight, fever or illness. What happens when exposed to HSV-1? Most people get HSV-1 infetions during infany or hildhood and usually ath the virus from an infeted family member or friend. Only an estimated 30 perent of those infeted atually develop the harateristi blisters. If sores do develop, they an appear anywhere from two to 12 days after exposure to an infeted person. Other persons with a primary infetion may have flu-like symptoms suh as a high fever, sore throat, swollen nek glands and mouth soreness. Cold sore vs. Canker sore Like old sores, anker sores, also known as aphthous ulers, an be quite painful and often reur. While old sores are aused by a virus, the formation of a anker sore may be triggered by multiple fators suh as stress, food allergies or a weakened immune system. A anker sore forms in the soft tissues of your mouth and is not ontagious, whereas a old sore usually appears around the lip area outside the mouth and is highly ontagious. If a old sore appears inside the mouth, usually it appears on the non-movable parts suh as the roof, rather than the tongue or soft palate. And guess what - All the hard work paid off! The survey overed 52 PCMH - speifi requirements through five operational harateristis and thanks to staff and leadership, CHI passed the site visit with no major findings! True There may be long periods when the herpes virus remains inative but an easily be triggered by everyday ativity. To prevent transmission of the virus to another person, avoid: Intimate physial ontat with others Touhing the blisters (always wash your hands after applying mediation) Sharing utensils, toothbrushes, and razors How to minimize reurrent outbreaks? Replae your toothbrush True or False Engage in relaxing ativities to redue stress Apply sunsreen to fae and lips before going outdoors

6 2012 LONG TERM CARE NATIONAL PATIENT SAFETY GOALS The purpose of the National Patient Safety Goals is to improve patient safety. The goals fous on problems in health are safety and how to solve them. IDENTIFY RESIDENTS CORRECTLY NPSG Use at least two ways to identify patients. For example, use the patient s name and date of birth. This is done to make sure that eah patient gets the orret mediine and treatment. USE MEDICINES SAFELY NPSG Take extra are with patients who take mediines to thin their blood. NPSG Reord and pass along orret information about a resident s mediines. Find out what mediines the resident is taking. Compare those mediines to new mediines given to the resident. Make sure the resident knows whih mediines to take when they are at home. Tell the resident it is important to bring their up-to-date list of mediines every time they visit a dotor. PREVENT INFECTION NPSG Use the hand leaning guidelines from the Centers for Disease Control and Prevention or the World Health Organization. Set goals for improving hand leaning. Use the goals to improve hand leaning. NPSG Use proven guidelines to prevent infetion of the blood from entral lines. PREVENT RESIDENTS FROM FALLING NPSG Find out whih residents are most likely to fall. For example, is the resident taking any mediines that might make them weak, dizzy or sleepy? Take ation to prevent falls for these residents. NPSG Use proven guidelines to prevent infetion of the blood from entral lines. PREVENT BED SORES NPSG Find out whih residents are most likely to have bed sores. Take ation to prevent bed sores in these patients. From time to time, re-hek residents for bed sores.
