1 Concurrent Sessions Morning Session PLEASE SELECT FIRST AND SECOND CHOICE Session 1 - Innovation in Undergraduate Professional Preparation Promoting Excellence in Pre Registration Nursing Programmes at the University of the West of Scotland Margaret Brown University of the West of Scotland This presentation details how the aspiration of the Promoting Excellence Knowledge and Skills Framework has been integrated into the largest centre for pre-registration nurse education in Scotland. The session will describe a new pre-registration programme UWS have produced, for both adult and mental health fields, that integrates the Dementia Strategy, Promoting Excellence Framework and the Standards of Care for Dementia. The result is a curriculum that has been mapped to key policy documents and reflects the person-centred, rights based approach espoused by these. The presentation will outline a range of innovative methods being used to promote students knowledge and skills in working with people with dementia including the Domus initiative, the virtual dementia tour and work with local schools. The University is committed to ensuring all nursing students reaching a minimum of the Dementia Skilled level of Promoting Excellence on graduation. The presentation will also outline how opportunities are offered to students in the mental health field to achieve Enhanced knowledge and skills through individualised learning plans in their third year. Session 2 Promoting Excellence in E- Learning about Dementia 1. Dangling conversations: an e-learning resource Rosie Stenhouse Robert Boyd Stefan Koehn University of Abertay / University of the West of Scotland This presentation outlines the development and evaluation of digital stories. The digital story making process is co-productive. Each participant worked with a workshop facilitator to develop their story using personal photographs as prompts. The focus and content of each story is different; many people talk about their working and social lives, family, and sport as well as the impact of their dementia. The stories were screened in a small cinema at the university attended by the participants, family/friends, health and social care workers. Following each story, the audience were asked to record what the story made them think, feel, and what they might do differently in their practice. A focus group of staff (n=8) also undertook this process. The data collected was used to develop reflective questions for each story. The reflective questions were put together with each story in an e-learning resource. 57 year-1 student nurses participated in an evaluation of the resource. Data were 1
2 collected using a short self-completion questionnaire to evaluate the digital stories as a pedagogical tool. Findings: Participants found the stories a useful and engaging tool for learning. 2. Promoting Excellence in cognitive assessment; the ACE III Trainer Stephanie Crawford University of Glasgow Accurate diagnosis of dementia requires accurate assessment of cognitive skills such as memory, language and perception. One instrument that is recommended by the Scottish Intercollegiate Guideline Network (SIGN) is the Addenbrookes Cognitive Examination, now in its third edition (ACE III). Although health care professionals such as nurses, doctors and psychologists are trained to administer tests, until now there was little in the way of training programmes for specific cognitive screening tests such as the ACE III. Commissioned by NHS Education for Scotland, an online training programme was developed aimed at improving the standard of administration of the ACE III. In this presentation the ACE III Trainer will be described. It includes video clips of a health professional administering the ACE-III, pre-diagnostic conversations with people, information on how to administer and score the test accurately, and how to give score feedback in the clinic. Video vignettes of case examples provide the opportunity for practice in live test scoring. Session 3 Promoting Excellence across Local Authority and Care Home Sectors 1. Changing the Status Quo- Putting Service Providers In The Know Mairead Rea Aberdeen City Council This presentation describes work under taken by 3 Social Work Occupational Therapists from Aberdeen City Council initially as part of the Dementia Champions programme. This has involved working in partnership with the training facilitators of the Social Care and Well Being Department of Aberdeen City Council from the Learning and Development team to improve the skills, knowledge and understanding of those working with people with dementia, their families and carers leading to improved Quality of Life outcomes. We used the four levels of Promoting Excellence Framework in response to the individuals role and responsibly in dementia care, treatment and support. Each level was discussed and a Dementia Champion was assigned a level to work with. Engagement in training is currently on going. 2. Generating Meaning Embedding Change Allan Logan Pamela Mackay Crossreach Crossreach is a third sector organisation working across Scotland. Crossreach s response to Promoting Excellence can be presented as two stages: 2
3 supporting staff to become interested in and to understand the meaning for them in Promoting Excellence s Quality of Life outcomes. addressing knowledge and skill gaps around areas such as the stigma that can accompany diagnosis and the need to retain a focus on capability as opposed to disability. Crossreach has developed two programmes of learning. Course 1 is a one day course based around the quality of life outcomes and the capability/skill/ability statements identified in Promoting Excellence Informed Practice Level. Course 2 is designed around the same principles, but is a two day course. Both courses are open to all staff within residential units and also to informal carers. This session will outline the development and impact of these programmes and demonstrate the contribution of Kolb s learning styles model (1984), Scottish Dementia Working Group s DVD Through our Eyes, an organisational requirement to complete follow-up practice application, mapping of the course to the PDA Promoting Excellence in Dementia Skilled Practice and finally Crossreach s Dementia Ambassadors. Session 4 Strategic Approaches to Promoting Excellence Across Organisations 1. Promoting Excellence Improving Dementia Care Andrew Swanson and Janice McAlsitar NHS Ayrshire and Arran* This presentation gives an overview of the support and training offered to colleagues within Acute Hospitals, Local Authority & Private sector. Improving care and support for people with dementia is not only a priority for the Scottish Government but also NHS Ayrshire and Arran and as such, developed a training programme based on "Promoting Excellence. A 4 tier training programme was developed and delivered by experienced multi disciplinary staff which addresses the knowledge and skills required at each of the four levels within the framework. The approach ensures staff have a clear understanding of patient centred approaches to care for people with dementia. This initiative has at it s core, the vision of Rights, Relationship & Recovery in breaking down traditional organisational/professional boundaries with the aim of driving forward service improvement for people with dementia that is person centred and strengths based. It has brought about partnership working with a common goal of positive change in practice in dementia care. To date 2,500 staff have undergone training. 2. Alzheimer Scotland Staff Training - Promoting Excellence Framework Anne-Marie MacDonald Alzheimer Scotland Alzheimer Scotland is at the forefront of dementia care. Following the introduction of the Promoting Excellence Framework in 2011, the challenge we embraced was to ensure that our workforce were not only knowledgeable and skilled in the care and support of people with dementia and their families but that they also they understood the requirements of their 3
4 role according to the framework. Our new training package was rolled out in April This includes the requirement that every member of staff /volunteer is given the Informed About Dementia DVD, ensuring that they have the basics before joining the organisation. Our already extensive Induction course was revamped. Our 600 plus Support Staff are well on their way through the seven-day mandatory training courses, and on-line modules. On completion they will be presented with our in-house certificate stating that they are at Skilled Level. Our 300 plus Enhanced Level Staff are embarking on their new courses, which also include a range of personal reading materials. Alzheimer Scotland staff have fully embraced the Promoting Excellence Framework - it gives a common language / shared ethos when partnership working. A common theme from the evaluations is that courses are specific to worker s roles in the support they provide to people with dementia and their families. Course content will be discussed in detail. 4
5 Concurrent Sessions Afternoon Sessions PLEASE SELECT FIRST AND SECOND CHOICE Session 5 Promoting Excellence in Working with People with Dementia Experiencing Stress and Distress Sustainably implementing national training to meet local needs. Leigh Whitnall and Susan Ross NHS Lanarkshire In response to Scotland s National Dementia Strategy, and the subsequent Promoting Excellence Framework and the Dementia Standards, a large scale training initiative has aimed to increase the knowledge and skills of NHS Lanarkshire and Local Authority staff in how to prevent, assess and respond to distress in dementia for people with dementia, and their families and carers. To date a formal 2-day training model developed by NHS Education for Scotland (NES) has been delivered to 186 staff working at the enhanced/expertise levels across Lanarkshire by local staff. In conjunction with GP awareness raising and medication reviews, positive outcomes have been evidenced including a reduction in antipsychotic prescribing, improved person-centred care, and reduced multiple placement changes and re-admission to hospital. This session will describe how the strategy was developed, progress to date and proposed ongoing work to ensure sustainability. The major issues enabling this large scale multi-agency training are discussed, including identified barriers to change and possible solutions. Session 6 Using the Supporting Change Process to Promote Excellence in Dementia Care 1. Enhancing dementia care in the Highlands Caroline Parr, Kerrie MacLean and Dan Fraser NHS Highland Scotland s National Dementia Strategy highlights a number of new commitments including "to set out plans for extending the work on quality of care in general hospitals to other hospitals and NHS settings. This session describes the work currently being undertaken in NHS Highland with staff from two different community hospitals in Badenoch and Strathspey who are piloting the Supporting Change Programme developed by NES. Both hospitals contain GP beds and one of the hospitals also contains a dementia assessment unit. The Dementia Standards Supporting Change Programme was designed to support staff and services to implement the Quality of Life Indicators incorporated within Promoting Excellence. This has been utilised to focus on two of the Quality of Life Indicators. The presentation will outline work undertaken to date involving general and mental health nursing staff, sharing expertise across both hospitals. 5
6 2. Promoting Excellence in Care Homes Jackie Sloan, Edinburgh City Council and Rene Rigby, Scottish care This session describes a partnership between the City of Edinburgh s Planning and Commissioning and Workforce Planning and Development teams and representatives from NHS Lothian and Scottish Care. The project involves working with all 60 care homes in Edinburgh to evaluate their services and expertise with respect to dementia care and support. While in its early stages of development, the process will begin with pilots using the Supporting Change tool in 2 care homes. This is a major undertaking which we envisage will take up to 18 months including: assessing services using the Supporting Change tool; providing the training using the Informed about Dementia and Skilled level resources; advising services of resources at Enhanced and Expert level; promoting the role of Dementia Ambassadors and access to the Dementia Managed Knowledge network; and evaluating changes in the quality of care and support, and staff skills, knowledge and practice, again using the Supporting Change tool and other evaluation methods. Session 7 - Innovations in Supporting Change and Palliative Care 1. Supporting Change within Ward 3, Wishaw General Hospital Lee Ann Reily, Angela Campbell and Karen Spiers NHS Lanarkshire The Supporting Change process aims to help staff change and improve services and practice for people with dementia, their families and carers. The programme uses the Standards of Care for Dementia in Scotland and quality of life indicators from Promoting Excellence. The principles of the programme are based on an appreciative inquiry approach to change. A Plan Do Study Act (PDSA) approach was used to apply the learning and improvement methodology to just one acute admission ward for people with a diagnosis of dementia, Ward 3 Wishaw General Hospital. The ward had also completed a self assessment of their service against the Dementia Standards and had recently completed their SRI2 action plan. We reviewed both the self assessment tool and the SRI2 action plan and identified areas of practice that were established and areas requiring development. 2. Palliative and End of Life Care in Dementia Angela Campbell and Marie Doyle, NHS Lanarkshire This session delivers an overview of the Palliative and End of Life Care in Dementia training and education programme delivered to staff across health, local authority and the independent sector within Lanarkshire. The aim of which is to support and develop best practice in this aspect of dementia care. The session will demonstrate how this training initiative, developed in line with the knowledge and skills of the Promoting Excellence framework, enables meaningful partnership working between education providers, service partners, people with dementia, their families and carers. The aim is to encourage staff, families and people with dementia to have an equal partnership to support change and improve services and the care. The training has been facilitated and delivered through 6
7 partnership working with groups of healthcare, social care, care home staff and carers learning together and sharing experiences. The implementation of this training is ongoing throughout Lanarkshire Care Homes and a proposal is being developed to disseminate the training across older adult mental health in-patient and community mental health services. Session 8 Dementia Champions Promoting Excellence 1. Aspiring to Excellence: Dementia Champions Barbara Sharp Anna Waugh Alzheimer Scotland University of the West of Scotland This session will describe the Dementia Champions programme, designed jointly by the University of West of Scotland (UWS) and Alzheimer Scotland. The programme is based on an 'Appreciative Inquiry' approach; building on existing good practice in a strengths-based manner. This model nurtures a positive attitude to learning and practice development, mirroring the expected response of champions to their caring relationships. A 'Supporting Change' tool developed by the training team specifically for the programme aligns closely with the Promoting Excellence framework and National Dementia Standards. Quality of Life Quality of Life indicators facilitate practice self-assessment and action planning by participants with their own teams. The presentation will provide a positive example of what can be achieved when educational and practice expertise, driven by shared values in dementia care, are brought together in a spirit of co-operation. Evaluative outcomes will be shared to illustrate the impact on participants and illustrative examples will reflect changes that dementia champions are making in their areas of practice. 2. The Dementia Box an example of the work of one dementia champion Lucinda Gorrie NHS Fife This session outlines the work of one dementia champion in designing an educational activity for staff working in a busy accident and emergency department to appreciate what it is like for a person with dementia when they are a patient in the department. Accident and emergency departments are noisy, bustling, stressful environments, so for a person with dementia this can cause increased stress and distress. To educate staff about dementia one option was to deliver a formal teaching session. However this was felt not to be the best way to engage staff to ensure effective learning. A novel approach was selected through the introduction of the dementia box. The dementia box is primarily a tool to help professionals empathise and understand the experiences of someone with dementia, promoting their personal development and thus improving clinical practice in their area. An unusual approach to learning has had a hugely powerful effect and resulted in staff being more aware of the needs of people with dementia, and hence improved the care of people with dementia in the accident and emergency department. Workshop participants will have an opportunity to experience the dementia box learning. 7
