4 Smartcare in Scotland aims.. to improve the health, care and wellbeing of 10,000 people aged 50+ within Ayrshire and Clyde Valley, by enabling a better coordinated and more effective approach to falls prevention and management focusing on the role that ICT services and applications can play in supporting integrated care

11 NHSL Community Falls Pathway continued General advice and info provided as per Falls guidance including Falls prevention leaflet Progress to Level 2 multi factorial assessment if indicated by Level 1 screening and it s within your scope or request Level 2 via register Send to Falls Register Team via secure or MiDIS (multi disciplinary information system)
12 Community Falls Pathway continued If consent recorded admin will add to register and check for any previous registrations/care manager or if Pt. Is on the case load of Health or SW staff. If known inform appropriate staff If Level 2/3 required sign post to relevant team for completion If information only indicated information pack will be sent out Future Plan Level 2 Assessment outcome added to register

Complete the following level 1 falls screening tool and send to the falls registry at the address below. * Please note that this is not a direct referral to the falls specialist service or falls medical clinic. BASIC SCREENING QUESTIONS 1. Have you fallen more than once in the last 6 months (NOT the result of simple accidental slips/ trips)? 2. Do you have any unsteadiness on your feet, or have difficulties with your walking or balance? OR Has the screener observed any unsteadiness or difficulties with the persons walking or balance? 3. Did you experience a blackout or any dizziness/ light- headedness when you fell? 4. Have you experienced any difficulties carrying out your usual activities since you fell? 5. Are you worried about falling again? 6. Were you unable to get up from the floor after you fell? If the answer is YES to any of the above questions in the shaded boxes, further assessment is required and a level 2 falls assessment needs to be completed. Please ensure the person is aware of this and is agreeable to further input.
