3 Standard Emergency Reporting Procedure To report an emergency: In the event of an emergency, dial for Security. Security is available 24 hours a day, 7 days a week Security Officers have been trained to dispatch the appropriate personnel: 911 EHS department, And Facilities (if necessary)
4 Standard Emergency Reporting Procedure Provide the following information to the operator/security officer: Your name and the extension that you are calling from Location of the emergency If you are unsure where you are, note the closest conference room or cubicle number. Nature of emergency Injured employee Chemical spill, etc. Answer all questions, asked by the Security Officer to the best of your ability.
5 Standard Emergency Reporting Procedure You can call 911 directly, however please remember the following when doing so: Dial Inform 911 operator, to have the emergency vehicles meet the Security Officers in the front of the lobby. Place a follow up call to the security desk (64700). Security will dispatch the EHS department and facilities (if needed). Security officers will direct the incoming local emergency vehicles to the location of the emergency.
6 Standard Emergency Response Procedures Following your call to Security, appoint someone to retrieve the blue flag next to the nearest First Aid Kit. All First Aid Kits in the building have been equipped with blue flag attached to the side. The purpose of the flag is to alert emergency personnel or the EHS department as to the location of the emergency. Hold the flag above the cubicles, so it is easily seen in all directions. Do not remove it until the emergency personnel are on scene.
7 Standard Emergency Response Procedures Employee Calls Security Grabs blue flag and holds up over emergency location. Security Calls 911, EHS, and Facilities (if needed) Strategically places officers w/in parking lot to direct emergency responders Officer(s) report to emergency location and set up as crowd control. EHS Calls ERT member EHS and ERT member report to emergency location and provide medical attention
8 Emergency Response Team (ERT) Volunteers who have committed to under-go multiple CPR/AED/First aid trainings in order to assist EHS in responding to 911 calls. Red Flags identify each members cubicle. An ERT member located near the emergency location, will be notified to respond to the emergency to start assessing the situation and if needed to start First Aid/CPR. EHS will assist the ERT member once they reach the emergency location. For a list of the ERT members, go to the village, under the EHS website, locate the Evacuation page.
9 Specific Emergency Procedures
10 Medical or First Aid Incidents In the event a co-worker becomes ill or is injured: Assess the situation first. If the illness or injury is serious: Immediately call for Security to report the emergency Make co-worker as comfortable as possible Do not give them food or water Do not move them Crowd control limit the number of people in the area Find out if co-worker takes any medications or has a preexisting condition Less serious injury or illness Notify supervisor/manager and the EHS department
11 EHS Recommendation If you have a medical condition (e.g. diabetic, food allergies', etc.) Develop a buddy system Train your buddy on the following: What is the condition? What are the symptoms of the condition? Where do you keep your medicine(s)? How do you administer your medication? Provide contact information of a family member
12 Fire or Explosion Only use a fire extinguisher if you are trained on how to use and handle an extinguisher and the fire is relatively small (no bigger than a waste basket), or if you need to clear a path to exit the area. Otherwise, immediately evacuate the area, closing doors behind you as you leave and informing your co-workers to evacuate. Dial to start the standard emergency reporting procedures.
13 Fire Doors Fire doors serve as a barrier to limit the spread of fire and restrict the movement of smoke. The Medtronic Diabetes facility is equipped with fire doors throughout the facility. The fire doors at Medtronic are connected to the building alarm system, meaning they will automatically close when the alarms sound. During an evacuation, the doors will close, however employees can still proceed through the doors to exit the building. Just remember the following: Before opening any doors, feel the door first: If it is hot, do not open the door use an alternate exit. If it isn t hot, brace yourself against the door, open it slightly, and if heat or heavy smoke is present close the door and use an alternate exit. Always close doors behind you as you leave, informing co-workers to evacuate.
14 Chemical Spill Chemical Spill Immediately call EHS or dial to start the standard emergency reporting procedures. Only trained employees are permitted to assist in clean-up procedures. If trained proceed as trained. If not trained: Evacuate the immediate area and wait for EHS to evaluate the situation.
15 Bomb Threat, Criminal or Violent Behavior Bomb Threat Immediately call Security, EHS and Facilities Person receiving call: Remain calm and ask as many questions as possible: When is the bomb going to explode? Where is it located? Keep caller on the phone as long as possible and record as much information as possible Date and time of call Age and sex of caller Speech pattern and/or accent Criminal or Violent Behavior Immediately call Security, EHS and/or your supervisor/manager.
16 Earthquake During an earthquake while at work: Duck, cover and hold under a desk or sturdy table. Shields employees from falling objects (e.g. ceiling tiles, light fixtures, books, shattered glass, etc.) If desk moves, move with it and stay covered. If a desk is not available, move against an interior wall, and protect your head with your arms. Stay away from windows, file cabinets, and heavy objects that could fall. If you are in a lab, take shelter where you can, and immediately following the earthquake move to a safe location outside of the lab.
17 Earthquake After an earthquake: Remain indoors. Employees are safer inside the building. Running outside raises the risk of being injured from falling debris (e.g. shattered windows, building facade and other debris) Do not use elevators, always use the stairs Immediately following an Earthquake: Facilities, EHS and Security will conduct building sweeps As soon as a hazard is found, the building alarms will be sounded to start an evacuation.
18 EHS Recommendation California has several active faults, so to prepare for an earthquake: Create an earthquake kit for your car and/or desk. Medtronic does not have the room or resources to maintain supplies for all employees. Attend training provided by LAFD on how to prepare for a disaster: Disaster Preparedness Training or Community Emergency Response Training (CERT) For more detailed information regarding the various emergencies' consult the Emergency Evacuation Procedures located on the Village under the EHS website.

20 Emergency Evacuation Plan and Procedures In the event an emergency requires a building to be evacuated, you will be notified by the following signals: Emergency alarms will sound Emergency red lights will flash Emergency overhead paging announcement will be made (if possible) In the event we have a power outage, you will be notified via word of mouth.
21 Emergency Evacuation Plan and Procedures Once an evacuation is initiated: Only take items that you brought in with you that day. Purse or Briefcase Car keys This only applies if you are at your desk when the evacuation is initiated. After grabbing your personal items, exit the building through the nearest exit and report to your assigned staging area pole and People Manager.
22 Emergency Evacuation Plan and Procedures If you have any medical conditions, such as being pregnant or you are on crutches, inform your Manager. Individuals with special medical needs will be provided with a blue flag which will identify them as needing accommodations such as: Transportation to alternate staging areas (i.e. CSUN s football field) During an evacuation you will not be able to drive in the parking lot. All entrances and exits will be secured to prevent vehicles from entering or leaving the parking lot. Only emergency vehicles will be allowed to enter the parking lot.
23 Emergency Evacuation Plan and Procedures Staging Area Poles: There are 8 light poles in the parking lot, 4 in the east and 4 in the west, that have been designated as staging areas. These poles are painted with the color they represent and are equipped with a sign indicating the color of the pole. Each light pole contains a box that is equipped with orange vests for the Managers.
24 Emergency Evacuation Plan and Procedures Managers will don a bright orange vest to ensure they are easily visible to all employees. You are responsible for reporting into your Manager during an evacuation. You are responsible for knowing where your Manager will be located in an evacuation. Each Manager has designated a staging area location around the designated staging area pole.
25 Emergency Evacuation Plan and Procedures Following taking roll, the Managers will send their head counts to the Facility Team Leaders. The Facility Team Leaders are responsible for reviewing the head counts to ensure all Managers have completed their head count. In the event a Manager or his/her alternate fails to turn in their sheet, a runner will be sent to assist in completing the head count for that staging area.
26 Emergency Evacuation Plan and Procedures You are expected to remain with your Managers unless instructed otherwise by the Crowd Control personnel. Crowd control personnel are responsible for relaying information from the command center to all employees in regards to the status of the evacuation. They will do this with the use of a megaphone. The Command Center will consist of the head of EHS and/or the head of Facilities, and the local emergency agencies.
27 Emergency Evacuation Plan and Procedures Staging area information can be found on the Village. Simply scroll to the bottom of the Village home page and click on your evacuation location. It will open the Cost Center vs. Staging Area Poles spreadsheet. Locate your cost center number to see your staging area location

29 Global Employee Hotline In the event there is a city wide emergency, such as an earthquake, and you are unsure whether or not to report to work, you can call the following number, 24 hours a day / 365 days a year Or toll-free
30 Injury and Illness Prevention Program (IIPP) The purpose of the IIPP is to ensure that Medtronic Diabetes provides and maintains a safe work environment for all our employees. Cal/OSHA requires every employer to, at a minimum, establish, implement and maintain an effective Injury and Illness Prevention Program. There are 7 sections to the IIPP. The following slides briefly explain how we comply with each section.
31 Injury and Illness Prevention Program (IIPP) Designation of Responsibility: The head of the EHS Department has the responsibility for implementing the program. Department Managers and Supervisors are responsible for those whom they supervise. You are responsible for following all safety rules and procedures.
32 Injury and Illness Prevention Program (IIPP) Identification and Evaluation of Workplace Hazards The EHS department: Reviews regulations that impact our facility on an annual basis to ensure the facility is in compliance. Completes Job Hazard Analysis to identify potential hazards. See the Code of Safe Work Practices for information regarding the hazards associated with each job and measures employees can take to protect themselves. Conducts New Equipment, Chemical and Process Audits to ensure all safety and environmental items have been addressed.
33 Injury and Illness Prevention Program (IIPP) Accident Investigations All accidents, first aid incidents and near misses must be reported to the EHS department within 24 hours. Managers/Supervisors must ensure the injured employee receives immediate medical attention. Injured employees will be sent to the company s authorized medical clinic for treatment. All incidents will be investigated by the EHS department to determine actions necessary to prevent a re-occurrence.
34 Injury and Illness Prevention Program (IIPP) Inspection Procedure Managers/Supervisors are responsible for conducting periodic inspections of their areas to ensure there are no hazards. Frequency of inspections depends on the risks associated with each job. For instance, office employees have minimal risks, therefore their inspections occur on a semi-annual basis. Monthly building inspections are conducted by the EHS department. Safety Committee/Employee inspections are conducted every other month.
35 Injury and Illness Prevention Program (IIPP) Communication of Health & Safety Info to Employees Managers/Supervisors are responsible for holding periodic safety meetings with their employees. EHS department supplies each department with a safety topic to cover during their meetings. Frequency of the meetings depend on the risks associated with each job. For instance, manufacturing employees have higher risks, therefore their meetings occur on a monthly basis. The EHS department also posts information regarding EHS on the Village, (Medtronic Diabetes intranet website) as well as sends out EHS Bundle s.
36 Injury and Illness Prevention Program (IIPP) Safety & Health Training Program The EHS department is responsible for conducting job specific safety training as required per job function. Specific safety training will be assigned to employees based on their cost center and job title through SABA. Manufacturing employees will receive in-class instruction. Managers/Supervisors are required to review the EHS training matrix for their cost centers to ensure each job title is receiving the appropriate safety training. The EHS department also provides additional special trainings, such as First Aid/CPR/AED, to all employees interested.
37 Injury and Illness Prevention Program (IIPP) Method for Ensuring Compliance with the Health and Safety Program You are responsible for complying with the health and safety program. To ensure we are in compliance, we will follow Human Resources disciplinary procedures.
38 Health & Wellness Medtronic Diabetes offers an array of Health and Wellness events. Events are categorized in the following manner: Rideshare Brown Bags - seminars that are focused on ridesharing topics such as: Defensive Driving Meditative Walking Bicycle Safety EAP Brown Bags - seminars that are focused on lifestyle and health such as: Stress Reduction Dressing for Success on a Budget Financial Planning for the Future Health & Wellness events that are focused on employee health and well being Blood Drives Flyers with event information are ed out to all employees in the EHS bundle s.
39 Security Information Universal Protection Services (UPS) is our security provider. Officers are onsite 24 hours a day. Security can be reached at ext Security provides the following services: Escort employees after hours upon request. Assist in handling emergencies. Print new hire badges and badge reprints.
40 Badge Policy Medtronic Diabetes has a badge policy that states the following: You must visibly wear your employee badge at all times. If you forget your badge, you will have to obtain a temporary paper badge from the front lobby officer and enter the building through the front lobby for that day. If you loose or break your badge you can request a replacement badge by submitting a badge access request form to the EHS department. The form is located on the village on the EHS webpage. All perimeter doors are locked from 6:00 PM to 5:00 AM and on weekends. If you are working after 6:00 pm and step out of the building, you will have to come in through the front lobby to gain entry back into the building.
41 Parking Enforcement The map provides areas employees can park in. GREEN AREAS = Medtronic Parking Please park in an unmarked or Visitor space Please Note: Handicap Spaces and Fire lanes are enforced by parking enforcement by the City of Los Angeles. Violations in these areas have monetary penalties. RED AREAS = Non-Medtronic Parking City or State Enforcement Violations have monetary penalties Be sure to read the parking signs
42 EHS Forms Vehicle Registration Form You must register your vehicle(s) with the EHS department. Security will distribute the vehicle registration form during the badging process. In order for you to receive your employee badge and parking permit you must turn in the vehicle registration form to security by the end of day. Your parking permits must be displayed on the rear view mirror with the number visible through the windshield. Ergonomic Assessment Form The ergonomic assessment form is designed to help you set up your workstation and to identify any ergonomics items you may need (e.g. mouse rest). The ergonomic assessment form will be ed to you. You will have to two weeks to complete the assessment. EHS will follow up with you after the form is submitted and if further evaluation is needed.
