1 Getting started on Otezla Yor gide to starting and staying on treatment Otezla (apremilast) is a prescription medicine approved for the treatment of patients with moderate to severe plaqe psoriasis for whom phototherapy or systemic therapy is appropriate. Otezla is a prescription medicine approved for the treatment of adlt patients with active psoriatic arthritis. Please see Important Safety Information on back cover. 2 Otezla Is Different Otezla is different Otezla (apremilast) is not a biologic, an injection, or a cream Otezla is a daily pill taken once in the morning and once at night, every day* For people with severe kidney disease, Otezla is one pill a day Otezla works from inside the body to help redce inflammation The Prescribing Information for Otezla has no reqirement for rotine lab monitoring Take Otezla exactly as directed by yor doctor. *For yor first day, yo only take one pill. Selected Safety Information Yo mst not take Otezla if yo are allergic to apremilast or to any of the ingredients in Otezla. Otezla is associated with an increase in adverse reactions of depression. In clinical stdies, some patients reported depression and sicidal behavior while taking Otezla. Some patients stopped taking Otezla de to depression. Before starting Otezla, tell yor doctor if yo have had feelings of depression, sicidal thoghts, or sicidal behavior. Be sre to tell yor doctor if any of these symptoms or other mood changes develop or worsen dring treatment with Otezla. 2 3

3 Proven in clinical stdies Selected Safety Information For plaqe psoriasis Some patients taking Otezla (apremilast) saw 75% clearer skin after 4 months. Otezla redced: Redness Scaliness Skin thickness Side effects of Otezla were diarrhea, nasea, pper respiratory tract infection, tension headache, and headache. The majority of patients reporting nasea and diarrhea did so within the first 2 weeks of treatment and these side effects tended to resolve over time withot stopping Otezla. For psoriatic arthritis Some patients taking Otezla for 4 months had improvements in their joint swelling and joint tenderness. In these stdies, some patients also experienced significant improvement in: Physical activities of daily living Joint pain Swelling of joints affected by psoriatic arthritis (fingers, toes, knees, and heels) Side effects of Otezla were diarrhea, nasea, and headache. These side effects occrred within the first 2 weeks of treatment and tended to resolve over time withot stopping Otezla. Some patients taking Otezla lost body weight. Yor doctor shold monitor yor weight reglarly. If nexplained or significant weight loss occrs, yor doctor will decide if yo shold contine taking Otezla (apremilast). Some medicines may make Otezla less effective, and shold not be taken with Otezla. Tell yor doctor abot all the medicines yo take, inclding prescription and nonprescription medicines. These are not all the possible side effects with Otezla. Ask yor doctor abot other potential side effects. Tell yor doctor abot any side effect that bothers yo or does not go away. Tell yor doctor if yo are pregnant, planning to become pregnant or planning to breastfeed. Otezla has not been stdied in pregnant women or in women who are breastfeeding. Dring the first 5 days of yor treatment yo will gradally increase yor dose ntil yo reach the recommended dose of 30 mg twice a day. This is intended to lessen the gastrointestinal side effects that may occr when yo first start taking Otezla. Day 1: Take only one 10-mg pill in the morning Day 2: Take one 10-mg pill in the morning and one 10-mg pill at night Day 3: Take one 10-mg pill in the morning and one 20-mg pill at night Day 4: Take one 20-mg pill in the morning and one 20-mg pill at night Day 5: Take one 20-mg pill in the morning and one 30-mg pill at night Days 6 and beyond: Take one 30-mg pill in the morning and one 30-mg pill at night Once yo complete yor Starter Pack, yo shold take one pill in the morning and one at night. 5 Staying on treatment Otezla (apremilast) is taken daily as directed by yor doctor. Do not stop taking Otezla nless directed by yor doctor. After yor initial 5 days on Otezla, keep in mind: Take one pill in the morning and one pill at night Can be taken with or withot food. Do not crsh, split, or chew the pill Keep yor Otezla bottle where yo can see it so that yo remember to take yor medication every day Store tablets below 30 C (86 F) Staying on Otezla Connect taking yor medication to something yo do each day, sch as after yo brsh yor teeth If yo have severe kidney disease, speak to yor doctor abot how to take Otezla.
