3 October 11, 2012 To the Skokie Commuity - I am pleased to preset the Skokie Commuity Health Pla: A Five-year Strategy for a Healthier Skokie. For 18 moths a vibrat, diverse commuity group has worked to develop the pla, which is a first for Skokie. As part of the process, people who live, work ad atted school i Skokie were asked to share their observatios about health issues i Skokie through a widely distributed Commuity Health Survey. More tha 1,300 people completed the survey, sharig cocers alog with thoughtful commets ad suggestios. This pla simply would ot have bee possible without such strog commuity ivolvemet. The Skokie Commuity Health Pla establishes five-year commuity health priorities for the Skokie Health Departmet ad its parters, ad creates a visio for a healthy commuity. Policies ad programs have bee developed for four priorities: improvig access to healthcare, prevetig obesity, prevetig tobacco use, ad protectig the eviromet. Of course the Health Departmet will also cotiue to focus o core public health fuctios, such as esurig safe food ad water, ad respodig to ifectious diseases i the commuity. Fulfillig the Commuity Health Pla will require a committed effort, ad we will provide aual updates describig progress. 4 Creatig the Skokie Commuity Health Pla Why a Commuity Health Pla? The Skokie Health Departmet is charged with periodically assessig commuity health eeds ad determiig strategies to improve health. The process leads to a commuity health improvemet pla with measurable goals. A healthy commuity is the resposibility of may idividuals ad orgaizatios workig together. Comig Together The Skokie Commuity Health Pla was created by a diverse group of commuity members determied to idetify ad respod to key health issues i Skokie. The Skokie Health Departmet sought broad commuity iput, ad ivited commuity parters committed to public health i Skokie to participate. The group, kow as the IPLAN Committee 1, esured that health issues addressed i the pla were importat to the commuity. This is the first public health commuity egagemet project of this size i Skokie. Commuity Egagemet To begi the commuity egagemet process, a comprehesive Commuity Health Survey was distributed, ivitig iput from ayoe who lived, worked or atteded school i Skokie. May commuity groups ad orgaizatios shared the survey with their members, icludig busiesses, citize groups, religious istitutios, public ad private schools, huma service agecies ad govermetal etities. 2 Thaks for askig. It is importat for the village to give residets the opportuity to speak up about issues. Skokie is a great place to live ad everyoe i every eighborhood should be part of the solutio to keep it that way!!!!! 3 The eergetic commuity respose to the Commuity Health Survey was excitig, with more tha 1,300 people completig surveys. These idividuals raged from 18 to 92 years of age, ad were from diverse groups. The Commuity Health Pla would ot have bee possible without the thoughtful isights, cocers ad suggestios received by the IPLAN Committee through the survey. The IPLAN Committee recogized that certai groups i Skokie were ot well represeted i the Commuity Health Survey, ad the Health Departmet took further steps to give all residets a opportuity to participate. For example, because the survey was oly offered i Eglish, special sessios were held at the Niles Towship ELL (Eglish Laguage Learer) Paret Ceter with iterpreters. We love Skokie ad are always pleased that the city is costatly tryig to improve itself. It is a very clea city ad has some of the best services aroud. Thak you for elistig feedback from the residets. Data Review I additio to the Commuity Health Survey, the IPLAN Committee reviewed a wealth of Skokie public health data o commuicable diseases, causes of death, materal ad child health, the 2010 U.S. Cesus, school health, emergecy medical service data, health system commuity eeds assessmets, evirometal health ad gaps i huma services. 5 The Commuity Health Survey Overview The Skokie Commuity Health Survey was distributed widely i Skokie over a six-week period i early The commuity respose was impressive, with 1,361 surveys retured. Aswers were compared for a umber of variables, icludig age, geder ad foreig-bor status. Sigificat differeces i health cocers were related to age i this survey. For example, youger idividuals were cocered about domestic violece ad access to low cost immuizatios, issues that were ot i the top 10 for older groups. Nearly 600 people submitted commets for the two opeeded questios, which mirrored the health rakigs. Sample commets are icluded throughout this documet. Who completed the surveys? 67% were Skokie residets, with more tha half livig i Skokie for 19 years or loger 69% were female ad 31% were male. 6 Access to Health Care Goal: Esure that all Skokie residets have access to high quality, affordable healthcare, icludig medical care, detal care, visio care ad metal health services. Overview Access to healthcare was the top cocer idetified by Skokie residets durig the Commuity Health Pla developmet process. More tha 150 people submitted commets through the Commuity Health Survey describig challeges with access to healthcare i Skokie. We eed more resources for commuity metal health, so that people do ot have to wait so log for services. Sice the ecoomic dowtur bega i 2008, both the Skokie Health Departmet ad Huma Services Divisio have received icreasig requests for assistace from uisured residets seekig care. I respose, Health Departmet staff visited area healthcare sites ad spoke with commuity members, to evaluate existig healthcare resources for people without isurace. There is a eed for more providers who accept Medicaid ad/or a slidig scale. There are curretly o medical cliics i Skokie servig people without isurace. Low-icome, uisured Skokie residets are referred to cliics located i Chicago. A equally pressig problem is the lack of detal services for people without isurace; there are few optios for these idividuals. Medicaid availability for uisured adults was substatially reduced by the Illiois legislature durig the 2012 legislative sessio. Similarly, resources for metal health services i Illiois have bee reduced over the past five years 1. Without isurace, idividuals put off seekig care, ad do ot receive prevetive care, turig to area emergecy departmets for services. A 2010 Healthcare Accessibility Survey coducted joitly by the Skokie Health Departmet, Skokie Huma Services ad the Niles Towship Food Patry revealed a strog correlatio betwee educatioal level, employmet ad healthcare access through isurace. Of the 275 cliet surveys completed, 77% were by Skokie residets, ad most respodets had a aual icome of less tha $25,000. Commuity Health Survey How easy is it for you ad your family to get Medical Care? (Skokie residets) Status More likely to aswer Medium/Hard Full Time Studet 7.4 times Uemployed 2.5 times Foreig Bor 1.7 times Homemaker 1.5 times Similar to atioal studies, 93% of low-icome seiors completig the survey had access to healthcare through Medicare, but for those age 18 to 29 years, most (54%) had o isurace ad 31% had Medicaid. The Commuity Health Survey, cofirmed that youg adults who were full-time studets were 7.4 times more likely tha other residets to idicate that it was ot easy to obtai healthcare. Uemployed ad foreig-bor residets also metioed challeges obtaiig healthcare services. Accordig to the America Commuity Survey (ACS) estimates, there are 15,800 ± 2,000 Skokie residets who live below 200% of the federal poverty level ad would qualify for services provided by Federally Qualified Health Ceters (FQHCs) 2. The surveys ad iterviews coducted as the basis of the Commuity Health Pla support the ACS estimates; there are a sigificat umber of Skokie residets who do ot have access to medical or detal care. 4

7 Where do we go from here? Policies Advocate for all Skokie residets to have access to comprehesive medical care, icludig primary care ad specialty care services. Advocate for area healthcare systems to collaborate with the ew Evasto/Skokie Erie Family Health Ceter, a FQHC, to provide specialty medical care (for example, cardiology, ocology, eurology, etc.). Advocate with commuity metal health parters ad providers to icrease fudig for ad access to metal health services for the uisured ad uderisured. Advocate with commuity detal health parters ad providers to icrease fudig for ad access to detal health services for the uisured ad uderisured. Programs Provide data regardig commuity eeds to area healthcare providers i order to advocate for eeded healthcare, icludig primary ad specialty medical services, metal health ad detal health. Collaborate with Turig Poit Behavioral Health Care Ceter to icrease access to metal health services. Collaborate with various commuity social service agecies to idetify ad eroll eligible residets for Medicaid/ Medicare ad All Kids Isurace before emergecy services are eeded. Overview Skokie residets idetified obesity prevetio as a top priority durig the Commuity Health Pla developmet process. More tha 100 people metioed cocers about obesity o the Commuity Health Survey, ad may submitted suggestios for policies ad programs to address this issue i the Village. We should ecourage bicyclig, more paths, more bike laes, more bike lockig areas, especially i dowtow Skokie ad at Old Orchard. Proposed approaches were i four categories: Icreasig healthy food optios Icreasig opportuities for walkig ad bikig Icreasig utilizatio of public trasportatio Icreasig affordable optios for recreatio, especially for families ad childre, icludig durig the witer moths We eed more healthy food choices. More restaurats featurig good, orgaic optios ad more accessible grocers dedicated to whole or orgaic foods would be a great start. Obesity is a commo problem i Illiois; the Illiois Alliace to Prevet Obesity estimates that more tha 31% of Illiois childre, ad 62% of adults, are overweight or obese. 1 Most cocerig is the dramatic icrease i childhood obesity sice 1990, because beig overweight or obese as a child icreases the likelihood of chroic diseases such as diabetes, high blood pressure, heart disease ad early death. Childre who are overweight are likely to cotiue to have excess weight as adults. Our childre eed to lear at a early age that ivolvemet i physical recreatio, exercise ad youth sports activities is ecessary ad a key compoet to learig how to lead a healthy ad productive life. The most successful ad cost-effective strategies for combatig obesity begi with weight loss through diet ad exercise. Recet studies suggest that lifestyle itervetios ca result i a 8 to 10% weight loss i six moths. 2 A goal of the Commuity Health Pla is to prevet childhood obesity by determiig obesity rates i Skokie ad developig targeted approaches. A key step is to help mothers ad families with proper utritio ad breastfeedig. Durig iterviews coducted as part of the Commuity Health Pla process, parets ad teachers expressed cocer that studets were ot able to safely reach school o foot or by bicycle. Overview I 1977, the Skokie Board of Health recommeded a ordiace requirig o-smokig areas i all public places. Thus bega a 26-year jourey towards Skokie becomig the first muicipality i Illiois to pass a Clea Idoor Air Ordiace protectig people from exposure to secod had smoke i all workplaces ad public places i Steps alog the way icluded prohibitig tobacco sales to miors, elimiatig self-service tobacco vedig machies, ad forbiddig the free distributio of tobacco products. I am cocered about so may youg people smokig, particularly at the commuity college. The Village has implemeted other measures to limit tobacco use i Skokie. The Health Departmet receives fudig to coduct tobacco cessatio programs, provide oe-o-oe couselig, ad offer icotie replacemet therapy. Through the State Tobacco Eforcemet Grat Program, the Skokie Police Departmet visits all 54 tobacco retailers three times a year to check for sales to miors. It s importat to keep all residets healthy by reducig smokig amog adults ad especially tees. Although tobacco use rates have declied over time, smokig cotiues to be a serious prevetable cause of illess ad death i Skokie. Of the 2,333 Skokie residet deaths betwee 2005 ad 2008, cacer was the leadig cause of death with lug cacer aloe accoutig for 6% of all deaths. Accordig to the Surgeo Geeral, smokig harms early every orga i the body ad causes at least 30% of all cacer deaths, early 80% of deaths from chroic obstructive pulmoary disease, 30% of heart disease associated death, pregacy complicatios ad sudde ifat death sydrome 1. Target groups Three groups have bee idetified for targeted tobacco prevetio ad cessatio efforts i Skokie: Childre, adolescets ad youg adults Foreig bor residets from coutries with high rates of tobacco use People with metal disorders The 2010 Niles Towship High Schools Studet Survey idicated that 15% of studets ackowledged usig tobacco. A goal is to reduce that umber to 12% through active collaboratio with Skokie schools ad parets over the ext five years. Protectig Soil ad Water: Batteries, sharps, expired medicatios ad compact fluorescet light bulbs are accepted at Village Hall for disposal. Educatio: The popular aual collaborative Skokie Sprig Greeig: A Commuity Expo, is held each year to promote gree livig ad electroics recyclig. Although the Village proactively seeks opportuities to protect the eviromet, a comprehesive evirometal pla has ot yet bee developed.
