NPS in Europe New Psychoactive Substances among People Using Drugs Heavily: Towards Effective and Comprehensive Health Responses in Europe SANANIM, Czech Republic, Aleš Herzog, Jiří Richter LOCAL PILOT INTERVENTION in Czech Republic Final Seminar &Third Project Meeting Lisbon 25/26 th Nov 2015
SANANIM Drug ServicesCzech Republic founded in 1990 nowadays one of the largest providers of complex services in the field of non-alcoholic drug addiction 9 main projects and several additional ones more than 140 employees
SANANIM in numbers, 2014 More then 9000 clients. It means: hours of individual psychotherapy hours of group psychotherapy about 250 clients in daycare programs daily more than 50 clients in residential programs daily more than 200 children in treatment with mothers 1, needles exchanged
NPS AMONG PEOPLE WHO USE DRUGS HEAVILY IN CZECH REPUBLIC The use of new synthetic drugs (cathinones alpha-pvp called FUNKY). The main factor for choosing new substances were changes in the availability and the quality of primary drug methamphetamine - PERVITIN.
NPS among people who use drugs heavily in Czech Republic The use of new synthetic drugs (opioid medications). The main factor for choosing new substances were changes in the availability andin the quality of primary drug HEROIN.
Intervention plan - Needs - PUDHuse NPS (mainly cathinonesand opioid medications) and have health and mental problems - Professionals have difficulties in accessing and communicating with NPS users and low ability of harm reduction services to react on NPS - Limits and low function of early warning system in Czech Republic - Important stakeholders (e.g. police, local politicians) do not support substitution treatment. They have little information about possible risk of balloon effect.
Intervention plan - Objectives - Obtain information about cathinonesand opioid treatments - Influence attitudes of PUDH about using cathinones and opioid treatments - Activate Early Warning System in Czech Republic
Intervention plan - Activities - Direct communication with drug users about risk in using NPS. - To offer updates on information about NPS on web site - Direct campaign focused on reduction of risks through Dekontaminace. - Trainingof outreach and contact workers - Peer driven activities at trafficking places of NPS - Identify, describe and define problems and limits of existing Early Warning System Czech Republicby meeting and pressure on National Focus Point - Change opinion of police and local politicians to support substitution treatment by articles and meeting.
Implementation Main problems/constraints Low interest of the stakeholders. Ourvoiceisnot as strongas thevoiceofdrugmarket
Direct campaign focused on reduction of risks
Direct campaign focused on reduction of risks
Change opinion of Police to support substitution treatment Training Summer school HR, Mechenice, ; Discussion with Police; Col. JUDr. Frydrych
Training of Outreach and Contact workers
Evaluation of the interventions Raised awareness of PUDH about new synthetic drugs. Empowerment of negative attitude to cathinones. An increase in awareness about the risk of overdosing with fentanyl. We raised awareness about the issue among professionals in low threshold services. We were unable to find a system improvement in the functioning of Early Warning System (EWS).
Lesson learned/questions Focus groups are really good tool to learn and teach Are new psychoactive substances worse then those, that are now used by PUDH? How can we do effectively work with attitudes of PUDH? How can we influence our police authorities to support substitution treatment?
Recommendations Good way how to influence knowledge and attitudes of PUDH is to ask them about their opinions and experiences Good platform for communication with PUDH is magazine. Our magazine Dekontaminace has been published for 20 years, it is distributed by 65 lowthreshold services, and circulation is 6000.
