PRINT/DISTRIBUTION PERMITTED. Agenda eccf Paperless Process NRCME* and Rule 2 Drug and Alcohol Clearinghouse Upcoming Proposed Rules What we Learned by Going Digital? *National Registry of Certified Medical Examiners (NRCME) 1 eccf Paperless Process 2 1
2 eccf Paperless Process Registration Since 1999, one of the most highly anticipated changes MRO in the industry for clinics, laboratories, Medical Review Officer s (MRO), and the Designated Employer Representative (DER) SAMHSA 1 certified labs Authorization Collection Site Tips for successful eccf paperless process adoption: 1. Notify driver that a test is required 2. Donor proceeds to collection site with an authorization/protocol/confirmation/who is paying for this transaction document 3. Collector selects a system and authenticates 4. Specimen Collection service(s) performed 5. Paperless process generates the custody control form and captures collector and donor signatures 6a. Alcohol only reports directly to employer 6b. Specimen is packaged for shipping to SAMHSA lab 7. Laboratory receives and conducts test(s) KEY CHANGE is differentiating between electronic and paper CCF for corrective action 8. MRO receives result and CCF, confirms results, then reports to DER 1. Substance Alere escreen Abuse and Mental Connecting Health Services safety, Administration. health, and wellbeing 3 NRCME and Rule 2 4 NRCME and Rule 2 National Registry of Certified Medical Examiners May 21, 2014 Requires all interstate CMV drivers have physical exam performed by an examiner in good standing on the National Registry Medical Examiners must complete training and pass a certification test to participate in the registry Examiners must refresh training every 5 years; test every 10 years to maintain certification NRCME Registry Physician shortage did not materialize Technology supports confirming active registry status Automated submissions support reporting of completed exams RULE 2 4/19/16 New Forms/Certificates Revised Health History Revised Exam Questions New Certification Statuses - Determination pending - Separate federal/state - Incomplete Exam 5 2
3 Drug and Alcohol Clearinghouse 6 Drug and Alcohol Clearinghouse Requires motor carriers, medical review officers, third-party administrators, and substance abuse professionals to report substance abuse violations to a central repository Carriers will be required to search the repository for unresolved violations on current or potential drivers Drivers must grant consent to a carrier to search the clearinghouse; and consent to FMCSA to release the records 3 year compliance period ends January, 2020 Clearinghouse will contain and report information about drivers who: Test positive for drugs or alcohol Refuse drug and alcohol testing Undergo the return-to-duty drug and rehabilitation process Key Considerations: Paperless Consent/Electronic Signature Capture Common Identifier for Driver Records Latency in reporting inbound/outbound Refusal to consent process Managing duplicate records Alias and Surnames Technical integration capability 18 Upcoming Proposed Rules 8 3
4 Upcoming Proposed Rules Alternative Testing Matrices Continued focus around Hair and Oral Addition of Certain Schedule II Drugs to the Panel Add Hydrocodone, Hydromorphone, Oxycodone, Oxymorphone Add MDA as a confirmatory analyte Remove MDEA as a confirmatory analyte Sleep Apnea Public Meetings held August, 2016 Medical Review Board Recommendations published Expect significant activity in this area What we learned from being digital? 10 What have we learned from being digital?
