3 Background In families living with type 2 diabetes, relatives have a significantly heightened risk of developing the disease. In many families, both the person with type 2 diabetes and his/her relatives lack detailed knowledge about this risk. An obstacle to constructive intra-familial prevention and risk reduction is the lack of perceived familial disease relevance. The objective of this study is to explore barriers to prevention in families with at least one adult with type 2 diabetes. 3

8 Sole responsibilities and the absence of collective practices I ve tried to meddle in the things I think he s doing wrong. But that has not come out to my advantage. Since then I haven t meddled at all. He takes care of himself and that s just fine (wife) 8

10 Intra-familial differences in perceptions of risks and future health I think I didn t do a good enough job of communicating to them, that they could get it too. I think I felt, that it didn t concern them. My children are adopted, so they are not genetically connected to me but they still have to exercise and eat healthily (woman with diabetes) 10

We have tried to mention it, but he won t listen. He mocks us and makes fun of us. His wife is the same way. (man with diabetes) 11

12 Lack of perceived disease significance and the ensuing lack of mutual care We never really talked about it. They didn t tell us what it was - and it s a disease you can t see. So it never felt like she was sick. She was just tired all the time. I have a brother who s 18 now and I don t think he knows that our mother has diabetes (daughter) 12

All his sisters have it and his mother has it and my grandparents on my mother s side as well. So we ve known about it but I ve never really given it any serious thought (daughter) 13

14 Conclusion Intra-familial prevention is a complex matter that calls for complexity-oriented approaches in healthcare practice. Theories of horizons of significance and social imaginaries seem relevant to apply in this context, and they add new insights into and perspectives on the area of familial dynamics and complexities that may generate barriers to prevention. These new insights and perspectives could potentially help shift the focus from the negative aspects of wrongness and limitations to actively working with familial potentials, resilience and connectedness.
