1 Behavioral Health Focus Washington County Mental Health Provider List For Children Who Are Oregon Health Plan (OHP) Health Share Eligible or Uninsured Outpatient Mental Health Services Including: assessment, counseling, medication evaluation and management, and case management services. Services available in other languages. Eligibility Guidelines: OHP/Health Share eligible children OR Have a family income 250% below the federal poverty level AND Have no insurance coverage AND Are a resident of Washington County Services are available at the local agencies listed on the back of this page. You can also request a referral from the Washington County Member Services Access Line at Accessing Mental Health Benefits Through Your Private Insurance Call the member services number on your insurance card and ask about the following. Benefits, co-pays, types of covered mental health services Contact information for agencies or therapists that accept your insurance Do you have Employee Assistance Program (EAP) benefits through your employer? Ask in Human Resources if your EAP offers free mental health counseling. Mental Health Crisis & Urgent Care Services Washington County Crisis Line: Hawthorn Walk-In Center If you or someone you care about is in a mental health crisis, call the Washington County Crisis Line except when the person is in immediate danger or harming oneself or someone else-then call 911 or go to the nearest emergency department. The crisis line is answered 24/7 and can contact the Crisis Team. The Crisis Team staff can see individuals at the Hawthorn Walk-In Center to provide crisis intervention as well as brief follow-up services. The Hawthorn Walk-In Center is Washington County s mental health urgent care center open seven days a week, 9am-8:30pm, located at 5240 NE Elam Young Parkway Suite 100 in Hillsboro right next to the Hawthorn Farm MAX stop. Both walk-ins and appointments are accepted. Services are provided at no cost to the individual. Addressing the needs of children, youth and families in Washington County

To Apply for the Oregon Health Plan Online: Call: As of Jan. 1, 2018, the Oregon Health Plan (OHP) became available to more children and teens younger than 19, regardless of immigration status. This includes youth with Deferred Action for Childhood Arrivals (DACA) status and those previously only eligible for Citizen Alien Waived Emergent Medical (CAWEM or CAWEM Plus). All other OHP eligibility criteria for those under 19, such as household income, remain the same.
