4 Palliative Care Part of the DSRIP program and aims to increase access to Palliative Care in PCMH practices. Palliative Care is specialized medical care for people with serious illnesses. It is focused on providing patients with relief from symptoms, pain, and stress of a serious illness whatever the diagnosis. 6 Clinical Dashboards Allows the practice to monitor patient outcomes over time. Can be created to meet any criteria you would like to follow. i.e. Age, gender, insurance. Can have multiple dashboards on one screen, which is set up through Chart Central. Setup takes two steps creating the Clinical Dashboard template setup and then creating the Dashboard. 7 Medent Document Code Utility

8 Document Code Utility Medent has a utility that can be run to change one document code to another quickly and wherever it appears on a patients chart. This process is done through a Support Ticket with Medent Medent runs the utility for you. Refer to the Medent Manual by searching Utility for more details Proprietary and Confidential

19 1. Preferences->Other Preferences->Clinical Dashboard Setup 2. Clinical Dashboard Master Templates

21 6. Name the Group 7. Then Set Conditions 8. The first criteria that is selected will be the denominator.

22 10. The next criteria that you select will become the numerator. 9. Enter appropriate rules for criteria item.

24 Template is now completed. Now you will create the dashboard to attach the template to.

25 13. Once again select New To create the dashboard add a Name, select the doctors to include, attach the template you just created, select the insurances to search on, determine if patient ACO status is applicable, and select patient status and location.

26 Scroll down until you find Clinical Dashboard1 and select it to send it to the Selected side. To run the dashboard, you must add a button to chart central. To do this- click the chevron and then the gear button.

27 You will now see that a new button has been added to Chart Central. It will initially be titled Clinical Dashboard 1. You can click this to run the dashboard. The following message will appear when you click on the button. Select continue. Your screen will go back to Chart Central. Give it a few minutes and click the ACO Members button again.

28 Running the cache from Cart Central will now produce this graph. It gives the percentage of patients that you ve counted in your denominator that are meeting the measure definition in the numerator. To see more detailed data, click on the bar of the corresponding month. When you click on the month it gives a more detailed breakdown.

29 This screen also shows you the measure definitions. From the screen above clicking on the percentage bar gives an even more detailed breakdown including patient name and account number and if they meet the criteria or not.
