Author Kian Yong Toh, Shan Han Lun In Brief 傷寒論 is the work from 張仲景, AC, he has written 傷寒雜病論, 傷寒論 is the first book of the two. 傷寒論 has 10 rolls, 22 chapters, 397 laws ( 法 ),112 prescriptions ( 方 ) It is recognized as the first book to set the foundation for TCM, it is learnt from the past scholars and practitioners, it is still the basic learning in all TCM universities in China. 傷寒 means harm due to Cold literary. Some scholars interpret the 寒 in a broader sense, to include all other pathogens, such as Wind, Damp, Heat etc... The TCM diagnosis in 傷寒論 is based on the six stages concept. This concept is used daily in TCM practices since 2000 years in China and elsewhere. The prescriptions that are written in the books are a collection of 張仲景 s personal clinical experience and the experience before his time. To master how to use these prescriptions is a must if one wants to be successful in writing Chinese herbs for his patients. Some very known TCM practitioners in China used only one or two of these prescriptions with their variations to treat most patients in their daily work. There are practitioners who study the 傷 寒論 for their whole life, they did not manage to try all formulae in 傷寒論.

2 Tai Yang Stage ( ) Channels involved 2 The Bladder (feet, Water) and the Small Intestine (hands, Fire) channels define the Tai Yang stage. General Description and Causes The Tai Yang stage is the beginning stage of the external induced febrile disease. The main causes are the external Wind and Cold pathogen attacking the body surface (Biao), the fighting territory between the pathogenic Qi and the upright Qi take place at the body surface. If the Wei Qi is weak and is not strong enough to fight off the pathogenic Qi, the Wind Cold pathogen attacks the body surface could result in the Ying Qi and Wei Qi out of balanced, thus illnesses starts at the Tai Yang stage. Main Symptoms Aversion to Cold / Wind, fever, neck pain, headache, floating pulse are common symptoms for illnesses at the Tai Yang stage. At Tai Yang stage, the symptoms depend on where the pathogenic Qi stays, when the pathogenic Qi rests at the channels level, often the patient has neck pain, when the pathogenic Qi rests at the skin surface, the lung Qi is attack, often result in coughing, when the pathogenic Qi stays at the Biao level, and the Li (inner) is out of balance, often result in vomiting and diarrhea. Differentiation Tai Yang illnesses can be divided into: Jing (channel) and Fu (organ) illnesses Jing illnesses are the main one, main signs are floating pulse, head and neck pain, aversion to Cold / Wind. Due to the difference in body constitution and the pathogenic Qi, Jing illnesses can be sub-divided into: 中风 : fever, sweating, aversion to Wind, head and neck pain, floating and slow pulse, Wei Qi is strong, Ying Qi is weak 伤寒 : fever, no sweating, aversion to Cold, shortness of breath, head and neck pain, floating and tight pulse. Wei Yang blocks the surface, Ying Yin stagnated 表郁轻证 : fever and aversion to Cold, long standing Biao illness not resolved Fu illnesses: when the Jing illness is not solved, the pathogenic Qi enters via the channels into the Fu organs. In the case for illnesses at the Tai Yang stage, the pathogenic Qi can combine with the Water element or with the Blood element.

3 - 类似证 : this refers to illnesses that are not originated from external Cold, yet the symptoms are similar to illnesses at the Tai Yang stage. 3 The treatment principle and representative prescriptions For Jing illnesses, use pungent herbs and sweating method to eliminate the pathogenic Qi. Main prescriptions are: Ma Huang Tang, Gui Zhi Tang, Xiao Qing Long Tang, Ge Geng Tang. For Fu illnesses, use Water draining herbs, Qi movers herbs, Blood invigorating herbs. For all other cases at Tai Yang stage, based on the condition of the tongue and the pulse, the strategy should be adapted accordingly. Pathogenesis If the illness at Tai Yang stage is treated successfully with sweating method, then the patient returns to its health stage. If the illness is not treated successfully at the Tai Yang stage, the illness might be progressed into other stages, such as Shao Yang, Yang Ming, or a combination of stages. Illness at the Tai Yang stage could also turn into Yin stages, among these, Tai Yang turns into Shao Yin is the most common one, when Yang Qi in Shao Yin is weak, the pathogenic Qi in Tai Yang has more chances to go into the Shao Yin stage. It could be a combination of Tai Yang and Shao Yin illness at the same time. As the saying goes: Fullness at Tai Yang, deficiency at Shao Yin.

4 Yang Ming Stage ( ) Channels involved 4 The Stomach (feet, Earth) and the Large Intestine (hands, Metal) channels define the Yang Ming stage. General Description and Causes At Yang Ming stage, the pathogenic Qi enters into the channels as well as the Fu organs. Upright Qi and pathogenic Qi are fighting fiercely with each other, inner Heat resulted from this fight, body fluid is consumed, dryness appeared due to the over consumption of body fluid. The causes of illnesses at Yang Ming stage are: - From Yang stages, illnesses at Tai Yang or Shao Yang stages due to wrong or delayed or no treatment. - From Tai Yin stage, the pathogenic Qi could enter into the Fu organs from the Zhang organs, so called Yin illness enters into the Yang stage. - Pathogenic Qi directly attacks the channels of the Yang Ming stage. Precondition here being the with a full condition, abundant Yang that injure the body fluid, accumulation of the food in the body. Main Symptoms High fever, big pulse, big Heat and big sweat are signature symptoms of illnesses at the Yang Ming stage. For the beginning of Yang Ming stage, the patient could have the Biao symptoms as well. Inside (Li), Heat and fullness are involved here. Differentiation Illnesses at the Yang Ming stage can be divided into: - Jin (Heat) type: high fever, heavy sweat, big thirst, big pulse. - Fu (Full) type: pathogenic Qi binds with the waste of the body, resulted in full condition in the Fu organs. Aversion to Heat, sweating, agitation, tidal fever, delirium. The patient could also have constipation, pain in the abdomen. -- If Heat from Yang Ming combined with the Dampness from Tai Yin, this could lead to jaundice, Damp Heat condition, difficulty in urination, thirst, fullness in the abdomen. This is termed as the jaundice in Ying Ming.

5 -- If the Heat of Yang Ming enters into the Blood level, one can see Bloody stool, bleeding nose, delirium too. 5 The treatment principle and representative prescriptions Yang Ming is based on full and Heat condition, use clearing and downward purging methods. For Yang Ming Heat: use herbs that clear heat. Bai Hu Tang or Bai Hu Jia Ren Shen Tang. If the Heat is not treated right, the Heat may stay in the chest area, one could try to use Zhi Zi Gu Tang ( 梔子鼓汤 ), if however the Heat injures the Lower Jiao, the pathogenic Heat may combine with the Water element, this often result in fever, thirst and desire to drink, little urine, floating pulse, one may try to use Zhu Ling Tang. For Yang Ming full condition: use herbs that have downward purging effect. Da Chen Qi tang, Xiao Chen Qi tang, Tiao Wei Chen Qi tang or MaZhiRen Wan, Mi Jian Fan, Zhu Dan Tang, Yin Chen Hao Tang, Di Dan Tang. It is to note that once the herbs have achieved the desired result, the patient should not prolong taking them in order to avoid injury to the Stomach organ and channel. Pathogenesis The classical theory says that the pathogenic Qi from Yang Ming will not be transmitted to other stages, but in fact, one often observe that the Biao and Li relationship with Tai Yin do exist. As the saying goes: Fullness at Yang Ming, deficiency at Tai Yin.

6 Shao Yang Stage ( ) Channels involved 6 The Gall Blader (feet, Wood) and the San Jiao (hands) channels define the Shao Yang stage. General Description and Causes Among the three Yang stages, Tai Yang being Biao (surface), Yang Ming being Li (inside), Shao Yang is the stage between the Biao and the Li stages. The causes for illnesses at the Shao Yang stage are: - The body is too weak to defend against the pathogenic Qi, and the pathogen attacks the body and enters into the Shao Yang stage directly. - From the Tai Yang stage, illnesses that are treated wrongly or delayed or no treatment, the illnesses might enter into the Shao Yang stage. - From the Jue Yin stage, Shao Yang and Jue Yin have Biao and Li relationship, when the upright Qi from Jue Yin is recovered, the illness could enter into the Shao Yang stage before the recovery. Main Symptoms Intermediate Cold and Heat manifestation is the signature sign for illnesses at the Shao Yang stage. Symptoms include liver Fire signs: bitter taste in the mouth, dry throat, dizziness, flank pain, agitated and vomiting, no appetite. Shao Yang, having closed relationship with Tai Yang, Yang Ming, when Shao Yang is mixed with Tai Yang illness, often seen symptoms are fever with aversion to Cold, pain in the joints, if combined with Yang Ming, often seen symptoms are hard stool, light vomiting. Differentiation Type of Shao Yang illnesses: - Jin type: pathogenic Qi in the channels, deafness, red eye, headache, flank pain, alternate Cold and Heat. - Fu type: Liver Fire, the function of Spleen and Stomach is disturbed. Bitter taste in the mouth, dizziness, dry throat, agitated, vomiting, no appetite. It is to note that the distinction between the Jin and the Fu types for illnesses at the Shao Yang stage is often not clear, in Tai Yang and Yang Ming, the distinction between the Jin and the Fu is much clearer. 7 To treat illnesses at the Shao Yang stage, use harmonization method. As Shao Yang is not at the Biao level, so it is not advisable to use sweating method, it is not at the Li level, so one should avoid using downward purging method. If the pathogenic Qi is not blocked at the chest level, so it is not advisable to use vomiting method. 7 If the Biao condition exists as well at the Shao Yang stage, use harmonization plus the elimination of the Biao, one possibility is to use Chai Hu Gui Zhi Tang. If the Yang Ming Li full exists at the same time as Shao Yang Stage, then, harmonization plus elimination the inner full condition should be applied, Da Chai Hu Tang or Chai Hu Jia Man Xiao Tang should be considered. If San Jiao is blocked with Shao Yang, and the body fluid cannot be distributed, then harmonization plus transform Qi and nourishes the body fluid, Chai Hu Gui Zhi Gan Jiang Tang should be considered. If Biao, Li, San Jiao and Shao Yang all exist, then Chai Hu Jia Long Gu Mu Li tang is often the choice for treating such complex case. The representative prescription is Xiao Chai Hu Tang, is sometime also known as Xiao Chai Hu Tang symptoms. Pathogenesis If the patient receives the right treatment, Shao Yang illnesses could be cured. If however, it is wrongly treated, such as body fluid is injured and dryness appears, often the illness may enter into Yang Ming stage. If the Yang is injured and the Yin is in excess, the illness may enter into Tai Yin stage. There is also the danger that Biao and Li are not treated in the right way, and the illness might eventually enter into Jue Yin stage. As the saying goes: Fullness at Shao Yang, deficiency at Jue Yin.

8 Tai Yin Stage ( ) Channels involved 8 The Spleen (Feet, Earth) and the Lunge (Hands, Metal) channels define the Tai Yin stage. General Description and Causes Tai Yin illnesses are the beginning stage of Yin illnesses among the three Yin stages. The causes of Tai Yin illnesses are: - Prenatal weakness manifested in Spleen Yang deficiency, this allows Damp Cold attacks the body directly. - Inner injuries that lead to Cold accumulation, over taking bitter and Cold herbs could also injure the Spleen Yang. - Yang illnesses wrongly treated, or not treated lead to Spleen Yang injured. Examples such as Tai Yang illnesses wrongly used downward purging method, Yang Ming illnesses overused bitter and cold herbs, the middle Yang is injured and thus allow the Yang illnesses turn into the Yin illnesses. Main Symptoms When the illness turns from Yang into Yin stage, the strength of the body to fight the illness is reduced, Spleen Yang is weak, and Cold and Dampness are the dominating signs. Fullness in the abdomen area, vomiting, no appetite and diarrhea are the common signs. Illnesses at the Tai Yin stage are related to Spleen, mainly manifested in the abdomen area. Dampness, deficiency in Spleen, fullness / pain in the abdomen area, vomiting, no appetite, diarrhea, no thirst are possible symptoms. The treatment principle and representative prescriptions Tai Yin illnesses are mainly the Spleen deficiency with Damp Cold, if the Spleen Yang is restored, then the illnesses are recovered. For Tai Yin illnesses, use warming method to eliminate Cold, strengthen the Spleen and dry the Dampness. Li Zhong Wan, Si Ni Tang are the representative prescriptions used for Tai Yin illnesses. Pathogenesis If Tai Yin illnesses are not treated, they could be progressed into Shao Yin and Jue Yin, at the same time, one observes often that illnesses in Shao Yin and Jue Yin have Spleen Yang deficiency signs, which confirms the point.

9 Tai Yin and Yang Ming are Bio and Li related, the illness from one stage could be progressed into the other stage. For example. When too much downward clearing method is used for Yang Ming illnesses, the illnesses could be progressed into Tai Yin stage, also when too much warm and drying method is used to treat Tai Yin, the illnesses could be progressed into Yang Ming. 9 As the saying goes: Fullness at Yang Ming, deficiency at Tai Yin.

10 Shao Yin Stage ( ) Channels involved 10 The Kidney (feet, Water) and The Heart (hands, Fire) channels define the Shao Yin stage. General Description and Causes Shao Yin is a dangerous stage of illness development from the six stages model. When the illness is at the Shao Yin stage, the defense mechanism is weak, Heart and Kidney are involved, Yin, Yang, Qi and Blood are all deficient, sleepy with weak pulse. Causes of Shao Yin illnesses: - The Shao Yin stage could be a result of pathogenic Qi (external Cold) attacks the Shao Yin channels directly or Kidney Yang deficiency due to old age. - Illnesses progressed from other Yang channels, mainly from Tai Yang. The reason being Tai Yang and Shao Yin has the Biao and Li relationship. When Yang Qi in Shao Yin is weak, and is not able to defense against the external pathogen, then the pathogen from Tai Yang will invade into Shao Yin. - Illnesses progressed from other Yin channels, mainly Tai Yin. Main Symptoms The signature symptoms are Cold and deficient in the whole body, sleepy. Differentiation There are two categories of Shao Yin illnesses: - Heart and Kidney Yang deficient, Cold type: aversion to Cold, like to lie down, vomiting and diarrhea. The pulse is weak and small, sleepy Water and Dampness accumulation, Lower Jiao prolapsed. - Heart and Kidney Yin deficient, Heat type: agitated, small and fast pulse, red tongue with inner Heat. Water in the Lower Jiao combined with Heat or Blood, Bloody diarrhea. The treatment principle and representative prescriptions Cold type: Tonify and warm the Yang, based on Si Ni Tang and its variation. Heat Type: Nourish the Yin and clear the Heat, based on Huang Lian Ai Jiao Tang and its variation. Commonly used formulas include: Ma Haung Xi Xin Fu Zi Tang, Da Chen Qi Tang, Si Ni San etc.. 11 If the patient does not receive the right treatment, Yin and Yang depletion might occur, which is a bad sign. 11 Shao Yin is an illness with upright Qi deficiency, avoids using sweating and purging downward methods, and if the patient could return to the Heath stage or not depending on if the Yang is rescued. Failure to do so might result in death. As the saying goes: Fullness at Tai Yang, deficiency at Shao Yin.

12 Jue Yin Stage ( ) Channels involved 12 The Liver (feet, Wood) and the Pericardium Channels (hands) define the Jue Yin stage. General Description and Causes Jue Yin is the last stage of the six stages. This is the final struggle between the Upright Qi and the pathogenic Qi. At this stage, Yin is depleted and Yang is weak, it is a dangerous stage if the patient does not recover. If there is a transition into the Yang stage, then it is a positive sign. Illnesses at the other 5 stages, due to wrong / no / delayed treatment, could progress into the Jue Yin stage. Direct attack of pathogenic Qi to the Jue Yin channels could happen, but seldom. Main Symptoms Alternative Cold and Heat feeling in the extremities is a signature symptom of Jue Yin illnesses. This is due to the depletion / re-appearance of Yin and Yang, which result in Cold and Heat feeling. It is to note that at Shao Yang stage, the alternate cold and heat is much more violent and it takes a longer time to change from Cold to Heat, and vice versa, say a few hours, yet in the case of Jue Yin, it takes a few seconds / minutes to change from Cold to Heat and vice versa. Another signature sign of Jue Yin stage is the patient feel Cold in the lower part of the body and Heat in the upper part of the body. The depletion of Yin leads to the appearance of the Yang, Yang rise. The depletion of Yang lead to the appearance of the Yin, Yin sinks, thus Lower Cold, Upper Heat. Liver channel, being part of the Jue Yin stage, when the Liver is out of balance, it could lead to disharmonies of Stomach and Spleen, Wood attacks Earth, vomiting and diarrhea belong to symptoms of Jue Yin. Differentiation At the Jue Yin stage, as the depletion of Yin and Yang lead to Cold and Heat symptoms, some scholars divide the illnesses at Jue Yin stage into Jue Yin Cold and Jue Yin Heat. The treatment principle and representative prescriptions Treating illnesses at the Jue Yin stage, use warming method to treat Cold, use clearing method to treat Heat. When Heat and Cold alternate, then use both methods. If there is upper Heat and lower Cold, then use clear upper and warm below.

13 For Jue Yin Cold symptom, use warm Liver nourishe Blood herbs. For Jue Yin Heat symptom, use Cool Liver and detoxify herbs. 13 Pathogenesis Before recovery, Jue Yin illnesses could turn into Shao Yang, Yang Ming stages, this is the case where the Yin illnesses turn into Yang stage. It is to note that when the Yang is recovered too quickly, it could lead to soar throat, Blood in urine. When both Yin and Yang are depleted, it is a dangerous sign. Prescriptions used for illnesses at this stage are: WuMeiWan, Gan Jiang Huang LianHuangQinRenShengTang, MaHuangShengMaTang.
