3 Basics of 5 Element Diagnosis Part XIX - Sciatica Dr. Homi J E Kaikobad MBBS DPH DIP AC NCCAOM LAC Bachelor of Medicine & Surgery Ret. Diplomate in Public Health. Diplomate in Acupuncture & National Board Certified. Licensed Acupuncturist All of diagnosis is reading signs, and all of reading signs is being on active look out for these. Sciatica is a Healing SCIATICA with Acupuncture very painful acute condition which can come about suddenly which causes great agony and prostration and which virtually renders the patient immobile and is typically difficult to treat unless systematically approached The pain locates in the gluteal region, is usually one sided and radiates down the back or side of the thigh and leg. The severity of pain is intense. Rest seems to allay it to a degree but activity, especially after prolonged rest, aggravates it. Western Medicine calls it Sciatica because its path runs largely parallel to the sciatic nerve. The proper name for it in 5 element parlance is: Rising Liver Fire due to Metal Harassing Wood. 3
4 See the path of the Greater Sciatic nerve below. The pain begins at the origin of the nerve in gluteal region and can track along part or whole of the nerve. Atypically it may remain in gluteal area and just radiate locally to hip. Path of Gall Bladder channel runs parallel to Sciatic nerve, but more on side of hip, thigh and leg. Oddly while Wood is involved, pain tracks only along GB channel, never on pathway of Liver, the yin of Wood. 4
5 Genesis 1. A primary disharmony in Metal [LU LI] causing 2. A secondary disharmony involving Wood [LIV GB]. He red arrow below marks trajectory of the genesis. 5
6 Progression 1. A disharmony involving LU due to ancient, chronic, overwhelming Grief 2. LU yin diminished 3. Metal in distress [happens over years] 4. Metal overly controls Wood 5. LV Qi and Blood Stagnate 6. GB channel runs Empty 7. Heat seeps into GB 8. Empty Heat Rises [Wood is Wind] 9. Heat ignite and becomes Fire. Sciatica has arrived. What next? Establish LU disharmony [signs, symptoms, tongue, face color, pulse] Next do the same for LV Treatment 1st harmonize LU [tonify] 2nd - halter LV [sedate, reduce] 3rd - address whatever remains of pain 4th - prevent [keep LU in harmony] To reiterate the proper name for the disharmony in 5 element parlance is: Rising Liver Fire due to Metal Harassing Wood. 6
7 DIAGNOSTIC DESIGNS All diagnosis is seeing the design. Example below: PT has troubling spasmodic cough with sticky brownish or greenish mucus LU Yin Deficient - Pulse Choppy, Fast LU Deficient but all symptoms seem to be an Excess. Is this a real Excess??? What if it is a Deficiency? Sedating it would be disastrous Discussion Even if the cough which is distressing, severely spasmodic and forceful, seems to make the condition seem an Excess, it is not, being actually a Deficiency. If the condition is sedated using needles, one has caused a deficiency, in a Deficiency. A Deficiency is: Chronic with prolonged history Has relevant signs and symptoms which precede the condition Has signs of Cold Has signs of Yin Is more Internal than External Has a Slow or normally paced pulse Has pale tongue with thin or deficient coat PT fears Cold Beware of a Cold, Deficient condition masquerading as an Excess, Heated situation. 7
