2 Message from Stuart Rosenberg, President...we must continue building synergies to better address the demands of an ever changing health care industry Page 2 At Johnson Memorial Hospital we keep making strides to fulfill our most fundamental goal: to be a compassionate and transformative healing presence within our communities. Our commitment to providing accessible, affordable and high-quality care has remained steady throughout the years, and now, as a member of Trinity Health New England, we are fortunate to have the support and the strength we need to remain true to this promise. However, we must continue building synergies to better address the demands of an ever changing health care industry, precisely what our partnership with the cancer care team at Saint Francis Hospital and Medical Center has allowed us to do. By enhancing already existing oncologic services, we have been able to bring Saint Francis and Smilow Cancer Center at Yale-New Haven s expertise to western Massachusetts and north central Connecticut communities, positioning ourselves at the forefront of cancer treatment while bringing healing and comfort to cancer patients and their families. The milestones and projects described in the following pages speak of the commitment and dedication of our colleagues in reaching these goals, just a few examples of the many accomplishments that, with the support of our Trinity Health New England family and the generosity of local organizations such as Big Y World Class Markets, lay ahead of us. 3 Message from Jonathan Sporn, M.D., Section Chief, Hematology and Oncology This year we saw the staff at the Saint Francis Johnson Memorial Cancer Center in Enfield commit to a model of patient-centered care that combines the highest level of technical expertise with the unique personal touch for which both Saint Francis Hospital and Medical Center and Johnson Memorial Hospital are known. The integration of both Johnson Memorial and Saint Francis programs has resulted in the standardization of patient care protocols between the sites, consistent with those of our partner Smilow Cancer Hospital at Yale-New Haven. An important part of this has been the creation of the practice nurse role, to coordinate medical and supportive care activities for patients and families and serve as a contact point with the clinical team. This position is being filled by long-term staff member Julie Kadamus, R.N., B.S.N., O.C.N. Additionally, we have put in place both in Enfield and Johnson s main campus the same Epic electronic medical record system currently used at Saint Francis and at Smilow Cancer Center, which in turn has allowed us to streamline and benchmark our care delivery processes. Due to increases in our patient population at the Enfield practice, we have outgrown the existing 142 Hazard Avenue facility, where the Karen Davis Krzynowek Infusion Center is located. For this reason, most of our follow-up appointments are now taking place at 140 Hazard Avenue, across the parking lot from the Infusion Center, to allow the clinical team to focus their attention on patients receiving chemotherapy and other infusions. In the process, we have consolidated the management of the practices of Jay Thumar M.D., Susan Burroughs, M.D., and Pat Nugent, P.A. Additionally, Dr.Thumar is also seeing patients at Saint Francis, which has allowed him to supervise care for Enfield-area patients who participate in novel clinical trials delivered at Saint Francis. The Enfield area will soon be seeing positive outcomes from our union with Trinity Health New England. Construction will soon begin to replace the existing pharmacy with a state-of -the-art compounding unit that will increase the safety and efficiency of medicine preparation. In addition, renovations will take place at the radiation therapy component of the building, to allow replacement of our older linear accelerator with a new unit, along with CT-guided treatment planning that should be ready by July. Operation of this unit will be managed by Eric M. Van Rooy, M.D., Richard Shumway, M.D., Joseph M. Colasanto, M.D., and David Grew, M.D., all Saint Francis-based radiation oncology specialists. As we look forward to 2017, we begin to see how the shared components of cancer care continue to come together, to provide the communities we serve with the best quality and most efficient care. 5 Turquoise Takeover...Johnson Memorial Hospital colleagues wore turquoise to raise awareness of lung cancer as the number 1 cancer killer of women and men in the U.S. Last spring, Johnson Memorial held a Turquoise Takeover in support of the American Lung Association s Lung Force campaign promoting Women s Lung Health Week. On May 11, 2016, Johnson Memorial Hospital colleagues wore turquoise to raise awareness of lung cancer as the number 1 cancer killer of women and men in the U.S. Associates from the Cancer Center hosted a table at Johnson Memorial, where they had information and activities related to lung cancer education. 6 Health Fair Johnson Memorial Cancer Center was asked to educate attendees on the dangers of smoking, currently the leading cause of cancer related deaths in the country. Johnson Memorial Cancer Center was invited to participate in a substance abuse prevention health fair on September 2, 2016, at Stafford Middle School. Lt. Thomas Duncan, of the Stafford Resident Trooper s Office, organized the program to raise awareness of the state s opioid epidemic. Jane Tait, chemical dependency program, Alicia Corneau, cancer program specialist and Maria L. Palomares, B.A., outreach and patient advocate, represented Johnson Memorial Cancer Center. lungs simulating the lungs of a long time smoker. Lt. Duncan indicated, the decision to offer the presentation to grades six to eight was meant to reach students on the cusp of the teenage years, when they may be more susceptible to negative peer pressure. Around 350 students attended the event. Sixth, seventh and eighth graders visited the display to learn more about the dangers and consequences of smoking. Pedagogic tools included a big cigarette displaying its contents, a mouth model showing the consequences of smoking and chewing tobacco, and a set of swine Page 6

7 Fundraising Efforts And Donations Partners of Hope check presentation Big Y Seeds of Kindness In 2016, Big Y World Class Markets and its Partners of Hope program donated $12,000 to the Johnson Memorial Cancer Center. For the past nine years, Big Y s Partners of Hope has raised funds in support of breast cancer awareness initiatives, clinical research and treatment protocols. This program provides monetary resources to organizations currently providing breast cancer patients, their families, and caregivers with access to screenings, medical treatment, and counseling. Partners of Hope supports different patient programs across Massachusetts and Connecticut, allowing them to offer free-of - charge medical care, as well as much needed financial assistance. Blanket Donation The Auxiliary at Johnson Memorial grew over 70,000 sunflowers in Somers to raise funds for the Cancer Center s Patient Assistant Fund. The seeds were donated and the land was loaned by a local breast cancer survivor. Auxiliary members, along with help from local farmers, picked and bundled all five acres. Geissler s Supermarket sold the bouquets and donated 100% of the proceeds to Johnson Memorial. In addition, sunflower bouquets were sold at several local shops and businesses including Stafford s Farmers Market, Johnson Memorial Hospital and its gift shop. With this fundraiser the Auxiliary raised over $13,000 for the Johnson Memorial Patient Assistant Fund, which provides monetary support to qualifying patients undergoing cancer treatment. The intent is to alleviate some financial burdens such as transportation costs, grocery, pharmacy, and fuel expenses. With this fundraiser the Auxiliary raised over Inspired by her care at Johnson Memorial Hospital, Mariah Pulver decided to give back to her local hospital and community by making soft, fleece blankets with an inspirational message for patients treated at Johnson Memorial Hospital Cancer Center. $13,000 for the Johnson Memorial Patient Assistant Fund... 8 Support Services...psychosocial services are provided to cancer patients and caregivers to address physical, psychosocial, social, spiritual, and financial support needs that result from a cancer diagnosis. Page 8 American Cancer Society onsite providing oncology resource library free wigs, scarves and hats. Look Good, Feel Better a cosmetology workshop for women undergoing cancer treatment. Road to Recovery a program that offers patients transportation to and from treatments. Oncology social worker onsite cancer patients have access to an oncology social worker who can provide support and referrals for ongoing counseling, end of life issues, and community support resources among others, as needed. Breast cancer support group women affected by breast cancer come together to talk about common experiences and concerns, and to provide emotional and moral support to one another. This group meets quarterly. Energy therapy a gentle, hands on relaxation technique, using soft touch. Sessions are free of charge for cancer patients. Art therapy activities are designed to help patients discover their meaning and focus on self-expression, empowerment and the development of coping skills. Art therapy is offered monthly. Nutritional counseling available to cancer patients, the registered dietitian answers questions around nutrition and eating during cancer treatment. Giving comfort Partnership with McKesson Foundation to provide low income cancer patients with a Comfort Kit filled with the most requested and needed comfort items for patients undergoing cancer treatment. Johnson Memorial Cancer Center is the first Cancer Center in Connecticut to be a partner in this program. Patient advocate onsite assists with access to financial aid, Medicaid and Medicare enrollment, disability benefits, medication assistance, community clinics, housing, and smoking cessation programs among others. Also certified to assist in enrollment of health insurance through Access Health CT. Psychosocial screening psychosocial services are provided to cancer patients and caregivers to address physical, psychosocial, social, spiritual, and financial support needs that result from a cancer diagnosis. Services are available onsite or by referral. Lung cancer screening The American Lung Associa on recommends lung cancer screening with low-dose CT scan for people who meet certain criteria. Qualifying candidates must be current or former smokers (aged 55 to 79 years) with a smoking history of at least 30 years and no history of lung cancer.

9 Educational Events Oncology Dinner Lecture Breast surgeon oncologist Kimberly Caprio, M.D., was the featured speaker at the 8th Annual Oncology Dinner lecture, held at Johnson Memorial Hospital on November 29, Hosted by Jonathan Sporn, M.D., Section Chief, Hematology and Oncology, and the cancer program staff, the event was attended by physicians, clinicians and other health care providers who received CME credits. Dr. Caprio s presentation featured a discussion on the evolution and innovation in the surgical treatment of breast cancer. Dr. Caprio also discussed treatment guidelines, and provided an in-depth look at staging related to axillary node dissection. In addition, her talk explored the emerging field of oncoplastics, covering several controversial surgical management topics that initiated a rich discussion. Evaluations from attendees demonstrated positive feedback on both the program and Dr. Caprio as a speaker. Nutrition and the Cancer Patient Lynn Kula, R.D., met with cancer patients and survivors to discuss proper nutrition. Participants were able to ask questions and some even left with nutritional supplement samples. Page 9

10 Pa ent Assessment Algorithms All new patients at Johnson Memorial Cancer Center are assessed through various means, in order to identify their individual needs and determine the resources and services required to address them. One approach includes the use of psychosocial distress screening and navigation algorithms, which establish guidelines and illustrate the quality care provided to patients. Distress screening tool algorithm By standardizing the process followed by new cancer patients, the distress screening algorithm allows patient advocates to identify and explore any problems encountered by patients receiving chemotherapy treatment for the first time. 11 Navigation algorithm The navigation algorithm expands to also include the medical and support staff, to help patients navigate the system and find solutions to potential barriers that may impact the successful completion of their treatment. Staff continually monitor the barriers identified by patients at the Cancer Center and the community in general, so that our programs and services are developed with these issues in mind. Throughout 2016, transportation and financial needs continued to be the most challenging, requiring coordinated efforts between our patient advocate and patients in order to develop case-specific solutions.
