2 Testing If a patient tests negative for SIBO, but has symptoms, would you still treat? If you see a patient with low abundance and diversity on GI Effects testing, negative SIBO but with all the symptoms for SIBO, how would you treat the patient differently? What if a person has SIBO symptoms but testing fails to show it?
3 Testing Is it possible to have a false negative with a client doubled over with abdominal pain during the breath test? Do you use glucose or lactulose testing? Can dietitians do SIBO tests since we can t diagnose? Do conventional doctors know about SIBO or does this have to go through a gastroenterologist?
4 Testing What company do you do your cortisol and other hormone (estrogen/testosterone and metabolites) tests with? I would appreciate having recommendations on SIBO test products. We talked about generic testing of one, but not specific products to consider using. I am considering adding SIBO testing to my practice, but not sure about science and quality of testing options.
5 Test Prep Are thyroid medications ok to take prior to testing? Are Vitamin C and Magnesium ok to take if not being used for a laxative effect? Does fiber content of diet need to be adjusted for test prep? Can collagen powder (e.g., Great Lakes collagen hydrolysate) be okay to use in prep diet?
6 Test Prep Is there a prep diet for a vegan or vegetarian? Also, eggs can't be eaten when following a low-histamine diet. Is it okay to eliminate eggs from the prep diet?
7 Hydrogen/Methane What if you have elevated hydrogen, but not elevated methane? Would specific herbals be used for methane vs hydrogen SIBO? I have heard about a third type of SIBO that does not show up on hydrogen and methane tests. Can you speak to this?
8 Anxiety, etc. Is there a dopamine connection to gut? Please give more information on anxiety/depression connections. What if a patient doesn t have classic GI symptoms? Is SIBO still likely/possible when they have other symptoms like: muscle weakness, joint pain, brain fog, anxiety, insomnia, headaches, skin rashes, PCOS?
9 Antibiotics Does Rifaximin work in the large intestine too? If using just herbal antibiotics how long do you treat? Is it still the same 2-4 weeks? Can you comment on doses for children? Would you recommend herbal antibiotics be used alone or in combinations?
10 Antibiotics / Elemental Diet Case studies given used herbals and antibiotics. What kind of success can we using herbals and elemental diet alone? I ask on behalf of dietitians who cannot prescribe antibiotics. Can you repeat how long the elemental diet should last and give a comparison of your success rates with elemental diet vs. herbals vs. antibiotics? I gathered from your talk that a premade elemental diet product is preferred. I know Integrative Therapeutics makes one are there others?
11 Supplements Any need for SIBO patients to take activated charcoal and liver supplements? What is the dosage for Neem? Please comment on use of the Biocidin protocol for treatment of SIBO. When treating do you continue multivitamins?
12 FODMAP What do you do for continued prevention of SIBO other than prokinetics? And do you recommend a moderate FODMAP diet long term, especially for those patients who have recurrent SIBO? If structural issues you listed cannot be addressed, do they have to be on a long-term low FODMAP and keep doing antimicrobial rounds?
13 Cardio / Other Diseases Can high blood pressure be a side effect of SIBO? Have you ever heard that SIBO can trigger atrial fibrillation after meals? A family member has atrial flutters after eating that sometimes triggers atrial fibrillation. When you suspect NAFLD/NASH based on other tests-- when should you consider SIBO versus LPS?
14 Other Diseases / Parasites Lupus patients and SIBO? To clarify, do you still test those with Lyme's for SIBO or is it that the symptoms were the same? Any tips on differentiating parasite symptoms from SIBO? Sometimes it's hard to know where to start and what to test, especially if budget is limited.
15 SIFO / Probiotics How do you test for SIFO? Is someone has both, which do you address first, or both at the same time? So, what do you do for the candida or SIFO? I have a really hard time treating it even when patients are 100% compliant! But most patients aren't Any specific strains of probiotics for SIBO? Sporebased (like Megaspore) versus other types?
16 Sensitivities If someone feels better avoiding their food sensitivities (like dairy and gluten), but has symptoms when added back-- do you always consider SIBO? Or could there just be more fixed/permanent food sensitivities that they need to avoid long-term? Do you find sugar sensitivity and intolerance to spices also possible part of SIBO?
