5 Let s break this down into areas of priority for you so that we can establish some goals that need to be achieved: 1st Priority: We need to get your iron, B12 and ferritin pools increased. We will need to measure your iron level. Low iron levels reduce the conversion of T4 to T3. Insufficient iron levels effects the first two of three steps of thyroid hormone synthesis by reducing the activity of the enzyme thyroid peroxidase, which is dependent on iron. Thyroid peroxidase drives building iodine to tyrosine (amino acid) into the thyroid hormone T4 and T3. Low iron levels, which low ferritin reveals, alters the conversion of T4 to T3, besides binding T3. Low iron levels can increase TSH (thyroid stimulating hormone). The American Society of endocrinology recommends taking action at a TSH of 2.5. It is clear that there is an imbalance here but we need more labs to figure out what step to take. Obviously, the first step is to improve your iron deficiency and then figure out what happens. Your thyroid issues could solely be because of the iron and ferritin deficiency that you have. Ferritin 33ng/ml very low Ferritin is a component of a red blood cell. When ferritin is low you can feel fatigued and weak. Ferritin is also needed to the binding of thyroid hormones to cells so that it can stimulate metabolic function. Other signs include: Minor aches Abdominal pains Increased pulse or palpitations, Loss of libido Increased irritability and confusion Heartburn Numbness or tingling in extremities. Decrease in mental sharpness Paleness of the skin or eyes Brittle nails As mentioned above, with both iron and ferritin being low, this is a sign of iron deficiency anemia. When you are low in iron, you can feel tired, run down, not think clearly and restless leg syndrome. You can either not absorb the iron due to chronic inflammation in the GI tract or your diet may be deficient in iron.
6 MCV 100 is elevated: This means that you have a B12 deficiency and I would also recommend taking folic acid. When this is elevated combined with the low ferritin, it makes it difficult for your blood to carry oxygen, as it should. This can lead to excessive free radical damage. 2 nd Priority: We need to improve cortisol regulation: Cortisol 19.4 When cortisol is elevated it disrupts thyroid and sex hormone production. In addition, elevated cortisol causes more histamine to be released in the gut which then leads to food sensitivities and allergies. So cortisol will cause leaky gut and then lead to growing unfriendly flora such as candida (You are familiar with this). Also when elevated stress hormones occur over time, you can begin to get agitated mid day, and at the end of the day you may not feel as though you can think as sharply or have as good of critical thinking and coping skills. 3rd Priority Healing the Gut MEB value 25% with Eosinophils 14% both very elevated As you know there is a correlation to endurance athletes and GI problems. You have a history of GI distress. When cortisol becomes elevated, it fires the sympathetic nervous system. This causes a change in intestinal permeability. The gut becomes leaky and the result is an increase in inflammatory signaling through out the body. With more histamine being released your body becomes prone for more food and environmental intolerances to occur. 4 th Priority Build up protein intake through the day. Protein will help with anabolic drive and repair. It will also help with building thyroid hormones and brain neurochemicals. 5 th Priority Thyroid hormone balance TSH 2.43 (trending high) T4.98 (low normal) Free T3 2.5 (low) There are two issues driving the low thyroid performance and potentially a third. Correcting the first two may take of the problem but we will have to consider all of the following. Iron and ferritin deficiency
