1 Notice regarding Flu Vaccinations Conducted by the Kenpo Association Joint Project Influenza spreads every winter and an increasing number of people are therefore getting flu vaccinations. The realization is that flu vaccinations can prevent infection or reduce the risk of severe symptoms even if the person has been infected. The general incorporated association Toshinkyo is cooperating with the Kenpo Association in a flu vaccination project. In preparation for this year s flu season, the flu vaccination project will commence from October. Please take this opportunity to get vaccinated. Please choose one of the three ways listed below to receive a flu vaccination. Follow the instructions of the medical institution concerned to make a reservation and get a vaccination. - When making an application (reservation), inform them that you are a Toshinkyo Flu Vaccination user. - We may not be able to comply with requests depending on the production volume / stocks of flu vaccines. 3 Issuance of Vouchers 4 Getting a Vaccination (present your voucher + health insurance card) - You must enter the required information from your own health insurance card (be aware that any discrepancies between the entered information and the information on the health insurance card will invalidate the voucher!!). - Applications (Reservations) are not complete by only the issuing a voucher being issued. - Issue a voucher immediately after making a reservation (if a medical institution stops accepting reservation, the institutions then, it will be eliminated from the medical institution selection screen and thus issuance of vouchers disabled). - Ensure to present your voucher and health insurance card at the reception of the medical institution. - Submit your voucher to the medical institution. Do not bring it back with you.

3 5 Making User Copayments - A user copayment is the difference between the hospital vaccination: vaccination fee defined by the individual contracted medical institution and the amount of aid from the association stated on the voucher, and the payment shall be made at the reception of the medical institution concerned. When making an application (reservation) ensure to inform them that you are a Toshinkyo Flu Vaccination user. 2. On the day you get a vaccination, present your voucher and health insurance card at the reception of the contracted medical institution. Without your health insurance card, or if your health insurance qualification has been terminated by the day of vaccination, you will not be able to receive vaccination (in the case of on-site vaccinations, an application for use / applicant list instead of the voucher is required). 3. The vaccination must be received within the expiration date printed on the voucher (or application for use). In addition, the number of times you can receive a vaccination within the expiration period is, in principle, one time per person (junior-high students and younger are not eligible for group vaccinations). 4. The vaccination fee is the difference between the aid from the association indicted on the voucher (or application for use) and the vaccination fee shown in the list of contracted medical institutions (refer to the above downloadable list), and the payment shall be made at the reception of the institution concerned. 5. Input on a vaccination pre-examination form and other documents may be required.
