5 FOREWORD Recurring episodes of outbreaks of acute diarrhoeal diseases is a major concern in Chhattisgarh. This has affected large population primarily northern and southern districts of Chhattisgarh. The worst case in recent times happened in 2010 when remote and difficult to reach areas of Bijapur and Dantewada districts. Large numbers of people were affected and several deaths were also reported. The Directorate of Health Services, Chhattisgarh (DHS, CG) has taken several steps not only to respond to the outbreaks but also to nip such occurrences in the bud. The IDSP Chhattisgarh has an efficient surveillance system based on which the DHS, CG formulates protocols for case management, laboratory surveillance and treatment. UNICEF and the DHS, CG have started working together to develop a comprehensive guideline for outbreak management in the State. Working in close cooperation with the IDSP and Medical Colleges, UNICEF along with DHS, CG has attempted to identify the key areas that need to be addressed and also the key systemic linkages that are required to be strengthened. Hence the purpose of the Guideline is not limited to build an effective response framework but expands to plan for preparedness and capacity building of key functionaries. The outbreaks of Acute Diarrhoeal Diseases may be attributed, apart from contamination of drinking water, to poor sanitation conditions (inadequate toilet facilities), poor environmental sanitation (stagnant water bodies, inadequate drainage), and most importantly serious gaps in hygienic behaviour (personal, domestic and environmental). Thus it is imperative that a comprehensive framework to address the entire gamut of public health issues will be required. The present Guidelines draw relevant components of the WHO/UNICEF compendium on Management of Diarrhoeal Diseases. The antibiotic Therapy will be updated corresponding to emerging infections and their culture and sensitivity reports. These Guidelines covers technical measures which are both hospital as well as community based. The key users of this document are technical persons of both the Health Department and PHED. Therefore, successful application of these guidelines will warrant both the DHS and PHED to adopt and implement the suggested lines of action, keeping the community at the centre of all operations. I would like all concerned functionaries to ensure effective applications of these guidelines for management of acute diarrhoeal diseases at health facilities to mitigate the risk of such outbreaks.
