4 FIRST BOOK REGARDING THE FACULTY OF DENTISTRY TITLE I Only chapter General Framework Art. 1. This body of regulation is a regulatory instrument from Chapter Three (III), Part Four (IV) provided in the Organic Statutes, in reference to all Schools and Faculties of the University. This regulation contains general norms and the functioning of the Faculty of Dentistry in accordance with the mission, organization, functioning, government and administration of the Universidad De La Salle Bajío. Art. 2. The Faculty of Dentistry of Universidad De La Salle Bajío is governed and administered on the basis of this chapter and attending individual and collective power provided by the Organic Statues for Academic Boards. Art. 3. The Faculty of Dentistry, according to the Organic Statues and its academic regulations, has the following authorities: I. Individual authority. 1. The Director, who is the last responsible for the administration and operation of the Faculty and its clinics. 2. Academic coordinators for Undergraduate and Graduate programs, who are the first responsible for the operation of such programs. 3. The Clinics coordinator, who is direct responsible for the academic and dental functioning of Clinics. 4. The Clinics Administrator, who is direct responsible for the Clinics administration. He depends from the General Director of the Department of Finance and administration of the University. The Clinics Administrator is a link between this Department and the Clinics. II. Collective authority. 1. The Academic Board, who supports and decides for the Director of the Faculty in accordance to its own regulation. 2. The Clinics Board, who supports and decides for the Director of the Faculty and for the Clinics coordinator concerning the Clinics operation and functioning. 3. The Disciplinary Board, who establishes sanctions according to the Student Affairs Tribunal and in accordance with the Regulation of the Disciplinary Board of the Faculty of Dentistry. The Faculty of Dentistry may have other Boards who act either collectively or independently, whose role is to support courses, areas and specific functions of the Faculty's Academy and Clinics.

5 All job descriptions and other characteristics concerning the Faculty's Staff are detailed in the Human Resources General Organization Regulation and manuals, as well as in the Academic General Regulation and the Clinic Model. All these regulations have been approved by the authorities of the University. Art. 4. The main objective of the Faculty of Dentistry is to develop professionals capable of evaluating, diagnose and classifying systemic and dental diseases in human beings so that they can establish treatment plans for the stomatognathic system, in order to give back proper dental functioning and aesthetic. Art. 5. In addition to the capacity given in the Organic Statues, General Academic Regulations, the General Organization Regulation and other University norms, the Faculty of Dentistry can: I. Establish its own criteria and teaching models in order to improve academic quality and scientific improvement, in accordance to the University authorities. II. Coordinate teaching methods within the Faculty, Internship centers and other academic support centers, in accordance to the Academic program and its applicable norms. III. Administer all academic processes that depend from the Faculty of Dentistry. IV. Promote and follow-up exchange and mobility programs for its students, as well as Professional Internships. V. Help its students to find a place in the job market once they have graduated and analyze the evolution of the job market itself. VI. Promote cultural, social and other activities that can relate the Faculty of Dentistry. Art. 6. It is considered to be part of the Academic and Clinic staff: I. Course Professors, who are responsible for teaching basic, medical and dental science as well as clinics. II. Clinic Professors are responsible for assessing and following up each student's treatment plans during clinics at the same time patients are being studied. III. Tutors, who are responsible for the follow up for the learning of the student's clinics, as well as the student's treatment plans. IV. Project professors, who are responsible of developing academic projects that require additional time outside class hours. V. Chief of Department, who is responsible to establish in a collective way rules and procedures to the Clinics treatment plans, as well as to assess specialized areas. Assistants are graduated dentists who wish to work with a Course professor or a Clinic Professor in order to learn and improve his/her knowledge, abilities and competencies in its field of specialization. To be an assistant does not constitute any type of working relationship with the Faculty`s staff or with the University De La Salle Bajío. All job descriptions for academic staff, as well as all procedures for selection, permanence, promotion and retirement are determined by the Regulation for Academic staff and by the Internal Work Regulation for Professors and by the Mexican law. Art. 7. Administrative staff, dental assistants and secretaries at the Faculty of Dentistry are

6 those who perform direct tasks related to administration, dental assistance, maintenance of material, financial and human resources, maintenance of facilities and equipment as well as other assisting tasks. It is considered part of the Administrative staff: I. Dental assistants, who are responsible for support tasks to students and professors inside clinics. II. Cashiers, who are responsible for the receipt of payments for clinical treatments from patients. They depend from the Department of Finance and Administration. III. Secretaries, who are responsible for administrative assistance in several of the Faculties procedures. The category descriptions for administrative staff, dental and service assistants as well as the procedures for their selection, permanence, promotion and retirement are determined by the Interior Work Regulation for Person, Service and Administrative Staff of Universidad De La Salle Bajío and by the Mexican Labor legislation. Art. 8. The category descriptions for students, their admission, rights, obligations, academic norms, sanctions and other dispositions are established in the Regulation for undergraduate and TSU students of Universidad De La Salle Bajío. All dentistry students have to observe the following considerations: I. To show in all times a respectful attitude toward authorities of the Faculty and the University, academic and administrative staff as well as classmates and patients. II. To take care for the Faculty's facilities and equipment. To show respect for academic and administrative staff, classmates and patients. III. To maintain an ethical and professional attitude toward patients, including their needs and rights. IV. To respect security protocols regarding clinics and laboratories established by national and international norms. V. To pursue the academic program in its theoretic, pre-clinical and clinical phases. VI. To participate in tutoring sessions (weekly for R2 and R3, monthly for R1, regarding art. 45 of this Regulation). VII. To complete all scheduled on calls and community programs that are part of their academic program. Art. 9. Patients are those who come to the Faculty of Dentistry in order to receive dental treatment. Students, under professional supervision, are responsible of taking care for patients in order to learn through practice. Art. 10. A patient's care procedure, its medical classification, diagnosis, treatment plans, and monitoring are established by the Faculty of Dentistry in accordance to the Clinic coordinator and the Department coordinator of the Clinic Model and are detailed in this Regulation.

7 TITLE II CHAPTER I Academic general framework Art. 11. To ensure a proper functioning of all academic programs, the Faculty of Dentistry takes into consideration the General Academic Regulation for Undergraduate and TSU studies, the General Academic Regulation for Graduate studies and the General Regulation for Continuing Education. The Faculty of Dentistry also applies what is stated in the following articles of this Regulation. Art. 12. The Faculty of Dentistry operates in various Dental Clinics to ensure practical learning for TSU, Undergraduate, Graduate and Continuing Education programs. Students are bound to all aspects of the Clinic`s particular rules, which is described further in this Regulation. Any fault to these rules can have a negative impact in the student's academic performance and evaluation. Art. 13. Academic periods at the Faculty of Dentistry are ruled by its corresponding Regulations. Clinical work is considered part of the Academic period which includes the months of January and July. These months are meant for the student to keep up with their academic program, to fulfill academic prerequisites for the following period or to complete unfinished clinical work form the past period. Art. 14. There are three (3) disciplinary areas that build the Academic Program for the Faculty of Dentistry. These disciplinary areas are: I. Basic Medical Sciences, which includes basic knowledge on health topics in order to understand in a systematic way human structure, functioning, dysfunction as well as illness processes and its relation to the stomatognathic apparatus. II. Basic Dental Sciences, which include the knowledge of the stomatognathic apparatus which is very important to identify its functioning, composition, prevention and resolution of diseases. III. Clinics, which is a group of courses that contribute to develop competencies around patient care as well as around judgment practices over health-sickness procedures which make possible the design and execution of dental treatment plans. The word Clinics refers to the curricular space where the student can practice what he has learned inside the classroom. Courses that are part of the Dental Academic Program are detailed in the Curricular map of the Academic Program. Art. 15. All courses of the program are to be completed. The Director and Coordinators of the Faculty of Dentistry are responsible to survey this obligation. In the case of Clinics, it is the Second coordinator who will be in charge of this task. All reports on this subject have to be sent periodically to the Rector and Vice rector of the University. The development and evaluation of clinical courses will be applied in accordance to the Clinics evaluation and care models.

8 Art. 16. Learning options for Majors and Graduate programs at the Faculty of Dentistry are ruled by its own academic programs in accordance to the General Academic Regulation for Graduate studies and by the official applicable law. Art. 17. The Faculty of Dentistry provides Continuing Education programs as part of the General Academic Regulation for Continuing Education Studies. Art. 18. The International Dental Revalidation Course (IDRC) has its own regulation. Art. 19. Research at the Faculty of Dentistry constitutes one of the three main pillars of the Dental Undergraduate program. The other two pillars are classes (teaching / lectures) and service (clinics / community health programs). Research is operated by the General Regulation for Research of Universidad De La Salle Bajío. CHAPTER II Courses and accreditation Art. 20. Necessary conditions for a student to enroll in the Faculty of Dentistry, as well as procedures for equivalence, academic withdrawals and modification to academic studying plans are those established in the General Academic Regulation for Undergraduate and TSU studies and in the General Academic Regulation for Graduate studies. Art. 21. Accreditation of a given course, as described in art. 14 will have to meet the evaluation criteria concerning its corresponding academic program. An ordinary evaluation will be applied which includes several partial and final ordinary evaluations, as well as attendance to classes and lectures. Art. 22. Accreditation for a given clinic course will have to meet the evaluation criteria of the Clinic Evaluation model of the Faculty of Dentistry and all other norms in the following section. Art. 23. To ensure effective learning form clinic courses, all students of all academic periods will take part of clinic care groups, which will be coordinated by a senior student and the guidance of a tutor, in accordance to art. 45 and its subsequent to this Regulation. It is mandatory to all students to apply dental treatments instructed by the Tutor. Not completing a treatment is considered a severe fault. Students have to obligation to be on call in given periods of time. Not completing on call schedules can be sanctioned according to the table of sanctions of the Faculty of Dentistry. Art. 24. Accreditation of a medical or dental course, the student must present personally to all classes and lectures and to complete all tasks and procedures specified in its related clinic courses. The criteria to have the right to pass an ordinary course, and extraordinary course as well as the criteria of the number of opportunities to repeat a course are considered inside the General Academic Regulation for undergraduate and TSU studies as well as in the General Academic Regulation for Graduate studies. i. Clinics evaluation for undergraduate programs

9 Art. 25. Evaluating clinic dental courses consists in a dynamic valuation in which several evaluating agent participate, such as a tutor, clinic professors and other students. A series of evaluations are applied in order to help in the development of the student s professional profile in accordance to the Clinics Evaluation model which is described in the next section. Art. 26. The evaluation for each student for clinic courses is done through a work portfolio which has evidence of the abilities acquired. This portfolio consists of treatment records and of performance evaluations during the various stages of the course. At the end of the academic period (a semester) a final grade is given for each clinic course. During the partial evaluation periods, a grade is given in accordance to the evolution of the treatments stated inside the treatment record. As a measuring and control tool, this evaluation takes into account the minimum criteria for each procedure. Art. 27. Undergraduate students will obtain the results of their ordinary evaluations when they have accomplished all standard requisites. If they accomplish such requirements within the academic period, they will receive an ordinary grade. If they do not, a failing grade will be given and the student will have to recover pending credits during the month immediately following the last month of the academic period. This is, January and July. If they complete all clinical requirements during these months, a passing score will be given but as an extraordinary evaluation. If the student, after this month, has not achieved to complete all requirements, he may be able to repeat the course in the following academic period and will receive a second ordinary grade, and so on. Framework concerning course repetitions is stated in the General Academic Regulation for undergraduate and TSU studies. Art. 28. The evaluation process for dental clinic courses is divided according to the different dental procedures: I. Clinic I 1. Prevention 2. Diagnosis 3. Exodontics 4. Periodontics 5. Operative dentistry II. Clinic II 1. Prevention 2. Operative dentistry 3. Periodontic surgery 4. Surgery and Exodontics 5. Fixed prosthesis III. Clinic III 1. Prevention 2. Preprosthetic surgery 3. Periodontic surgery 4. Removable prosthesis 5. Total prosthesis

10 6. Endodontics IV. Clinics IV and V 1. Prevention 2. Geriatric dentistry 3. Pediatric dentistry 4. Complex patients The Faculty of Dentistry will publish the mandatory number of treatments that will be evaluated in all clinics to their corresponding academic period. Art. 29. The evaluation process of all clinical courses is responsibility of the clinic professor: I. The supervision, orientation and correction of the activities performed by the student during the clinic, based on each patient's treatment plan, authorized by his/her tutor. II. To fill all evaluation forms for each stage of the corresponding treatment and feedback the student on the matter. III. To submit learning evidences for its registration in the student's portfolio. IV. To keep the tutor informed on possible faults that might be important for each student's performance. Art. 30. During the process of evaluating clinic courses, the tutor is responsible for: I. Taking control over the student's portfolio. II. Keeping reported results from clinic professors, number and quality of assignments and procedures done by the students during clinic courses. III. Evaluate the advance of each student in the acquisition of professional competencies and achieving standard criteria. IV. Feedback all students regarding to their academic performance, paying special attention to those who risk to fail their clinic course. V. Report a general appreciation over the results of each partial evaluation period according to standard criteria. VI. Assign a final grade to the academic period based on the portfolio and the established criteria. Art. 31. During the evaluation process of all clinic courses, the Clinic Coordinator is responsible for the adequate functioning of all clinical services in order to guarantee Clinic professors their tasks regarding protocols, biomedical security norms, procedures and evaluation formats, as well as the guidance of the student. The Clinic coordinator has to maintain close communication with the Academic Coordinator and especially with the Clinic Administrative Coordinator. Art. 32. During the evaluation process of all clinic courses, the Academic Coordinator is responsible for maintaining communication with tutors and with the Clinic Coordinator regarding students' clinic performances and paying special attention to special cases in order to assure that theoretic courses have a positive correlation with practical courses. Art. 33. The process of learning evaluations and the assignment of ordinary and extraordinary grades of a given clinic course has three stages:

11 I. The revision and grading of a stage of a treatment, or the whole treatment at the end of a given period. The results will be registered in each student's portfolio. II. The appreciation of the results of partial evaluations. A scale will be used for this purpose and it will not affect the student's final grade. III. To give a final grade after revising and comparing all evidences contained inside the student's portfolio, and taking into account the criteria for ordinary and extraordinary periods in accordance to the General Academic Regulation for undergraduate and TSU studies. The descriptions of each of the stages mentioned above are contained in the Clinic Evaluation Model authorized by the Vice rector. ii. Graduate Clinic evaluation Art. 34. The evaluation for dental clinic courses for graduate programs corresponds to the nature of the graduate program and it is determined by the Academic Board. CHAPTER III Uniform Art. 35. It is mandatory for all students enrolled in the Faculty of Dentistry at Universidad De La Salle Bajío to wear a uniform when attending all courses unless a different instruction is informed by the authorities of the University. The uniform has to meet the requirements specified in the Dressing code of the Faculty of Dentistry. Art. 36. It is mandatory for all students to follow these complementary indications toward the use of the uniform: I. They will dress a uniform whose color corresponds to the class the student is enrolled in. It will have to be clean and in good conditions. In case of not meeting this, the uniform should be changed for another one which does present the characteristics mentioned. It is mandatory to wear a white gown over the uniform at all times. II. They will use a recyclable gown authorized to by the Faculty of Dentistry, in order to work inside clinics and laboratories. When used inside a clinic, the gown can only be worn once when taking care of a patient. If taking care of a new patient, the gown has to be disposed and a new one has to be used. When inside laboratories, the same gown can be used for several sessions but will need to be clean and in good conditions. III. They will use protecting eyeglasses, cap, gloves and facemask in all clinics and laboratories.

12 TITLE III CHAPTER I Social service Art. 37. Social Service is a commitment for the student when enrolling in any program form Universidad De La Salle Bajío. The purpose for Social service is to contribute to the students profile to develop commitment and social participation skills as a professional response to the most vulnerable sectors of society. All students will be asked to provide social service to a community in accordance to the University's criteria. To this purpose, assisting one of the course professors is not considered social service. Art. 38. The Social Services Department of the Faculty of Dentistry will determine as alternate activities to cover social service hours: I. Dental programs in local elementary schools. II. Community programs in urban and rural areas. III. Public and private health programs. IV. Dental programs that are organized and promoted by the University inside the campus or in other institutions. V. Any other activity considered by the Social Services Department. CHAPTER II Professional Social Service Art. 39. It is considered for Professional Social Service all activities required by the law and that students perform for social benefit. Such activities have direct relationship with the student's academic and humanistic profile considered in the academic program. Art. 40. The Professional Social Service is mandatory for a one year period and it is requisite for obtaining a Diploma, in accordance to the regulation of the Ministry of Education (SEP) and of the University. SECOND BOOK REGARDING DENTAL CLINICS TITLE I CHAPTER I General Framework

13 Art. 41. In addition to its own learning techniques, the Faculty of Dentistry provides clinics which are considered as a group of learning activities in different dental specialization areas, which can be performed inside or outside the Campus. It is a place where academic staffs, students, administrative staff and patients interact outside the academic field. Art. 42. The Faculty's Clinics have two roles: I. Academic: It supports the student's learning. II. Of service: 1. They offer immediate care to specific dental problems to those patients who do not wish to use the Faculty's Dental services. 2. They offer a more complete dental service to those patients who wish use the Faculty's Dental Services. All patients are previously evaluated systematically. Art. 43. To achieve the academic profile for each student, according to their academic program, the Faculty of Dentistry offers different clinics where the student can learn through his own procedures. This is done with the assessment of staff areas. CHAPTER II Clinic Care Model Art. 44. The Faculty of Dentistry of Universidad De La Salle Bajío has its own Clinic care model (MAC) to secure effective academic and administrative functioning which facilitates the learning and development of competencies as well as a proper care level for the patient in accordance to the mission, vision, ideals and organization of the Universidad De La Salle Bajío. Art. 45. The MAC works on the basis of multilevel groups who take care of one patient. These groups are formed by residents of all levels in accordance to the following categories: I. Resident 1 (R1): First to third semester students who take classes during mornings and practice as dental assistants during evenings. One to two R1 students can be in the same group. Their functions in the MAC group are: 1. Dental plaque control. 2. Clinic assistance. 3. Participate in monthly meetings with their tutor or the group. II. Resident 2 (R2): Fourth to sixth semester students, who have dental care practices during different periods of the day and whose credit hours for clinic course range between sixteen and twenty hours weekly. One to two R2 students can be in the same group. Their functions in the MAC group are: 1. The serve as support in the Urgencies, Admission and Diagnosis clinics: Urgencies admission, diagnosis and treatment. Patient's diagnosis and treatment.

14 2. Perform the following procedures. Prevention Periodontics Preprosthetic surgery Intra-alveolar and transalveolar extractions Fixed, total and removable prosthesis Endodontics Operatory dentistry III. Resident 3 (R3): Seventh and eighth semester students. Only one student per group. Their functions in the MAC group are: 1. Coordination of the MAC group along with their Tutor. 2. Patient care: Complex Pediatrics Geriatrics Tutors are responsible for the academic coordination of all MAC teams. TITLE II CHAPTER I Care process and Patient monitoring Art. 46. The following stages need to be considered to ensure optimal care and monitoring of the patient. I. Admission and diagnosis II. Development of a treatment plan III. Authorization of a treatment plan (Clinic Professor) IV. Treatment plan surveyance (Tutor) V. Treatment plan acceptance (Patient) VI. Treatment plan execution VII. Monitoring of the patient. The descriptions for each of the stages above are detailed in the Care Clinic Model. Art. 47. Patients are classified in accordance to the way they approach the clinic. These categories are: I. Voluntary (first time) patients: It is a first time incoming patient. The Clinic coordinator will assign a MAC team to take care of the patient. If the patient has come because of a student's invitation, the student's group will be taking care of this patient. II. Returning patients: It is a person who requires a dental treatment but has not come back to the Faculty's clinics within a year at least. He/she will have to go again through the Admission and diagnosis clinic to be evaluated systematically. Medical Clinic Dental Classification for patients can also be established according to the

15 type of treatment they require: I. Patient Type A. Simple: Preventive and constructive dentistry which implies prevention, surgery, periodontal treatment (first phase) and intra-alveolar extractions. II. Patient Type B. Complex: Requires complex periodontal treatment, this is, surgical or endodontic treatment, transalveolar extractions and either fixed, removable or total rehabilitation. III. Patient Type C. Special Age: It is either a pediatric or a geriatric patient. IV. Patient Type D. One procedure: It is a patient who does only need attention to one specific problem which can be solve immediately. He/she can be taken care whether in the urgencies area, clinics or surgery rooms. CHAPTER II Clinic special regulations Art. 48. Working hours for Clinics are established by the authorities of the University, in order to not interrupt academic and clinical activities. Clinic users are expected to suspend their work when asked so, because of any technical problems or because they should finish the tasks assigned to a given schedule. At all times, clinics have to attend to this chapter. i. Hygiene and Security regulations Art. 49. All academic and administrative staff, as well as students and patients that enter the Clinic's facilities of the Faculty of Dentistry must meet hygiene and security regulations presented in this section, when applicable. If not meeting such regulations, they will be asked to leave the facilities. Art. 50. It is obligation for the Professor to survey the student on the following specifications: I. Wearing complete uniform and a recycled disposable gown according to the class he/she is enrolled in, both in good conditions. The robe has to be changed when taking care of a different patient or when presenting saliva or blood. It also has to be taken off when exiting clinics and laboratories. II. Not wearing any object in their arms and hands, such as rings, bracelets, watches, etc. III. Having cleaned and washed hands. Short nails without nail polish. IV. Use hand gloves up to the arm wrist and cover their nose and mouth with a facemask. V. Hand gloves have to be eliminated once exiting clinics and laboratories. VI. Facemask must be changed when taking care of another patient considering its effective use is of forty (40) to sixty (60) minutes. It also has to be changed when having used a high-speed instrument, a syringe or a ultrasonic dental scaler. VII. Use eyeglasses or professional eye protectors, which in case, have to be

16 used over normal eyeglasses. VIII. All tools have to be sterilized and have to be opened in front of the patient, including high and low speed tools. The procedure to verify if a tool is sterilized can be performed by dental assistants. Tools such as medical staples, bone files and surgical burs must be sterilized with steam (autoclave) not with disinfecting solutions. IX. Bags given by the Faculty are the only ones that can be used for sterilizing purposes. Perforated and covered cassettes can also be used, as long as they are provided by the Faculty. Close metallic boxes are not permitted. X. Exchanging working tools during any procedure is prohibited. XI. Before the beginning of any care activity, lines have to be purged, surfaces must be disinfected, and protection barriers over triple syringes, ejectors, handles, dampers, x-ray as well as light-cure equipment shutters must be assured. After finishing, covers have to be taken away, tools have to be secured in rigid boxes, all surfaces have to be disinfected once again as well as lines have to be purged, including the triple syringe line. Armchairs and benches also need to be disinfected. XII. All patients have to rinse the mouth with a dental antiseptic before any treatment. XIII. All patients have to wear protecting glasses, which have to be previously washed and disinfected. If the patient wears eyeglasses, these must be replaced, if best, being kept under the protection glasses. After treatment, protecting glasses have to be washed and disinfected. XIV. During any treatment, the patient's chest has to be covered. When a given procedure is considered to be surgical, the patient's head must also be covered. XV. When taking an impression, supplier observations on its disinfection must be followed. XVI. Any work that is being sent or received from the laboratory must be disinfected with antiseptic solutions. XVII. A lead apron should be placed over the patient when taking x-rays, taking care that it covers the neck and shoulders. Its use is mandatory. It is prohibited to stay inside the x-ray room when taking the x-rays. XVIII. Clean hand gloves are required to study x-ray photographs, whether they are processed with manual or automatic boxes. X-ray photographs have to be placed in a disposable container from which they will be processed and in which contaminated wastes will be later be placed. Once the x-ray photographs are done, all wastes and hand gloves must be disposed in order to continue with the patient's treatment. XIX. When filling in a form, the person taking notes has to begin with the purpose of the treatment, and in the shortest way possible, the description of the characteristics of the treatment, which will serve as a control for the continuing of such treatment. The person taking notes will also have to take care of filling in if the patient had to present any other health / physical study. The student number has to be noted as well as the tooth number, receipt number and name of the professors who are in charge of the clinic, as well as the patient's signature as authorization for his/her treatment. XX. To keep well all x-ray photographs, take them and process them correctly. To make sure they are classified and identified with a green note, which also has been filled correctly. XXI. All kind of waste has to be disposed in the corresponding cans. The student may use a plastic bag to put there all the working waste and after the treatment, he/she should separate it and then dispose it inside the corresponding cans.

17 Art. 51. It is prohibited to do several tasks at one with several patients using the same tools and the same material. Clinic facilities are designed to give attention to one patient at a time. Art. 52. It is prohibited for three (3) students to be together with a patient who is sitting on an armchair. Art. 53. It is prohibited to perform any treatment if the professor is not present. Art. 54. Only on call students and those who are under training assignments can stay inside the clinic facilities. Art. 55. Before beginning any clinic work, the student has to check the conditions of the facilities, room, equipment and tools with which he/she will be working. Any imperfection has to be reported to the Dental assistant along with a revision format. Art. 56. It is the student's obligation to know, follow and respect all regulations concerning characteristics, techniques, procedures and other specifications regarding treatments. Art. 57. Any professor can suspend the student's activities in any of the following cases: I. When the student does not have the tool required, or when the tools he is using are not well sterilized. II. When the student does not show to have enough knowledge on the subject. III. When the student does not follow hygiene security protocols. IV. When the student does not follow any other aspect of this regulation. Art. 58. The student must notify his/her professor when any complications arise toward a treatment. Art. 59. By any means, patients cannot receive any care outside the Faculty's dental facilities. Art. 60. No person should receive any dental care if not being properly registered and admitted as a patient at the Faculty of Dentistry. Art. 61. No patient is to be left alone inside the clinic. He will always be guarded by a student of the Faculty. Art. 62. All dental rooms have to be in order and clean, without clothing, books, bags or any other articles that are not required for a treatment. A disposable area has to be considered besides the working table. It is not allowed to any person to try to modify or correct the space and the well functioning of an instrument or equipment. All tools and equipment have to be clean and in optimal conditions for its use. Art. 63. Thirty (30) minutes before finishing clinic tasks, the student has to lead the patient to the exit, clean the room and pick up any personal belongings.

18 ii. Administrative regulations Art. 64. All academic and administrative staff, students and patients that enter the clinic facilities of the Faculty of Dentistry must obey and meet all administrative requirements established in this section of the regulation. Art. 65. It is obligation for the academic staff, students and dental assistant to present punctually to their patient's schedule. Art. 66. The student who performs any treatment is directly responsible for that patient and the guarantees implied in the clinic service. Art. 67. All patients can only be assisted by the students who have been assigned by the Professor. Art. 68. During clinic hours, students will have a five (5) minute margin to arrive to his working schedule. If he/she does not arrive, another student will be assigned to perform his/her tasks. a. File control Art. 69. A file is a legal document in which detailed information about a patient is registered on the matter of his/her illness, health situation and anything that has to do with a proper diagnosis and its treatment. Art. 70. The purpose for keeping a file is to have the exact information about the patient's health conditions, his/her diagnosis, treatment plans and clinic evolution. The Faculty of Dentistry of Universidad De La Salle Bajío has three (3) types of files: I. Adults (15 years old and older) II. Infants (0-15 years old) III. Mobile unit Art. 71. All files can only be used inside the Faculty of Dentistry. To be able to take out any file, authorization is required by the Clinic coordination. Art. 72. For legal reasons, only professors are authorized to write medical receipts, analysis requests for laboratories, dental care letters and sick leaves. Art. 73. A file is generated for the first time in the admission, diagnosis and urgencies room. Once the file exists, the treatment plan will be written down by the student under the supervision of a clinic professor who will sign in authorization. This treatment will be surveyed by the tutor. The patient will sign in acceptance for the treatment and its cost, in accordance to art.46 of this regulation. Art. 74. If a patient is sent to a different health service, this has to be noted down in the clinic evolution section of the file.

19 Art. 75. Technical and legal characteristics concerning files and disease control are those established in the Mexican Official Regulation NOM-168SSA and NOM-013-SSA as well as any other consideration in the Regulation for the Archive of the Faculty of Dentistry of Universidad De La Salle Bajío. Personal information criteria concerning these norms have to be observed as well. b. Archive Art. 76. The Archive is the center who has the custody of the files as well as other relevant dental documents and material for the Clinic coordination. Art. 77. Lending and revision of files are to be done under the procedures of the Regulation for the Archive of the Faculty of Dentistry. c. Laboratory Art. 78. Laboratories are centers that support the learning process through a practical approach. Art. 79. The functioning of a Laboratory is done under the Regulation of the Laboratory for Practices and Simulations of the Faculty of Dentistry. d. Treatment fees Art. 80. The Faculty of Dentistry is authorized its patients for the treatments they receive or any other service the Faculty provides. Art. 81. All treatment fees are charged before they receive any attention. This fee is established by the Clinic's Administration. The money from treatment fees is used for the maintenance of the facilities and the equipment. Art. 82. Dental treatments that do not meet the conditions for health, functioning and aesthetic have to be repeated. Art. 83. Students cannot charge patients for their services. Art. 84. It is prohibited to use facilities or equipment in order to charge for personal gain. e. Dental material and equipment Art. 85. The Faculty of Dentistry will provide all necessary material and equipment for the delivering of dental care. All material and equipment is provided upon requirement from professors to the Clinic Coordinator, in accordance to the basic chart for authorized material, which is revised each semester by the Clinic coordinator. The student, at the beginning of his academic program, will receive a list of the basic and complementary material.

20 Each student is responsible for acquiring and bringing in personal material, which is specified inside the lists, published by the Faculty of Dentistry. This equipment has to be sterilized inside the Sterilizing centers of the Faculty of Dentistry for every intervention. Art. 86. In case that a special instrument or material is required for a given treatment, it is the patient who will pay for its cost. Art. 87. The student can only use authorized equipment. In special cases, the student will ask for permission to use his own material to the clinic professor. If necessary, authorization to the clinic coordinator is also needed. Art. 88. When the student requires any material or equipment. He/she will be responsible for it. In case of losing it or breaking it, he/she will have to pay for it. Art. 89. Equipment and material that belong to the Faculty of Dentistry has to remain inside the facilities where they are officially kept. CHAPTER III Admission and Diagnosis process Art. 90. All Admission and treatment systems refer to a series of procedures established by the Faculty of Dentistry to execute correctly the learning approaches to practical dentistry. i. Admission process Art. 91. The admission system for the Clinics of the Faculty of Dentistry of Universidad De La Salle Bajío is described in this section and has to be observed under MAC's statements of this Regulation as well. Art. 92. Any person who asks for dental care at the Faculty of Dentistry must register and pay the corresponding fee. Art.93. Once registered, the patient will enter the Admission and Diagnosis Department to evaluate in a systemic and in a stomatological way, in order to establish a treatment plan. Art. 94. Academic staff and students will have to fill the information taken from the anamnesis records; they shall perform clinic examinations and fill all registration forms. If necessary, an extended revision and laboratory exams shall be practiced to arrive to a systemic and stomatological diagnosis of the patient's situation. Art. 95. The treatment plan must show that it has considered all systemic, psychological and social conditions to the patient, dental expectations, risk levels and preventive activities as well as orientation needed for the patient. The treatment plan should also mention all possible modification aspects of the plan and compensating actions if a systemic condition should be associated.

21 The treatment plan should specify care priorities, which have to be observed because they take into account the patient's needs and dental conveniences. Art. 96. When entering the Admission and diagnosis area, the Clinic Coordinator will assign the patient a medical and dental classification in order to determine which MAC team can assist him better. This classification is also important for the MAC team in order to establish a treatment plan. Once they have a proposal for a treatment, it has to be authorized by the Professor and surveyed by the Tutor. When necessary, the treatment plan may require a specialist confirmation. If any Professor considers it is suitable to modify the original authorized treatment plan, he may change it and will have to note such modifications and sign it, as well as to notify the Tutor about this situation. Art. 97. The MAC team receives a patient and is responsible for its care. The person who is in charge to assign teams to attend patients is the Tutor. The Tutor will take into consideration the complexity of the patient s treatment and if he/she has to go through a surgical procedure. ii. The treatment process Art. 98. The treatment process at the Clinic of Universidad De La Salle Bajío is described in the following section. Other parts of this regulation and of the regulation for MAC have to be considered as well for the treatment process. f. Treatment Art. 99. Once the treatment plan has been approved by the clinic Professor, the Tutor and the patient, the team will move on to its execution following closely indications stated under the order of importance of the steps for the treatment. Art In each visit, the student will take notes in the patient s record in order to summarize the procedures he applied according to the treatment established for that patient. g. Supervision Art All treatments that have been performed by a student need to be supervised and classified by the clinics professor. The clinics professor will revise each steps of the treatment and will verify that the patient receives proper care. Art Control over all clinic activities is measured by two aspects: I. Intermediate activities: Are daily activities. This activities may be graded by the clinic professor o any other authority specialist in the field. II. Final activities: It is the final activity to end a treatment. This last activity should preferably be evaluated by the clinic professor who authorized the treatment. In case the clinic professor is not available at the time of this last activity, any other clinic professor may evaluate and give authorization for the conclusion of the treatment.

22 Additionally, the Tutor will monitor each patient s treatment and will register all finished treatments as well as future appointments after a treatment is concluded. h. Conclusion of a treatment Art The conclusion of a treatment can happen in two situations: I. Partial conclusion: A treatment is partially concluded when only one of the specialized stages of the treatment has been fully completed. The patient will sign his treatment record at the end of each stage of the treatment. II. Total Conclusion: A treatment is totally concluded when all stages of the treatment have been fully completed. The patient will sign his treatment record and will be schedule for further controls. In both cases, before the last appointment, all patients will have to attend an information session for instructions on this process. i. Cancellation of a treatment Art The cancellation of a treatment takes place when the patient does not continue to receive any service from the Faculty of Dentistry. When this happens, the Faculty of Dentistry holds no further responsibility for the dental health of the patient. Cancellations can occur in two situations: I. Voluntary cancellation. When the patient, for any reason, does not wish to continue with the treatment, taking own responsibility for his/her dental health. The professor will take note this event in the patient s record, and will inform the patient which stages of the treatment remain incomplete. The patient has to sign as consent. II. Administrative cancellation. When the patient can no longer receive medical attention for the following reasons: a. The patient has missed more than three (3) appointments. b. The patient has not followed instructions carefully from the students or the professor. c. The patient has not fully paid his/her treatment. d. The patient behaves improperly inside any facility of Universidad De La Salle Bajío. e. Monitoring Art At the end of a treatment, the clinic professor and the patient will schedule a monitoring appointment for control and evaluation of the treatment. The clinic professor will also give instructions to the patient as maintenance guides for the treatment. The tutor will attend all monitoring appointments.

23 CHAPTER III On call Art On call are mandatory activities scheduled by the Professor or the Clinic coordinator from the Faculty of Dentistry. Regulations regarding on call schedules and working tools are those included in the On Call Regulation. Art During on calls, students must behave properly and with dignity toward other members of the campus. In all cases, students must obey the following conditions: I. To have a clean presentation. Men should shave and keep short hair. Women with long hair should have it drawn back. II. To wear a clean uniform, without wrinkles and in good conditions during the time the student is inside the facilities of the Campus. III. To attend on calls with complete, sterile dental equipment. To take care and keep spaces and surfaces clean. IV. At the end of any procedure, the student will arrange the room and dispose all wastes correctly. V. At the end of an on call, the student will request the on call supervisor to sign and seal their authorizing document for that on call. Art Students are responsible for following all procedures related to their schedules on call. In case of an emergency, the student is responsible to ask for a change of schedule to the clinic coordinator. This modification has to be written in its corresponding format. The original on call has to be replaced by another student. Art It is an obligation to all students the attendance and permanence during on calls. Art In case a student has scheduled two on calls in different facilities, it is his/her responsibility to ask the Clinic coordinator a schedule adjustment. Required formats have to be filled, signed and sealed. Art All On call changes have to be presented no later than three (3) business days with its corresponding formats, including the information for the student who will be replacing the on call. Art Any student with pending training appointments cannot leave the facility during on calls. Art When an exam is schedule at the same time of an on call, the student has to ask permission to leave the facility for the time the exam is scheduled. If the exam begins at the same time as the on call, the student may arrive to their on call once they have finished the exam.
