10 NONALCOHOLIC STEATOHEPATITIS AWARENESS (NASH) WEEK Not much information about this topic Your liver (or the liver of a loved one) may be in trouble and you don t know it A lot of misinformation Copious scientific evidence does not support the popular viewpoints A lot of money is being made by people and products who aren t really helping us

11 WHY YOU SHOULD LOVE YOUR LIVER The liver plays a central role in all metabolic processes in the body. Your liver: Filters toxins from blood Synthesizes proteins your body requires to function Manufactures biochemicals needed for digestion Helps regulate glycogen storage and blood glucose levels Disposes of old red blood cells Plus over 500 other functions You can t live without it, literally There isn t a machine that can replace it (no dialysis machine for liver)

12 FIBROSIS AND CIRRHOSIS Fibrosis Buildup of scar tissue resulting from ongoing inflammation and liver cell death Risk factors include fatty liver (NASH, high triglycerides) and insulin resistance Symptoms typically none without testing Cirrhosis Permanent scarring of liver tissue Severe impairment of liver function Increased risk of liver cancer End stage of chronic liver disease Treatment is liver transplant

13 LIVER ENZYMES AND OTHER TESTS Liver enzymes Like cardiac enzymes, released when damage occurs AST (SGOT) Present in other cells in the body, not just liver Acute or recent damage ALT (SGPT) Acute or recent damage More specific to the liver Albumin Protein made in the liver Low albumin indicates chronic liver injury Prothrombin Time (PT) and INR Shows acute and chronic injury Measures time for blood to clot Liver makes most of the proteins required for blood clotting Most sensitive test for liver disease

14 SIGNS YOUR LIVER ISN T HAPPY Before diagnosis of type 2 diabetes there is a long, silent scream from the liver. Often no symptoms Elevated liver enzymes Fatigue Risk factors High triglycerides High fasting blood glucose

15 HOW THIS STARTED FOR ME 2012 Complain to doctor about fatigue 2013 Continue complaining about fatigue, blood tests show high triglycerides and elevated liver enzymes 2015 Diagnosed with moderate sleep apnea May 2016 Referred to gastroenterologist for evaluation following blood tests indicating high insulin, elevated liver enzymes September 2016 Ultrasounds and biopsy indicate liver fibrosis, referred to liver specialist in Houston where people go to get new livers

19 THE CALORIE BALANCE PARADOX Over a period of about 20 years, I gained about 40 pounds This is an average of 2 pounds per year Given that it takes 3,500 excess calories to result in gaining 1 pound, this equals 7,000 excess calories per year 7,000 calories per year / 365 days = 19 calories per day Conclusion: I should have eaten one less bite of food per day and I wouldn t have gained weight

20 THE LAZY DOOFUS THEORY Medical advice for weight loss: eat less, exercise more Assumption: you continue gaining weight or don t lose it so you are a lazy doofus sloth who needs to eat less, exercise more

25 BODY FUEL STORAGE Glycogen stores carbohydrates Mostly stored in liver and muscles 480g typically stored X 4 kcal/g = 1,920 kcal (~1 day) Protein Stored primarily in muscles 6,000g typically stored X 4 kcal/g = 24,000 kcal (~ 12 days) Fats Stored in adipose tissue and liver (even if you are thin ) 12,000g stored X 9 kcal/g = 108,000 kcal (~54 days) Equal to 90,000g of glycogen (due to water content of carbs)

26 INSULIN RESISTANCE Eating carbohydrates causes your body to release insulin Insulin tells your body to store the sugar somewhere, anywhere, to get it out of your bloodstream Too much glucose is toxic in your bloodstream Insulin prevents your body from burning fat Constant exposure to insulin makes your body less sensitive to insulin
