2 Abstract Living with arthritis is challenging. I have been plagued by the condition in my right knee, my left hip and in both of my trapezium joints in the hand. Fortunately, I have practiced Pilates for the last 18 years and it has helped me to utilize my core muscles to alleviate reliance on the arthritic joints, thus alleviating the pain in those joints. In February of 2017, surgery removed the right trapezium (thumb) joint. The following research paper studies the recovery from the surgery. It includes analysis of the hand region, the resultant weakening of the affected muscles, and the prescription for recovery through physical therapy and my own Pilates practice. To maintain a strong, flexible, pain free, and happier me, I have employed Pilates: an exercise program with mental focus, breathing patterns, concentration and strengthening techniques that benefit the body in multiple ways and in multiple areas of the body. 2

5 The trapezium joint is the 1st CMC (carpometacarpal) joint and it allows the thumb to pivot, swivel and pinch as well as compress and move heavy loads. This joint, like all others, is covered with shock-absorbing cartilage. Most of the muscles that move the wrist, hand, and fingers are located in the forearm. These thin, strap-like muscles extend from the humerus, ulna and radius and insert into the carpals, metacarpals, and phalanges via long tendons. The muscles on the anterior side of the forearm, such as the flexor carpi radialis and flexor digitorum superficialis, form the flexor group that flexes the hand at the wrist and each of the phalanges. The tendons of these muscles pass through a small corridor in the wrist known as the carpal tunnel. The muscles of the upper arm are responsible for the flexion and extension of the forearm at the elbow joint. Flexion of the forearm is achieved by a group of three muscles the brachialis, biceps brachii, and brachioradialis. These flexor muscles are all located on the anterior side of the upper arm and extend from the humerus and scapula to the ulna and radius 5
6 of the forearm. Additionally, the biceps brachii operates as a supinator of the forearm by rotating the radius and moving the palm of the hand anteriorly. Case Study As a 55 year old woman who has been active her entire life, slowing down and succumbing to arthritis pain has been difficult. I had been able to avoid surgery (with the exception of a resurfacing of the left hip ball joint), and have been able to maintain hobbies such as cycling, gardening and walking. Pilates has been a source of relief and comfort as well as a necessary tool in keeping my body strong and flexible. My regular Pilates practice has included weekly Mat sessions as well as apparatus sessions. Unfortunately, for the last several years, I have suffered from increasing thumb pain and was diagnosed with arthritis in both trapezium joints. When I was told that surgery was inevitable (for my right/dominant hand), and it was scheduled for last February of 2017, I prepared myself by continuing with exercises within the several blocks of the BASI program. Of course, I included plenty of core work. The surgery involved attaching a band of Kevlar from the first metacarpal bone to the second, which allowed for internal healing (scar tissue) to grow. It also allowed for the second metacarpal bone to control the movement in the first metacarpal bone (thumb). The suggested recovery period was 3-6 months. 6
7 After the surgery my right hand and arm were immobilized in a brace for six weeks. I was unable to do anything, literally! I couldn t tie my hair back, nor tie my shoes, and I couldn t cook or clean ( ). The surgery caused the whole arm region to weaken and my muscle tone declined greatly. Specifically, the brachialis, biceps brachii, brachioradialis, flexor carpi radialis, flexor carpi ulnaris, as well as the smaller extensor muscles throughout the hand and wrist, were all weak, scrawny and without definition. Even though the surgery was done to my right thumb, the connective tissues in between the fingers, and up and down my arm were all affected. This was due to the immobilization of the entire arm in the brace for those first six weeks. To determine which recovery program would be right for me, I needed to establish my goals, analyze my current limitations, and then proceed with a program with those goals in mind. Specifically, I needed to build up those muscles and restore mobility to my thumb and hand muscles. Undoubtedly, the physical and massage therapy I received after the surgery helped immensely. But more effectual were the focus and efforts of the Pilates program I practiced below. 10 Conclusion After right thumb surgery to alleviate years of arthritis pain, I am healed and able to open jars, grasp small objects, tighten a screwdriver and cook and clean again. Although my recovery would have been successful without Pilates, I know that staying focused on a program that included the many body blocks/regions allowed me to retain good posture, and to have a body that is balanced and strong. At first, my weakened state was depressing. It is difficult to be armless and helpless. Reliance on others took away my pride and humbled me. I learned to accept help and to refocus my energy on what I could do instead of what I couldn t do. I could work my abs, my legs, my hips and stretch. I could do some exercises with my left hand, losing the worry that I would become imbalanced. During my Pilates sessions, it was necessary for my instructor to utilize an array of modifications such as fuzzy straps, stickies on the handles, and assisting with placement of the equipment. We also modified the way the hand was placed for Mat work. Using a fist to absorb the weight, or stretching the hand with the digits extended in an arc was easier than the flat palm. Using blocks to rest my forearm on was also a great modification. Doing some of the exercises on my left side and forcing it to become stronger, made me realize that I didn t need to be as reliant on my right. And over time, my right arm and hand did heal. The muscle tone returned, and I wasn t in pain any longer. I am grateful for my body and the BASI Program for guiding me back to health and strength. 10
