Vaccine Refusal: A Qualitative Look Into Parents and Providers Knowledge, Attitudes, and Beliefs Nikki Wagner, MPH Jason Glanz, PhD Kaiser Permanente Colorado (KPCO) Institute for Health Research

Parent Focus Group Objectives To talk with parents who choose not to vaccinate their children about ways to improve communication about vaccines

Parent Focus Group Methods-Recruitment Identify parents of children <4 years old Did not receive immunizations Immunization status verified with medical chart review Separate potential participants by geographic regions (4) Boulder, Denver, Aurora, Highlands Ranch Participants sent recruitment letter with opt-out option Phone recruitment contact

Parent Focus Group Methods-Focus Group Size limited to 2-6 participants 1.5 hours Audio Recorded $25 Safeway gift card incentive

Parent Focus Group Discussion-Introduction 5 minutes to open discussion What are some of the reasons parents are concerned about vaccinations?

Parent Focus Group Discussion-Transition minutes Brought discussion to parent s personal decision making process When did you first become concerned? What sources influenced your decision? Of these sources, which do you trust the most and why?

Parent Focus Group Discussion-Main Topic 40 min-1 hour Parent/Provider relationship How would you like the discussion about immunizations to go with your provider? When discussing immunizations what would you like your provider to say to you?

Parent Focus Group Discussion-Wrap Up Summary Is there anything else you would like to add? Do you have any recommendations for improving these discussions in the future?

Parent Focus Group Participants Boulder: 3 parents Arapahoe: 5 parents Denver: 3 parents Aurora: 2 parents

Parent Responses-Introduction Concerns about Vaccines: Vaccine Safety Chemicals Adverse events Live viruses Quantity of vaccines at one time Low risk of vaccine preventable disease, high risk of adverse events from vaccine Low trust in government and providers

Parent Quote-Introduction I know there s also a conflict of what if the doctor s office gets paid per shot so you don t know that their intentions are really for the best of your child.

Responses-Transition When Decision made Before pregnancy, when making birth plan Sources used Evidence of Harm, Dr. Kirby Generation Rescue, Jenny McCarthy The Vaccine Book, Dr. Bob Sears Most Trusted Sources Acupuncturist or Chiropractor Websites

Parent Quote-Transition I ve been to a couple of chiropractors, and the information they have is, it s very good. The Dr. Sears book by far was the best I read as far as giving the right information.

Responses-Main Topic Patient/Provider immunization discussion Present both sides Don t tell parents what to do, give options Parents are continuously evaluating this decision

Parent Quote-Main Topic Ultimately this is your [the parents ] decision, but we want to be here to support you and make sure that you have all the information, both pros and cons, or something. Sometimes I feel like the physicians themselves haven t done any personal research on the topic themselves and they re just going off what they learned in school that this is how it is.

Parent Focus Group Summary Parents were well informed Internet was the main source of information Do not believe physicians are well informed Vaccinating children is a continuous decision making process Parents don t want to be told what to do, they want options

Provider Focus Group Objective The provider focus group was designed to elicit physicians opinions of vaccine refusal, how they convey the benefits and risks to patients, and to generate ideas for approaches that would be helpful in working with parents who refuse vaccines.

Provider Focus Group Methods-Recruitment Recruited Kaiser Permanente Colorado Pediatricians Presented study information at Pediatrician meetings Contacted Pediatrician Physician Chiefs Focus Groups done with all interested Providers

Provider Focus Group Methods-Focus Group Limit size to 5 physicians 1 hour Audio Recording Food incentive

Provider Focus Group-Discussion Points Effective Messages How do you talk about and what information do you find helpful in: Vaccine safety Vaccine preventable diseases Handouts and Education materials Do you use them? Is there something you don t have that would be helpful? Dr. Bob Sears AAP sign off Physicians getting paid for vaccines

Provider Focus Group Participants Westminster: 5 Highlands Ranch: 3 Lafayette: 4 Aurora: (4 scheduled) Boulder: (3 pending scheduling)

Physician Responses All acknowledged a problem Differing views on best approaches Don t bother they won t change their minds OR Bring it up every time they come in Will follow any schedule they bring in as long as they are getting vaccinated OR will push for ALL vaccinations every time Important to be up on new concerns OR not worth the time

Physician Responses I actually don t have a problem at all with the delayed schedules. I think if parents are saying you want me to get them all done and we re getting them done in eight months rather than six, I don t have a problem with that. I can still be your doctor, we can still be friends, we can still like each other, but I m probably going to talk to you about this every visit and just ask you where you re at and try to answer your concerns

Physician Responses Varied approach depending on patient Scare tactics Stories about own children Stories about experience with vaccine preventable disease Statistics Handouts Dr. Bob s book Not many had read All wanted handout with short description and talking points

Physican Focus Group Summary Varied approaches in dealing with parents who are delaying or refusing vaccines Need multiple approaches based on patient decision making processes. Website or class to give patients more information would be helpful

Focus Group Limitations Only to be used to generate hypothesis Biased Sample Small number of participants

Discussion Points Vaccine Refuser versus Choosing not to Vaccinate Declining Vaccines Vaccine Hesitant Presenting both sides

Physician Comment I think as soon as we use the word refuser we ve alienated ourselves.
