2 Reducing Alcohol Related presentations & admissions at the Royal Albert & Edward Infirmary, Wigan Wrightington, Wigan & Leigh NHS Trust Ashton, Leigh & Wigan Primary Care Trust, Wigan Council, Greater Manchester West Mental Health NHS Foundation Trust. This project looked at the top 50 patients who were responsible for the most admissions specifically relating to alcohol issues. When a patient from the top 50 attended, consent was sought by the alcohol specialist nurse team to be referred to community alcohol treatment services. Between October 2010 March 2011 there was a 60% reduction in average monthly readmissions by the top 50 patients.

3 Smokefree Mental Health Today I Can Heart of Mersey ChaMPs, CMTA, MerseyCare, Five Boroughs Partnerships, Cheshire & Wirral Partnerships Aims to support individuals to reduce their tobacco intake or quit through training provision, policy & care pathway development. Forged multidisciplinary partnership working to ensure consistency, quality & equitability of information & service delivery. A consistent pathway has now been adopted by all trusts.

4 Stick & Stones 5 Boroughs Partnership NHS Foundation Trust The only anti-stigma campaign to be specifically requested and driven by service users and carers, following research about the words people use to describe people with mental ill-health & learning disabilities. Working in partnership with service users, this project collected 106,553 pledges in 12 months to reduce stigmatism & utilised social media platforms to generate celebrity engagement

5 Get Growing Project Landlife Knowsley 5 Boroughs Partnership, National Wildflower Trust Get Growing aims to improve understanding & selfmanagement of health amongst Knowsley residents through engagement in creative edible gardening. The project has engaged over 4,000 people in 12 months, including 300 schoolchildren; 1500 adults/family members, 300 health practitioners. Developed an inspirational best practice community garden in one of the most deprived neighbourhoods in Knowsley.

6 Good Work- Good Health Improving life chances in Greater Manchester New Economy Manchester Greater Manchester Public Health Network, Manchester Health at Work Group, Pathways Community Interest Company, MMU, University of Salford, Association of Greater Manchester Authorities, Business in the Community, Acas The project identified a number of actions that would improve health outcomes for individuals & secure economic growth in Greater Manchester. Engaged individuals, service providers & employers in designing services that support people with chronic conditions to remain in work. Secured 460,000 Euros for a joint project with Spain & Germany to help keep people with mental conditions in work.

7 Men s Health Twit-A-Thon In response to the hard to reach Wigan NHS Ashton, Leigh & Wiganman, NHS ALW delivered a health engagement campaign with a difference. A Health Improvement Team member undertook a 12 hour cycle ride throughout the borough whilst broadcasting tips, advice & opportunities via Twitter. Direct engagement throughout the journey alongside media engagement, allowed 50,000 males to be involved within the project in some capacity.

8 Improving Access to Flu Vaccination for Pregnant Teenagers Tameside Hospital NHS Foundation Trust NHS Tameside & Glossop, NHS North West, North West Health Protection Agency. Tameside Hospital recognised that pregnant women were at increased risk of serious complications from flu. The project was evaluated by measuring the impact on vaccination uptake. Between October 1 st 2010 & February 29 th 2011, there was an uptake of 85.9% which is double Tameside s average uptake for pregnant women.

9 Detect Cancer Early Campaign Greater Manchester Public Health Network & Greater Manchester & Cheshire Cancer Network Macmillan Cancer Support Social Marketing & Community engagement campaign aimed at the South Asian community across Greater Manchester to raise cancer awareness. Five 40-second advertisements featuring cancer survivor stories in both Hindustani & Sylheti which were aired on a mixture of South Asian Channels and had a National reach. Four Cancer surgeries were held in each target area, these events reached over 200 people across the communities.

10 North of England Tackling Illicit Tobacco for Better Health Programme Smokefree North West Fresh Smoke Free North East, Trading Standards North West, HMRC, North West Police Constabulary The Programme aimed to reduce the supply & demand for cheap smuggled & fake tobacco within the North of England. Within the North West the number of smokers admitting that they buy illegal tobacco is down by 2%. Awareness levels increased by 17% The Programme has also been cited as best practice in the National Tobacco Control Plan.

11 Five to Strive Project Creating Confident Communities Cheshire West and Chester Borough Council Cheshire West & Chester Council developed this partnership project to address health inequalities through action on the social determinants of health. Course participants were encouraged to set themselves three wellbeing goals & offered the support of a volunteer mentor

12 WINNER House 4 Life: Linking Health & Housing Salford City Council, NHS Salford, University of Salford Created a sustainable urban housing solution that supports a beautiful, vibrant, healthy & prosperous Salford. Explored ways of promoting healthy family living; responded creatively to issues highlighted in the Marmot Report

13 WINNER I Can Make It Happen Aspirations Project NHS Bolton NHS Bolton, Healthy Schools, Bolton Council: Positive Contributions, 5-19 Service, Children s Services, Teenage Pregnancy, University of Bolton, Bolton College, Home, Training for Today, Youth MPs, Bolton Wanderers FC, playing for Success, Octagon Theatre, Middlebrook Retail Park, Emergency Services, Tonge action group, Cultural Connection, Local businesses. Reduces teenage pregnancy rates in the area. Encourages children & young people into positive activities. To support children & young people to live happy healthy informed lives.

14 WINNER Creative Alternatives Creative Alternatives NHS Sefton, Sefton Metropolitan Borough Council Delivers a structured & varied programme for adults experiencing depression stress and/or anxiety as an alternative to medication & traditional healthcare interventions. Employment, volunteering & literacy skills are also enhanced. Creative alternatives has become a model of best practice.

15 WINNER Tackling Health Impacts of Worklessness Pathways Community Interest Company Central & Eastern Cheshire Primary Care Trust Produced a programme of support for young people most removed from the labour market. Created 750 six month fixed term jobs for unemployed young people 65% of young people involved in the project found mainstream employment

16 WINNER InfoLink Champions Central & Eastern Cheshire Primary Care Trust Public Health Central & Eastern Cheshire Primary Care Trust, Cheshire East Council, Age UK, Bowel Screening Unit Mid-Cheshire Hospitals Foundation Trust, Stop Smoking Service East Cheshire Trust Supports the development of community based health champions. The pilot of InfoLink saw 100 InfoLink Champions trained across 18 different organisations including pharmacy staff, probation service, Citizens Advice Bureau.

17 WINNER Enriched Opportunities Age UK West Cumbria Age UK Barrow, Cumbria Care, Cumbria Public Health, Adult Social Care, Cumbria County Council The project aims to develop a sustainable model of intervention, using person-centred approach techniques, community engagement and partnership working. 78% of those taking part in exercise sessions are more active in their daily lives, while 77% increased self-esteem. 70% of participants in nutritional activities reported improved availability of healthy foods.

18 WINNER Imagine Your Goals Everton in the Community Mersey Care NHS Trust Utilises the power of football as a positive vehicle to tackle a number of issues relating to mental illness. Established Merseyside s first mental health football league with two divisions catering for 16 teams. 15 Premier League clubs have adopted the scheme nationwide, with thousands of participants now benefiting from the scheme.

19 WINNER Active Lifestyles Tackling & Addressing Physical Inactivity Manchester City Council The primary aim of the Active Lifestyle s Service is to improve the health & wellbeing of all Manchester residents. From a benchmark of zero, there are 5,000 registered members on the Active Lifestyles programme. There are now nearly 250 Active Lifestyles sessions each week in varying locations.
