3 PROGRAM OVERVIEW Transformations Drug & Alcohol Treatment Center s Veterans Recovery Program The Veterans Recovery Program is tailored to meet the needs of military Veterans and their families. These brave men and women were subjected to a level of physical, emotional and mental stress unlike most civilians. Those increased stress levels have led to Veterans experiencing some of the country s highest rates of substance abuse, depression and suicide. There are also the issues of family dysfunction, divorce, isolation, trauma related nightmares, trust issues and reintegrating back into civilian life. Often when the demands of life after military service are not effectively met, substance abuse becomes the most popular and deadly avenue of escape in dealing with the experiences and traumas of the past. At Transformations Treatment Center we support our Veterans and their families in the reintegration process by clinically addressing these devastating and often unseen wounds. Our unique program has successfully treated Veterans nationwide, setting them on a path to success. Programs Offered We specialize in individualized treatment serving the young adult population (ages 18 to 29), the adult population (ages 30 and older) and a Christian program for those seeking faith based treatment. We employ a holistic therapeutic approach with small group sessions and 12-step meetings. We also offer recreational activities and even a recording studio for those interested in music. Our family program helps educate the patient s loved ones about the disease of addiction so they are better equipped to provide support. Treatment Model Our program has an integrated treatment model to help Veterans with co-occurring Post-Traumatic Stress Disorder (PTSD) and substance abuse disorders. We have successfully treated military Veterans whose lives have been affected by combat or lengthy non-combat deployment. When clients enter the Veterans Recovery Program they can receive the following services: Detoxification Individual Cognitive Behavioral Therapy Group Cognitive Behavioral Therapy Stress management Trauma management and resolution Relaxation and meditation therapy Grief and loss healing Anger management Rebuilding family relationships Suicide awareness and prevention Psychological trauma substance abuse disorder treatment EMDR trauma therapy Anxiety management Holistic nutrition with dieticians, natural supplement protocol and chiropractic services Upon graduation, clients have a plan in place to help cope with stress management, improving their physical, mental and emotional wellness. 5 PROGRAM LEADERS Dr. Maureen Esposito, PhD, CAP, LMHC, LMFT Executive Vice President of Clinical Services Dr. Esposito, has over 20 years of extensive experience and knowledge of the drug and alcohol field. As the daughter of a WWII totally disabled Veteran, her caring and empathy run deep for the Veteran community, both past and present. Dr. Esposito and her highly-trained and qualified staff are ready to help our deserving Veterans return to living happy and productive sober lives. Dr. Esposito received her Bachelor s degree from Hofstra University in New York, a Master s Degree in Psychology and a Doctorate Degree in marriage and family therapy from Nova University and trained at Hanley Hazelden for her Certified Addiction Professionals Accreditation. Today, more than ever, we are challenged to meet the needs of our Veterans, especially young men and women dealing with the aftermath of recent wars. PTSD, depression and anxiety, as well as re-entry into civilian life are just some of the issues facing our brave Veterans. - Dr. Maureen Esposito Carlos Farina, MS, MCAP, ICADC, CHT First Responder/Veterans Program Director Mr. Farina is a Veteran who has served in the law enforcement profession and understands the demands that military Veterans and first responders face every day. He has treated hundreds of Veterans and first responders who have sought mental health and substance abuse services in private as well as at community mental health and recovery centers. Carlos holds a state of Florida Masters level certification as an addiction professional; he is a board certified trauma specialist; and a board certified hypnotherapist by the American Board of Hypnotherapy. In addition to his strong academic background, Carlos was a staff clinician with the Broward County Sheriff s Department Psychological Services Division, where he was a Psychological Profiler, Hostage Negotiator, and rendered mental health services to the men and women of law enforcement. During his professional career, Carlos was also a visiting Instructor at the Broward County Criminal Justice Institute and a Certified Trainer for the Criminal Justice and Standards Commission. Asking for help can sometimes be difficult, but losing our Veterans to a treatable disease is tragic. - Carlos Farina Jawad Daud, MD Medical Director, Addiction Psychiatrist Dr. Daud is a board-certified in Psychiatrist with specialty training in Addiction Medicine. In addition to his strong training in the field of addiction, he specializes in the treatment of individuals suffering from mood disorders including depression and bipolar disorder, as well as anxiety disorders such as generalized anxiety, panic, social anxiety, obsessive-compulsive disorder and PTSD. Other areas of concentration include patients suffering from co-occurring psychiatric and substance use disorders. He has abundant experience in the medical detoxification of patients suffering from alcohol, benzodiazepine, and opioid addictions. Additionally, he enjoys working with patients in the rehabilitation setting. Dr. Daud completed his internship and residency training at the Miller School of Medicine at the University of Miami. This training took place at multiple hospitals including Jackson Memorial Hospital and the University of Miami Hospital. Additionally, he takes pride in having been trained at the Miami Veterans Affairs Medical Center where he proudly served our Veterans by providing multiple services including residential PTSD and substance abuse treatment, outpatient PTSD services and inpatient and emergency psychiatric services. Furthermore, while in training he was chosen for the highly respected position of chief resident. He went on to complete a specialty fellowship in Addiction Psychiatry which was also at the University of Miami. In addition to his medical training, Dr. Daud completed a postdoctoral research fellowship at the National Institute of Mental Health at the NIH in Washington D.C. Being trained at a Veterans Affairs hospital, I have the utmost respect for our Veteran clients and am grateful for the opportunity to serve. - Dr. Jawad Daud Josh Weaver, MS, LMHC, CAP, EMDR Certified Therapist Mr. Weaver obtained his master s degree in Mental Health Counseling from Nova Southeastern University in His focus as a therapist is working with clients suffering from chronic relapse, severe traumatic history, medical trauma and emotional, physical and sexual abuse. Josh has special training in trauma treatment using EMDR and rapid trauma resolution. I see how trauma has affected many returning Vets and having specialized in trauma treatment I offer a unique therapeutic approach. - Josh Weaver

7 HOUSING Integrity Way VIA Luxury Two-bedroom Condos Known as VIA, this garden-style complex has 36 apartments featuring a swimming pool and pond views. All condos include private bedrooms and baths, washers and dryers, updated kitchens with stainless steel appliances and granite countertops and cable TV on flat-screen TVs. Recreation Leisure time is important. Regular activities are held including trips to the gym several times per week, a running club, yoga, table tennis tournaments, food & fellowship BBQs, cooking classes, Friday movie night and more. Weekly events include shopping at a local supermarket as well as outings to area parks and beaches. We also coordinate special outings tailored specifically for our Veteran clients. Additionally, clients attend AA, NA, Celebrate Celebrate Recovery, as well as religious services if requested. Non-institutional Setting At Transformations Treatment Center we do our best to ensure that our Veteran clients are as comfortable as possible during treatment with a tranquil, home-like setting. Phones with free nationwide calling and a computer lab are accessible. A clubhouse, adjacent to the pool, is also on site with a community kitchen, as well as a picnic area and basketball court.

9 SUMMIT DETOX Summit Detox is part of the Transformations Treatment Center family. It is a 20-bed medical facility that provides a safe and comfortable detoxification from alcohol and drugs including heroin, opioids, and benzodiazepines. We are located in Boynton Beach, Florida, near the Intracoastal Waterway and the Atlantic Ocean and serve patients 18 years and older from across the country in our safe and nurturing environment. We are the first step toward living a healthy and sober life. We offer a medically supervised stable detoxification, including a vitamin regimen and innovative natural remedies that ensure you are not over- or under-medicated during this phase. This enables clients to be in their best frame of mind to accept continuing treatment. Depending on the substance, there are a variety of medications administered to detox the client safely and effectively while minimizing their discomfort. We provide a caring staff, 24/7 medical supervision and comfortable, nicely appointed rooms with amenities, to allow for a solid foundation for recovery. Some of our unique offerings include genderspecific floors, healthy food options, 12-step meetings at night, smoke breaks and regular chiropractic treatments. At Summit Detox, we have the most competent, caring, and committed medical staff. All clients are treated with respect and compassion. Our staff takes pride in making each client feel welcome and cared for. Treating an individual s needs from both a medical and clinical approach is fundamental to giving every person the care they deserve. 11 NEXT STEPS Military Veterans are ordinary people with extraordinary determination. Referrals If you have a patient in need of help with addiction, contact us today. Our Admissions staff will work with you or your patient and family members to make the process as easy and seamless as possible. We work with you to ensure that the proper authorizations, paperwork and procedures are followed. Transferring to a treatment facility can be confusing or concerning to clients, so our Admissions Coordinators are here to help them through the entire process and make it as organized and stress-free as possible. Admissions Process Call Transformations Drug & Alcohol Treatment Center at An Admissions Coordinator will explain and review the treatment program with your patient. A screening is done that usually lasts approximately minutes. Transportation to our facility and a check-in date will be arranged by our Admissions Coordinators. Once your patient arrives, they will be welcomed into the caring and competent hands of our staff.
