4 Program Overview Purpose Integration of peer staff as an equal member of the multi-disciplinary teams (MDT) at the Adult Psychiatric Support Services (APSS) programs located at Stockton Blvd and Bowling Drive. Provide, coordinate and support positive outcomes to consumers in reaching their goals Target communities which have been historically been underserved, especially the threshold language communities identified by the Division 4

13 Mental Health America of Northern California Cast from the shackles which bound them, this bell shall ring out hope for the mentally ill and victory over mental illness (Bell s Inscription) Our mission is to improve the lives of individuals living in diverse communities through advocacy, education, research and culturallyrelevant services. We provide peer services that foster recovery and reduce stigma and discrimination. We are a consumer-directed and consumer-operated agency. 13
14 Peer Partner Program Philosophy: Goals: Provide peer recovery support services to adult clients receiving mental health services (1) encourage self-advocacy (2) foster independence (3) improve individual service outcomes (4) provide education and resources for the client, family and community (5) eliminate discrimination and reduce stigma for those seeking services 14
15 Mental Health America of Northern CA & the Peer Partner Program Our agency employs four culturally-diverse peers who all have lived experience and are dedicated to the promotion of wellness and recovery in their own lives, and in the lives of those they serve Among us, our language capacity is Spanish, Cambodian and English We are divided equally between the two clinics, working as a cohesive team with Hmong Women s Heritage Association s Peer Partners Inter-agency client success story Personal recovery background 15
