These tasks include having confidence to deal with medical management, role management and emotional management of their conditions. 10 Living Well It consists of 6 highly participative structured workshops, held in community settings where participants learn processes and skills to better manage their own health and become actively engaged in their treatments. The programs is for persons with chronic conditions. The workshops are facilitated by two lay leaders who have been trained by two Master Trainers at Stanford University. 10
11 Living Well The Workshop covers topics such as: Communicating with your doctor Physical Activity Relaxation Managing Symptoms Eating Well Effective Problem Solving Goal Setting 11
12 Living Well Successful participants of both programs will: Learn new skills to better manage their health Improve their outlook on life Learn new problem solving skills Be empowered 12
13 Living Well Sponsoring organizations will: Increase community recognition Provide a service to the community Build partnerships with other community agencies Create healthier and happier members 13
14 Living Well How to implement the program: Stanford Licensing Agreement Cost of Program License Master training Lay leader training Course materials Administrative costs 14
15 Living Well Recruitment of Lay Leaders Persons that have a chronic disease or someone that lives with someone that has a chronic disease Leaders do not have to be health care professionals Leaders must be able to follow a programmed script Recruitment of Participants Persons that have a chronic disease or someone that lives with someone that has a chronic disease Persons need to be able to attend at least 4 sessions 15
