2 Vivienne Malcolm Solicitors for Older People 1. Welcome, Introductions & Apologies The Chair welcomed all to the first forum meeting of 2017 and invited colleagues to introduced themselves. Apologies were noted as above. 2. Review of Previous Minutes 14 th December 2016 The Chair apologised for not having the minutes from the previous meeting to present to the forum. These will be carried over to the next meeting. 3. Training Needs of the Third Sector and the Integration Joint Board (IJB) Claire Bernard attended the meeting to bring to the attention of the forum the following: She is attending an event at Glenbervie Care Home on 9 th February that was to start the conversation to ensure that all front line staff in health & social care received standardised training, regardless of whether working in the public, independent or third sectors. CB had checked and there were still places available and if any colleagues have questions they can forward these to CB by . The following questions/comments were raised: o It was felt by one forum member that this was short notice to raise the event and ask for input from the forum. CB stated that the event has been promoted in previous two CVS Falkirk e-bulletins. o It was questioned whether there was a need to map the training currently offered by third sector organisations and could this be supported by the IJB. CB raised that through her involvement with the Workforce Organisational Group, led by Rob Haden, she stresses the importance of training opportunities for the third sector being fed via her to be included in the CVS e-bulletin or raised at the forum. CB revisited an item raised by the Chair at the previous meeting about ensuring that updates from the IJB were shared with the forum. CB attends the Strategic Planning Group (SPG) and a paper providing a summary of decisions of the IJB is provided to the SPG. CB intends that from feedback on the IJB to the forum will be based on this summary paper. CB also reiterated that all IJB papers are shared on their website, and also on the dedicated Community Care & Health Forum webpage on the CVS Falkirk website ( o One forum member raised that they had read the IJB papers for the meeting on and there were two papers that she felt would be of interest to colleague. Both of these papers can be accessed on 2
3 the IJB webpage on the Falkirk Council website ( Homecare and Community Care Contract Self Directed Support Implementation Progress Update The final issues raised by CB was that there is going to be an evaluation happening of the My Day My Way pilot and that Louise Cross who is leading the evaluation is looking for input on the evaluation from those who were directly involved in receiving support, or their carers. If anyone is interested in being part involved who knows someone who might be, please pass the details on to CB. CB has offered Louise the opportunity to present the findings of the evaluation to the forum at a future meeting. 4. The Four Priorities of SOLD Firstly the Chair raised that the seven third sector forums that CVS Falkirk facilitates have now been recognised as the formal structure for the whole of the third sector, including community groups and representatives, to have their views shared into the Community Planning structure. The Chair continued that at all forum meetings moving forward there will be time taken to look at the four priorities of the Strategic Outcomes and Local Delivery (SOLD) Plan, particularly in relation to the work being done under the two sold outcomes that related to the forum. The four priorities of the SOLD Plan are: Improving mental health & wellbeing Maximising job creation and employability Minimising the impact of substance misuse Addressing the impact of poverty on children with these four priorities, this last priority will be given the greatest attention. These four priorities must be considered in all six of the SOLD outcomes. The two outcomes that relate directly to the Community Care & Health Forum are: Outcome 4 Our Population will be Healthier Outcome 5 People Live Full, Independent and Positive Lives within Supportive Communities The Chair finished by stating that to inform the forum of the work that is being planned within these two outcomes, he intends to focus on one at each of the next two forum meetings (9 th May and 8 th August). Ann McLaughlin, Lead Officer for Health Promotions with NHS Forth Valley has agreed to attend the forum meeting in May to discuss the work being planned under Outcome 4, and he 3
4 would be working to identify the best person to speak about Outcome 5 at the August meeting. The SOLD Plan is now available to download as a PDF file on the Falkirk Council website 5. Overview of Partner Organisation Strathcarron Hospice Susan High gave a presentation to the forum on the Community Development work that takes place at Strathcarron Hospice: SH started by providing a little of the 35 year history of Strathcarron Hospice, and sharing that she feels that people don t necessarily know what Strathcarron do. SH then provided an overview of the services offered through Strathcarron Hospice: o In patient unit with 24 beds 50% of in-patients are discharged again as in for symptom management, rather than end of life care o Day hospice with 20 patients each day. This offered more of a social therapy approach and gives carers time off o 11 Community Nurse who cover the whole of the Forth Valley plus bits of Fife and North Lanarkshire when patient is registered with a doctor within the Forth Valley. These are not hands on Community Nurses but provided advice to patients, their families and their support structures. o Lymphoedema Service state of the art regional service o Hospice at Home auxiliary nurses who specially trained to deliver hands on care at home, primarily during last 2 weeks of life. Service is starting to do a little more work around supporting discharge from hospitals as a difficult area. If people are in last stages of life and want to go home, a care package is difficult to put into place. Hospice at Home nurses provide cover until care package is set up, but in many cases they cover until end of life. o Bereavement Service: Adult service only when person who is lost has been using one of Strathcarron s services Children s bereavement service takes referrals from anywhere and are for children who are bereaved, whether they have had contact with Strathcarron or not. This services is supported by Children in Need. o Chaplaincy 2 chaplains that offer more spiritual support rather than religious. Also do weddings, and are currently planning a wedding 4
5 o Complementary therapy: offer therapies to in and day patients, and also visit people at home. Also offered a range of services to carers. o Befriending o Education: specialist training department that offer a range of different training packages. SH provided an overview of Strathcarron s funding: o About 40% from NHS o Donations o Shops o Legacies o Trust funds (such as hospice lottery o Strathcarron needs 11,000 per day to operate. Befriending service o Set up in partnership with St Andrews Hospice in Lanarkshire. Strathcarron focused on volunteer befrienders to help to reach people who wouldn t access hospice services. o Currently 70 volunteers, some supporting 2 people (some support patient and carer; some support 2 separate people. o Initial mandatory training, with ongoing training which volunteers can choose whether to take up training opportunities. Main focus of training is to help volunteers be comfortable talking to people about end of life. Strathcarron use both internal and external resources to ensure volunteers get the training they need. o Strathcarron has support mechanisms in place for volunteer befrienders when people they are supporting pass away. o Befrienders support people during the last year of their life and will work with people of all ages and regardless of the issue. o Strathcarron is currently recruiting for new volunteer befrienders. Finally SH talked about the Living Right Up to the End project: o The project has two aims: To find ways to help people with long terms conditions to manage their lives and maintain control through a range of challenges that they and their families face in the last year of life To enable more individuals to think about and plan ahead for the end of their lives at an earlier stage in their illness o Project has focused on two areas: Denny, Dunipace and Bonnybridge Alloa and Hillfoots o Asset mapped what services are already available in the two areas; and consulted with nearly 350 people (those with long-term conditions; their carers; health professionals and third sector) and asked them two questions: 5
6 What is important to you when you are living with declining health? What are the barriers to thinking about and making plans for the future? o Answers to these questions have been themed around a small number of key issues o Project now moved on to interventions based around the question: What can Strathcarron do to help communities to support people with long terms conditions and their carers in the last year of life? o Currently working on 2 nd stage of consultation to identify definitive options for interventions that Strathcarron can try. Looking to work with people who they will present a menu of options and see which people think will work better. Looking for support from colleagues over what they think might help communities. Also if any colleagues know people living in the Denny, Dunipace and Bonnybridge area who would want to be involved, can people contact SH susan.high@nhs.net o Strathcarron hope to test two interventions during current funding period but hope to have funding extended. They hope to have firm interventions in place by June. o SH finished by asking if colleagues had any questions: A forum member asked about higher level befrienders, and whether these could be tested? SH hopes that this could be tested but admitted that development has taken longer than expected due to Strathcarron being new to Community Development Another colleague questioned whether Strathcarron has a resource sharing agreement with other third sector organisations, when others have spare resources. SH responded that transportation is not currently done by Strathcarron but they are eager to explore further relationships with third sector colleagues. This could help to share some of the key messages around planning for end of life. It was asked whether Strathcarron hold events to promote end of life planning. SH responded that whilst they currently don t, it is something they are eager to explore. Possibility of holding an event in a community centre or similar to ensure that key information about available resources are shared not only with third sector colleagues but also wider communities and working with other services, such as Solicitors for Older People. It was mentioned by another member that there is a need to break down cultural barriers, 6
7 as Scottish men don t talk about illness and bereavement, as well as making the approaches practical. SH hopes wants to utilise existing resources to get the message out to the public. 6. Update from CVS Falkirk The following updates were provided by the Chair on behalf of CVS Falkirk: CVS Falkirk are working in partnership with Falkirk Council to host the Falkirk Funders Fayre on Tuesday 7 th March at Bowhouse Community Centre. Flyers were available for colleagues to take away and the Chair stated that further details were available on the CVS Falkirk website, including how to register. The Third Sector SOLD event that the Chair had mentioned at the previous meeting in December has now been cancelled. CVS Falkirk will lead on sharing information about the SOLD Plan with colleagues through the forums, online discussion boards and e-bulletin. The Alliance are holding an Integration Forum with the theme of Prevention on Monday 27 th February, 1-3pm at Robertson House in Glasgow. This is open to anyone with an interest in health and social care integration and is free to attend. More details can be found here - CVS Falkirk are co-ordinating the Syrian Refugee Resettlement Group, which comprises representative from CSREC, Trinity Church, Falkirk Muslim Forum and CVS Falkirk. The group would like to have a representative from the Community Care & Health Forum join the core group to help them engage with the wider third sector when needs of new or existing families are identified. For more details, contact Claire Bernard Claire.Bernard@cvsfalkirk.org.uk The Chair reminded forum colleagues of the changes to communications around the forums that will take effect from 1 st April. CVS Falkirk will no longer colleagues about the forum meetings, but share information through the e-bulletin, dedicated forum webpages and online discussions boards about the forums. 7. Discussion of Key Issues The Chair raised the earlier issue of training within the third sector. Using the practical example from the Children s Forum, the Chair questioned partners whether sharing a proforma might help to map both the current training opportunities and the training needs of third sector partners. There was a general agreement that this would help but if sharing with colleagues from other sectors, it was vital that this process is a two-way street. 7
8 A forum member asked whether Falkirk Community Trust could be invited to do a presentation to the forum on the work that they are doing around health, in their role of promoting health. Lee Kelso from the Community Trust responded that she would be happy to do this. The Chair will contact LK to confirm details of presentation for May forum. A forum member would like to see an overview of proposed funding cuts within health and social care, and wider third sector on the next agenda. Volunteer training are there going to be more opportunities for funding to train volunteers with Health & Social Care? The Chair responded that within the SOLD Plan outcomes there has been a lot of talk of the Third Sector helping to fill the gaps and deliver services, and in his role he has been promoting that in order to do this, third sector organisations need to be properly resourced to deliver services. Leigh Barnard raised that her role as Disability Employer Advisor at the Job Centre is changing and is looking for support to get the information about her new remit to third sector colleagues, particularly how she can help to advice organisations to support employees or volunteers with disabilities. She also raised Dynamic Purchasing which may replace tendering around services. A forum member said that it has been mentioned in another local authority. It was questioned how this might work with the Employability Training Unit (ETU) but LB responded that this would depend on how the opportunity was funded. LB is happy to share information about Dynamic Purchasing with partners. Derek Blues raised that Scottish Health Council offer a training package under community empowerment around help people to get a better knowledge of how Health & Social Care works in Scotland. DB will send to the Chair for it to be included in the CVS Falkirk e-bulletin. 8. AOCB A forum member asked about the hyperlinks on the e-bulletin as they felt they were unreadable due to the colour (pale turquoise). The Chair said he would pass this comment onto the CVS Falkirk Communications Officer to see if this could be changed. Public Partnership Forum Events: The Chair raised that any partners that have events on could send the details to the CVS Falkirk Communications Officer to be included in the CVS e-bulletin. Details should be ed to Laura Jamieson laura@cvsfalkirk.org.uk Tracey Fullerton raised that her and Jane Robertson are now managing Living It Up for the Forth Valley at a local level, as well as this being linked with ALISS. TF said that they would be happy to provide a presentation of the updated system at the forum meeting in August following the upcoming update. 8
9 9. Date of Next Meeting The date of the next meeting is Tuesday 9 th May 2017, 10am-12pm at Falkirk Fire Station. The Chair also let the forum know the other forum dates for the year: Tuesday 8 th August, 10am-12pm Tuesday 7 th November 10am-12pm. 9
