2 Hepatic Procedural Purpose: Prerequisites: Training: Assessments: This unit is designed to cover the theoretical and practical curriculum for ultrasound guided hepatic procedures. Learners should have completed the ASUM Physics Image Optimisation unit or accredited equivalent course. Recognised either through attendance at an ASUM accredited Hepatic procedural course or equivalent. Learners are required to provide evidence of satisfactory completion of training sessions, supervised ultrasound scans and documentation in a logbook. Course Objectives After completion of the course learners should be able to demonstrate: Correct patient positioning and the ability to effectively perform an ultrasound examination of the liver using different scan planes and techniques. An understanding of the utility of ultrasound in guiding liver biopsy An understanding of the different techniques of ultrasound guided paracentesis Correct patient positioning and the ability to effectively perform and interpret ultrasound for localization and guidance for paracentesis and liver biopsy. Course Content Courses will effectively present: Optimally positioning the patient How to image and identify:

3 The liver and the hepatic and portal veins, biliary tract and focal liver lesions using different scan planes and techniques and identify the best site for liver biopsy and Understand the different requirement for focal liver biopsy, which is beyond the scope of this unit. The kidneys, spleen, bowel and bladder,ivc and aorta The abdomen to identify ascites The different techniques of ultrasound guided drainage (indirect v direct method) and hepatic ultrasound: The advantages, disadvantages and different skill levels required for these techniques, and the utility of ultrasound in guiding paracentesis In general, the direct method with in plane visualization (i.e. with real time visualization of the whole needle throughout the procedure) should be reserved for those with greater experience and specific training in these techniques. Fluid collections and estimating the depth of fluid collections from the skin Limitations and Pitfalls Understand the limitations of ultrasound of liver and biliary system due to patient habitus, gas, positioning techniques and machine limitations and be aware of the need of formal ultrasound diagnosis and the need for other imaging modalities. Teaching Methodologies for the Hepatic Procedural Courses All courses accredited toward the CCPU will be conducted in the following manner: A pre-test shall be conducted at the commencement of the course which focuses learners on the main learning points Each course shall comprise at least 3 hours of teaching time of which at least 2 hours shall be practical teaching. Stated times do not include the physics, artefacts and basic image optimization which should be provided if delegates are new to ultrasound Learners will receive reference material covering the course curriculum. The lectures presented should cover substantially the same material as the ones printed in this curriculum document The live scanning sessions for this unit shall include sufficient live patient models to ensure that each candidate has the opportunity to scan. Models will include normal subjects and patients with appropriate pathologies. Given that it may be difficult to find subjects with sufficient pathology, it is appropriate to include a practical image interpretation session in which candidates must interpret images of the relevant pathology. If the latter are unavailable, there will be at least one image interpretation station with cineloops demonstrating the appropriate pathology. For interventional procedures, appropriate phantoms may be used.

4 A post-test will be conducted at the end of the course as formative assessment. Assessment and Logbook Evidence of satisfactory completion of training sessions Evidence of assessment of competence (summative assessment) signed off by a suitably qualified assessor (DDU, Radiologist, DMU, or CCPU in relevant field). The original completed competence assessment form is to be sent to ASUM with the candidate s completed log book. Logbook requirements need to be completed, and logbooks need to be submitted within two years of completing an accredited course. Formative Assessments At least 2 formative assessments (directly supervised with suggestions and advice provided during the scan) Summative Assessment Summative assessment is to be performed by a suitably qualified assessor (see above) using the competence assessment form supplied at the end of this document. Logbook Requirements Logbook requirements need to be completed, and logbooks need to be submitted within two years of completing an accredited course. At least 5 scans demonstrating an appropriate site for paracentesis At least 5 scans demonstrating an appropriate site for liver biopsy All cases are to be reviewed and signed off as adequate by a suitably qualified assessor (see above). At the discretion of the ASUM CCPU Certification Board candidates may be allowed an alternative mechanism to meet this practical requirement. This syllabus was created by Dr. Richard Johnson.
