Context Terms of Reference to support the Physical Therapy Technical Advisor TIQRA project - Afghanistan In Afghanistan, physical rehabilitation as physiotherapy (PT) and prosthetic and orthotic (PO), is not yet well known, understood in terms of its contribution to health and socio-economic development outcomes. This contributes to these services not sufficiently being included in health policies, the sector being under resourced and funded. Rehabilitation services are, if not absent, only partially available with concentration in urban centers, not spread uniformly throughout the country, and exist through Non Governmental Organization (NGO) initiatives that are often dependent on external funding and support. The WHO states that inequitable availability of services is one of the greatest challenges faced by healthcare services providers in Afghanistan 1. Based on the data gathered by the Department of Rehabilitation and Disability (DRD) of MoPH, only 22 out of 34 provinces (65%) have physical rehabilitation services which is insufficient to support the current needs. Another important challenge, is that the rehabilitation workforce is limited in number and quality to respond to the needs of the afghan population effectively. Only one training institution exist for PT in Kabul, and rehabilitation professionals are not well recognized by the government. This is also difficult to recruit the right people from other provinces in the country, particularly women. Many students from Kabul that have completed their PT training do not want to leave the capital. Knowing that many villages and towns lack basic infrastructure and are situated far from health services, combined with security challenges,, lack of roads, difficult economic situation of the families, cultural constraints for the women restricting their movement ; certainly suggest that chronic impairments and consequences of trauma, would be disabling when people face such difficult environment. In addition, there are estimates that 92% of the disease burden in the world is related to causes that require health professionals associated with physical rehabilitation 2. and by 2030, the top ten causes of disease will be conditions that require physical rehabilitation 3. 2 To support the government addressing these issues, since December 1 st, 2015, a consortium of 3 partners : Handicap International (lead organization), Norwegian Afghanistan Committee (NAC) and Swedish Committee for Afghanistan (SCA) has been contracted to implement the project "Towards Improved Access to Quality Physical Rehabilitation in Afghanistan (TIQRA)" supported by the European Union for 40 months. The key output of the project is that 200 trained physiotherapy students (50% female) will be graduated by the early On the same time, the project "Strengthening of P&O Services in Underserved Areas of Afghanistan has been developed. From January 25 th, 2016, a consortium of 2 partners : Handicap International (lead organization) and Swedish Committee for Afghanistan (SCA) has been contracted to implement this project which is also supported by the European Union for 36 months. The key output of the project is that 30 trained Prosthetic and Orthotic students (40% female) will be graduated by early Project proposals emphasize on advocacy efforts towards different actors, among them the Ministry of Public Health (MoPH) and System Enhancement for Health Action in Transition (SEHAT) donors and Grants Management Unit (GCMU) of MoPH. The aims of the advocacy are to improve : 1/the recognition of physical rehabilitation professionals ; 2/ the quality of the trainings (initial and continuous training) for these professionals, and 3/ the accessibility of training centers and physical rehabilitation services in remote areas, throughout the country in function of the needs. 2. General objective Under the responsibility of the technical coordinator, and in close link with the volunteer technical advisor, the volunteer work to insure the delivery of training contents and extra resources to the afghan team, according to the timeframe, for the 7 training centers run in consortium, throughout Afghanistan. 3. Objectives 1. To support the teaching material development on time for semesters 5 and 6 In conjunction with the volunteer technical advisor, identify resources from different places. In conjunction with the volunteer technical advisor, compile and complete, or develop training contents for semester 5 6 (details in table below). Recommend extra resources/references as per content of each semester for semester 5 6 (details in table below). 2. To support and ensure good quality of teaching Meetings with volunteer technical advisor, technical coordinator, quality assurance manager, support project officer. If necessary, meetings/skype call with course coordinators and teachers.

Place of work - Work will be done remotely from Lyon, or an other city in the world, from the place the volunteer is comfortable to work. - No laptop lent. - No provision of access to internet. 5. Methodology Discussion/meetings with HI-Afghanistan-Technical coordinator (Fanny Thiebault). Discussion/meetings with HI-HQ-Lyon volunteer technical advisor (Heather Dawson). Discussion/meetings with HI-Afghanistan-Quality assurance manager or Support Project officer (Atiqullah, Idress). (Potentially and if needed) technical coordination meetings with relevant staff every two weeks if the volunteer is available. 6. Expected Outputs The relevant documents will be needed to provide to HI : PT training contents developed before the beginning of the 5th and 6th semesters, according to the timeframe determined. The subjects to be developed are identified in agreement with the technical advisor and the technical coordinator. 7. Timeframe The action plan will be revised regularly according to the needs when necessary.
