1 Proactive Consultant Cycle 1 Connect 5 Select 2 Seek 4 Explore 3 Reflect Proactive Consultant Cycle: This cycle is for consultants working with someone who has reached out for assistance prior to any risky or dangerous situations, and who has a strong awareness. Connect Use Positive Physical Approach to assess the person s personality traits and possible emotional state. Seek Use open questions to identify known and missing Puzzle Pieces as a team. Reflect Reflect the words used by the person as a guide for the conversation and to dig deeper for history or information that may assist in understanding the current, or future, issues. Explore As a team, problem solve through the possible options of what could be done, or what the priority of needs are. Create a few viable options that would create a positive shift for the person. Select Work together to set 1-2 goals for Puzzle Pieces that can change. Possibly set an immediate action-based goal and a long-term goal. [1]

3 Teepa Snow s Five I m Sorry Phrases Sometimes as Care Partners, we make mistakes and need to apologize. Sometimes as Care Partners, we don t make mistakes, but we still need to apologize. We must look at situations from the point of view of the person living with dementia to understand when and where Teepa Snow s five, I m Sorry, phrases will be needed to keep the relationship strong and valuable. (CP = Care Partner, PLWD = Person Living With Dementia) INTENT I m sorry, I was trying to help. Why am I saying, doing, or not doing what the other person wants PLWD I think you are being mean, frustrating, intrusive, bossy CP I am trying to get you to see that I want to help you and that what you are doing is not OK, safe, what you mean, etc EMOTION I m sorry I made you angry. How I feel about what is happening PLWD I am (ANGRY, frustrated, embarrassed, sad, etc ) about how you are treating me. I do NOT like how you are making me feel. But I may not be AWARE that I am showing you exactly what I m feeling because I can t tell you or don t know what I am feeling. CP I am frustrated because I am only trying to help you and you are getting very upset and yelling or ignoring me, or crying, and I didn t do anything to cause you to be like this. It is the situation you are in! INTELLECTUAL CAPACITY I m sorry, I had no right to make you feel that way. You are absolutely right and you are one of the smartest people I know. Changing roles and abilities sense of being disrespected or less PLWD You think I am stupid, an idiot, that I don t know what I am doing or how to manage my life. I do not think I deserve to be treated like this. I have never been treated like this by you before. I was the boss and the leader before! I KNOW what I am doing! I am a smart and capable adult. CP I see that you are behaving like a child, you are being irresponsible, you are NOT as capable as you were. It is clear that you are wrong about what you can and can t do anymore! Can t you see that you aren t safe to manage your life, house, driving, money, medications, health, bathing, walking, eating, etc on your own? Really, I am scared of this change. DIFFERENT EXPERIENCES I m sorry, that should NOT have happened. Same experience having different memories and beliefs about what happened or has transpired causes different reactions or responses PLWD Someone or something happened that was WRONG. Since they don t understand what I saw, no one cares, and no one believes me. Someone thinks they are going to get away with something that is not OK. I know that you think you can ignore me and what I KNOW is true! CP The reality is that it DID NOT HAPPEN the way she or he thinks it did. They are wrong. If they would just listen to and trust me or think logically about it, they could get it! I am just trying to help them understand why they are wrong. CHANGE I m sorry this is HARD, I hate this for you. What comes next? what once was okay is no longer what is OK for someone not safe, not working, not an option PLWD People are trying to tell what to do and how to do it, and I don t think they have any right to do that. They think they know better than I do, what I need and what I should do. I think people are trying to take away my freedom, personhood, and rob me of who I have always been, for NO GOOD REASON. I don t have a problem, or I don t want you to know about it. CP- I am trying to help you stay safe, keep out of trouble, take care of your wealth, health, or life. You are not recognizing the need for a CHANGE and I have to get you to do it, whether you like it or not! This isn t you. [3]

12 PAC Consultant: Time to Change? Where Should I Live? Factors Home Home with Help Supportive Living Memory Specific Location Safety Ability Engagement Safety No safety issues in or around the home Person s awareness and abilities match up risks are limited & judgment intact Able to care for all personal & care needs or has helpers who are NOT stressed by providing support Stays involved in community, family & daily routines at expected levels Some safety concerns have been identified both in the home and in the surrounding community incidents have been minimal or resolved with interventions (treatment) No incidents have occurred and none are expected Physical supports & available monitoring are adequate for current conditions Person is working with the helpers to complete all needed &desired tasks & routines Helpers foster continued involvement in the home & community at desired & adequate levels without stress on helpers or missed opportunities Help is being generally accepted Level of help is not exactly what is needed, but seems to cover most issues Incidents occur when the help is not present Helpers are stressed but doing the best they can No incidents have occurred No concerns have been IDed Physical supports & available monitoring are adequate for current conditions Person is working with the helpers to complete all needed &desired tasks & routines Available help enables the person to participate in both community and in-house programs that meet needs & desires for all routines & needs Help is being accepted, but some needs/desires not being met The person does not recognize the need for the amount/type of help that is needed given the evidence -Issues & incidents are frustrating but not distressing Smaller and secured environment is accepted & helpful for improved monitoring & response times Person uses the guidance & assistance to participate in both care & life The limits improve the quality of time use & life Secured environment, smaller spaces, more intense guidance and structure permit and foster greater and optimal involvement in life and activities The amount of help and support needed for the person is at the maximum for what is available Safety issues have come up, but are not routine or extreme Self-initiated mobility is very risky lots of risk taking [12]

13 Ability The person can mostly make use of the appliances and equipment, can go where they want and need to, with their own resources The person does most of their routines and gets most of their needs met through their own efforts, but there have been some changes that are concerning Interest in using appliances & equipment EXCEED abilities & judgment Safety incidents have happened no desire to change anything Skills are not there the person may not realize they are not able to do what they think they can Helpers are stressed out and frustrated The person can get where they need to go and get their needs met between the use of helpers and their own abilities, but it is stressful to either or both parties Although the person can do - they are NOT Contacts are more limited, many offers are refused Used to and not this time are becoming common Some reports that there have been some issues with abilities & expectations Some indications that changes are getting started that concern adequacy Not always going to familiar and favorite programs and offerings Becoming lost in the building or routines others notice Asking What do I do now? Where is my place? Mobility getting worse requiring major physical help to safely move & eat Needs for support & help are starting to exceed what is there Engagement Limited interest in or involvement in activities outside the body 1:1 attention & help is needed most of time to get attention & focus Safety Refusals for needed help, given the situations &changing needs Helpers can t keep the person safe due to the setting or person s behaviors or desires vs. abilities Elopement occurred Wandering happens regularly or involves invading others privacy & space Abilities are NOT adequate for safe function in the setting The amount of help and support needed for the person is beyond what is available Safety issues are significant and incidents have occurred Distress is extreme most of time Person s abilities are failing and the severity & time span is such additional support is not enough Family or care deciders are not at the same place regarding plans & care Ability Person and maybe some helpers think things are better than they are there have been incidents, but no major changes or improvements have been made to reduce risk or change situation Person refuses help & helpers Person cannot see the need Person or their helper does NOT recognize the changes, risks, and possible consequences Person s skills are no longer adequate for living within the physical, social, and support environment on a regular and routine basis incidents are beginning to happen Engagement Person is not engaging in any of their usual routines & habits Person needs more help than is available to get or stay involved in life Person is not participating in routines & engagement opportunities in the building without major efforts and major amounts of guidance & support Person is spending almost all the time without engaging with the outside world Almost all time is spent drowsing or in distress [13]

14 Teepa s 10-Minute Stress Tamers Take Care of YOU! Sit quietly in calm surroundings with soft lights and pleasant scents Aromatherapy lavender, citrus, vanilla, cinnamon, peppermint, fresh cut grass Breathe deeply rest your mind and oxygenate Soak in a warm bath, or just your hands or feet Read poetry, spiritual or inspirational readings, a chapter of what you like Laugh and smile Watch classic comedians, Candid Camera, America s Funniest Home Videos, look at kid or animal photos Stretch front to back, side to side, and across Garden work with plants Beanbag heat therapy Fill a sock with dry beans and sew or tie closed- Heat sock with beans in a microwave for 30 seconds at a time- Place on tight muscles and massage gently; relax for ten minutes Remember the good times Record oral memories: scrapbooks, photo journals, keepsake memory, picture frames. Just jot! Do a little on a favorite hobby Have a cup of decaffeinated tea or coffee Play a brain game crosswords, jigsaws, jeopardy, jumbles Look through the hymnal and find a favorite hum it all the way through [14]

15 Books on Tape Rest your eyes and read Soothing sounds music you love, music for stress relief, recorded nature sounds Listen to coached relaxation recordings Pamper Yourself think of what you LOVE and give yourself permission to do it for 10 minutes Neck rubs or back rubs use just the right pressure Hand Massages with or without lotion Take a walk Sit in the sun Rock on the porch Pray or read a passage from scripture Journal Take the opportunity to tell it like it is Cuddle and stroke a pet Have that cup of coffee or tea with a special friend who listens well Pay attention to your personality - If you rejuvenate being alone, then seek solitude. - If you rejuvenate by being with others, seek company. Observe Watch and assess the person and the situation. What is the emotion and at what level? Is the activity Annoying, Risky, or Dangerous? Offer Offer your assistance as someone who may be able to help. Connect Use Positive Physical Approach to meet the emotion and make a personal connection. Support Provide comfort and validation for the emotion and situation. Respond to the Need Help problem solve through the current unmet need to ensure that the situation is no longer dangerous. Responsive Cycle 2: This cycle should be the starting point for consultants working with someone who may have been recommended for assistance, but did not seek it independently, or may have had a crisis already. Connect Use Positive Physical Approach to assess the person and connect Visually, Verbally, and possibly with Touch. Identify Stay curious and assess the level of awareness. Seek Ask this or that questions to identify unmet needs of the individual. Reflect Use validation and affirmation to ensure the need is heard and important. Offer Support the person using comforting touch and validation. Try to problem solve together to agree upon one step towards meeting the need. Responsive Cycle 3: This cycle should be the starting point for consultants working with someone who may have asked for assistance, but is not experiencing regular crises. Connect Use Positive Physical Approach to assess the person and connect Visually, Verbally, and Tactilely. Seek Use choices or open questions to identify known and missing Puzzle Pieces. Reflect Use the person s responses to guide the conversation and ensure correct identification of thoughts. Explore As a team, problem solve to create a short list of viable options to identify Puzzle Pieces to work through, what could be done, or what the priority of needs are. Select Work together to choose an action using what can change. [16]
