4 WHAT IS LIVING THE DEMENTIA JOURNEY? Evidence-based and transformative education program and philosophy. Supports the well-being of persons living with dementia across the continuum of care. Emphasizes living life to the fullest from the perspective of people living with dementia. Builds on the culture change movement.

18 OUTCOMES OF LIVING THE DEMENTIA JOURNEY EDUCATION Increases learners understanding and awareness about dementia and the experience of living it. Increases facilitators capacity to plan, prepare, and deliver education modules. cation modules. Short-Term Creates a shared understanding and philosophy of support, and transforms thinking. Empowers individuals to support the well-being of people living with dementia in supportive care settings and in the community. Intermediate Long-Term Builds and enhances knowledge, skills, and strengths of learners to support those living with dementia with compassion, dignity, and respect.

19 FACILITATOR OUTCOMES LIVING the Dementia Journey Facilitator Training: improved planning and delivering this program provided facilitation skills, tools and resources increased confidence to teach this program average score 4.3/5.0 (n=40)

22 PERSPECTIVES OF PERSONS LIVING WITH DEMENTIA It means a lot to know that such a program is offered to team members, residents, and family members at the Village because it makes them understand what actually goes on. 23 CARE PARTNER PERSPECTIVES It s been next to a miracle for me. All the things they re teaching me now I went through, not knowing what I was going through. 25 LEADERSHIP PERSPECTIVES This workshop makes you rethink how you approach people living with dementia. It helped me understand that people should be treated as people, and not just look at their dementia. You wouldn t call someone cancerous, yet we tend to focus on dementia as their identity. - Community/Home Care Manager It was a great feeling to reassure her that we were going to treat her mother like a human being and that she can still live well when she moves in with us. LIVING the Dementia Journey and the permission if offers each team member to deepen their relationships with residents is a big part of this transformation. - Long-Term Care Administrator
