1 Supplementary Report 7: Chart of the Palliative Care Strategic Framework in Lothian Prepared by the Lothian Palliative Care Steering Group as part of the development of the Lothian Palliative Care Strategy : Living and Dying Well in Lothian July 2009

3 Palliative Care Strategic Framework The short document overleaf outlines the strategic framework in which we have developed the Palliative Care Strategy in Lothian and takes into account work done both nationally and locally in Lothian. All the national publications referred to are available on the internet at the following addresses: Audit Scotland: Care Commission: Living and Dying Well: ww.scotland.gov.uk The NHS Lothian Living and Dying Well Delivery Plan, the Bed Capacity Review and the NHS Lothian Response to Audit Scotland can be found in supplementary reports 4, 5 and 6 respectively.

4 National Publications Audit Scotland Review of Palliative Care Services in Scotland Care Commission: Better Care Every Step of the Way Living & Dying Well: A National Action Plan for Palliative and end of Life Care in Scotland L&DW Short Life Working Groups and Implementation (see overleaf) NHS Lothian Strategic Planning Department NHS Lothian Response to Audit Scotland NHS Lothian completed the self assessment included within the report and developed an Action Plan to address any issues. This action plan will inform the strategy development. Bed Capacity Review The Bed Capacity review was completed in 2008 to consider future requirements for specialist palliative care inpatient beds. The final report for the review will be published in December 2008 and includes recommendations for the way forward. NHS Lothian Palliative Care Strategy Scope to include: Adult & Children s Palliative Care Palliative Care Partnership approach jointly developed and agreed by NHS Lothian, Independent Hospices etc. 5-Year Plan Vision to 2014 and high level strategic aims jointly developed clearly stated. Strategic service change priorities agreed and stated Financial strategy Local targets and PI s for monitoring strategic change Evaluation criteria NHS Lothian Living and Dying Well Delivery Plan Will address all actions from Living & Dying Well framework Critical service development and change actions , from Lothian strategy Monitoring delivery data and reporting against agreed trajectories Pan Lothian actions and specific CHP actions by area Management and clinical leads responsible for delivery stated in each service area (Living & Dying Well component to be submitted in draft by end of February Final end of March NHS Lothian Delivery Plan overall to be completed by September 2009 and clearly state priority actions for and Operational Delivery One Year Delivery Plan This will accompany the strategy and explain how we deliver the NHS Lothian Palliative Care Strategy and Living and Dying Well. NHS Lothian Palliative Care MCN Work Plan MCN will lead on the monitoring of strategic change. UHD and CH(C)P Management and clinical leads will be responsible for local delivery
