Activity Conditioning Group To regain the movements and skills required to perform everyday tasks such as shopping, carrying basket, clothes line,
3 bed/chair/floor transfers, negotiating steps, ramps etc. The graduation of activities is done weekly based on the assessment aiming at improving physical endurance, balance and confidence. The focus is on learning proper body mechanics in doing these tasks and incorporates energy conservation techniques in daily activities. Fatigue Management Group For suffering from fatigue secondary to their diagnosed health condition. The group is facilitated by occupational therapy and focuses on management of fatigue to maximise participation in activities of daily living. Clients must have the cognitive ability to participate in discussions and complete home activities. Falls and Balance Group For who need to improve their balance, strength and exercise tolerance. Physiotherapists and/or Therapy Assistants are present throughout group for overall supervision. Typically for who do not need 1:1 supervision and have a mid to higher level of balance. Patient eligibility needs to be assessed by the Physiotherapists. Carers Group Provides support and education to family members who care for their loved ones who suffer from Dementia. There are guest speakers from different services to address issues related to Dementia and caring. Cognitive Communication Group A group designed to provide support and strategies to experiencing communication difficulties due to memory or cognitive issues. Facilitated by a Speech Pathologist. Voice Group For voice and dysarthria treatment at conversation level. This group runs in 6 week cycle and will be made up of a mix of participants with varying diagnoses, including Parkinson s Disease, Multiple Systems Atrophy and Stroke. Wellness Group To empower to manage their health conditions within their daily routines. It is a 6-8 week holistic program with guest speakers relevant to the patient s needs. It will offer support to the to prevent ongoing hospital admissions. This group will evolve over time as Nurses discuss with the where they feel the gaps are in their overall health care.

(t) (f) Senior physiotherapist supervising groups of 8-10 in community settings (recreation centres etc) Referrals must be from public health professional although we also accept referrals from PDWA nurses. No restrictions on catchment area as are sent to closest location. Diagnosis of Parkinson s or parkinsonism. 12 month renewals can be made by GP. Referral is valid for 12 months Complete referral form which can be found on the North Metro intranet Usually 3 4 weeks 10 week series Strategy PD class (1.5 hours each class) 10 week series 2x week PD gym (1.5 hours each class) Public health service: no charge for land based classes Scarborough and Beechboro are PD gym classes, all other centres are PD strategy classes.
6 Contact details Staffing and type of service provided (eg MDT, sole practitioner etc.) Duration of service (eg. any limitation Community Physiotherapy Services- South Metro HS Various locations across SMHS including Riverton, Melville, Gosnells and Rockingham (t) (f) Weekly group based specific education, strategy reinforcement and exercise sessions Land based classes are generally 1x week but also run gym based classes for higher functioning which are 2x weekly Programs run by senior physiotherapist in groups of 6-10 Programs run at local recreation centers and other community venues Diagnosis of Parkinson s, Parkinsonism, PSP or MSA. Referrals must be from public health professionals although we also accept referrals from PDWA nurses. No restrictions on catchment area as are sent to closest location. Reciprocal arrangement with NM CPS. Complete referral form which can be found on the South Metro Health Service intranet Further information available on South Metro Health Service Intranet page Referral is valid for 12 months Dependent on waitlist but usually 2-3 weeks Service is time limited with discharge planning commencing at initial assessment There is not a set number of attendances or weeks for either program. Discharge is as clinically indicated but patient must be progressing towards achieving a sub acute rehabilitation goal to continue attending. Ongoing maintenance is not provided. Public health service: no charge for land based classes Some pool venues charge pool entry Patients can also access CPS neurology or general rehabilitation classes and multi gyms if unable to attend a Parkinson s specific class CPS offers hydrotherapy classes and people with Parkinson s can be allocated into a hydro class if clinically appropriate and they meet criteria for service/program.

Day Rehabilitation Full MDT available Referral criteria (eg medical diagnosis of PD, geographical catchment etc.) Outpatients: Ramsay Health- Hollywood Allied Health Services 1:1 assessments and treatments for all allied disciplines with a multidisciplinary team collaborative goal orientated approach to outpatient care. Inpatients: Booked admissions from home for inpatient rehab from the Dr managing the patient. Usually admitted to Edwards Ward. Admission for any PD related issues, decreased mobility, change in function. Can be admitted to general medical ward initially for medical issues. Rehab Ward Admission Details: Inclusion: Have achievable rehab goals; be accepted by a rehab
9 consultant, able to be funded by private health insurance or Dept of Veteran Affairs. Exclusion: Patients unable to participate in Rehab e.g. advanced dementia, severe behavioural problems. Day Rehabilitation Current inpatients have access to ongoing Day Rehab at HPH. Length of attendance determined by health fund. Eligible health funds - HIF, HCF, MBP, AHSA DVA patients can take community referrals for Day Rehab via Rita Malik or a Geriatrician. Patients need to be assessed by a HPH Geriatrician consultant first. Outpatients: Ramsay Health - Hollywood Allied Health Services Self- referral and/or referral from allied health/ G.P s/ consultants Inpatients To refer contact: HPH and ask for Rehabilitation Consultant or contact Area Manager/Admissions Rehab Nurse on Day Rehabilitation Available to inpatients at HPH or external DVA patients referred by Geriatrician. Day Rehabilitation health fund eligibility check for the patient is required to ensure no cost to the patient. DVA patients accepted by Rita Malik no fee Outpatients: Ramsay Health - Hollywood Allied Health Services is a private MDT out- patient department. Please call to discuss fees. Deep Brain Stimulation Surgery at HPH. Dr Julian Rodrigues/Dr Stephen Lewis. Pre and postop assessment and care available for patients through Outpatients or Day Rehabilitation.

Therapist assessments are conducted to assess suitability for group therapy Approximately 6 weeks to initial contact with Day Therapy Physiotherapist. Wait times for group activity vary Parkinson s groups run in blocks of eight weeks and include a group for high level and low level patients. It is a MDT service with five sessions specifically dedicated to physiotherapy services. Groups are run 4-5 times per year. No charge Carers are encouraged to attend the low level groups.

The Parkinson s Service at Mandurah is part of the Community Rehabilitation Service based at Rockingham General Hospital There are strong links between AH staff across the 2 sites No PT home visiting but OT does home visiting There are numerous community ex classes that can be linked into.

We are unable to accept referrals from allied health practitioners. Referrals are triaged by Head of Service and then allocated to a consultant at OPH for medical governance. They continue to see their neurologist for medical management. If medical management of the patient is at OPH we can receive cross referrals from any allied health staff - i.e. reassessment following CPS, discharge from hospital etc. We service North Metro Health Area. In some instances we do also see patients South of the River (mainly if they are medically managed by OPH consultants) 2-3 weeks Ongoing. Minimum review will be 12/12 Unless patient has a history of not attending appointments /cancelling/resides in residential care/housebound then we would see them as a as referred basis. Nil Service offers: 1:1 with PT- assessment, treatment, timetrials 1:1 with TA- exercise programs with strategy reinforcement Groups- delivered by OT, PT and TA. Speech therapists attend at the beginning for speech exercises. Class involves education and exercise components. Maintenance group- Exercise only Physio- Chi Homevisits

15 Rehabilitation in the Home Covers South Metro Health Area. Similar service also covers North Metro. Services may become Metro wide. Clients requiring intensive rehab directly following discharge from hospital where rehab can t be provided in an outpatient setting on discharge, to reduce length of inpatient stay, and or have goals around their home environment and accessing the community. Referrals can be accepted from the community where RITH input may prevent a hospital admission. All referrals have to be called and accepted by an Admissions therapist with referral paperwork scanned or ed prior to client discharge, with the client s written consent. Clients are seen the following day of discharge. Duration of service (e.g. any limitation Average duration 4-6 weeks, depending on the client s needs. Clients are seen intensively for 3 of more contacts weekly. Covered under Medicare. Cost to overseas if no reciprocal rights. NIL

In the case of Parkinson s disease a diagnosis or working diagnosis of Parkinson s Disease, with current management with Neurologist/Geriatricians. Geographically patients will have to reside in SCGH catchment. Preferable to have an e- referral for those with access to.
