AccessHC is a community health service operating across 12 sites in the Cities of Boroondara, Manningham and Yarra offering a range of medical, health and community services. AccessHC is a not for profit organisation with caring at its centre. Employing over 250 staff, our Mission is to build healthier lives together with our communities and deliver excellent health services for all. A high priority is placed on improving the quality of our client s lives through health education and health promotion and the
2 delivery of integrated services. Whilst the majority of services are provided through funding from Federal and State Government programs, private practitioners also operate at the clinical sites. The Alcohol and Other Drug service at AccessHC offers a number of specialist AOD services to the community at no cost to clients, including: Youth services for clients aged years Adult services for clients aged 16 years + Forensic assessment and treatment services Family counselling for family/friends affected by substance use Peer support services for families and individuals A secondary NSP on site to promote harm reduction for clients who are injecting drugs Pharmacotherapy services with AccessHC GPs As an AOD service based in a community health centre, we are also able to link our clients with a range of free and low-cost health services, including GPs, nurses, allied health and dental. In relation to AOD counselling services, The Alcohol and Other Drug service is funded to provide services to four key groups of our community: Adults AccessHC is contracted by the Dept of Health and Human Services (DHHS) to provide a range of assessment and treatment services to individuals (aged 16 years and above) in the Inner East Region of Melbourne, including Intake & Assessment, Care & Recovery Co-Ordination, Counselling and Non-Residential Withdrawal. These services are delivered as part of the Eastern Consortium of Alcohol and Drug Services (ECADS) in a partnership between AccessHC, Turning Point/Eastern Health (lead agency), Link Health and Community, SalvoCare Eastern and SHARC. In addition to these services, ECADS have close connections with the residential withdrawal,
3 addiction medicine, specialist pharmacotherapy, consultation liaison and dual diagnosis services offered by Eastern Health Turning Point. Linkages are also established with a broad range of community-based services, including community mental health services, other Alcohol and Other Drug services (including residential and non-residential services), Youth Justice, Corrections Victoria, and housing/accommodation services. Youth and young people The Alcohol and Other Drug service offers assessment, case management, therapeutic counselling for young people aged years, and youth outreach support for young people aged years who are experiencing substance use problems. AccessHC also operates the headspace Hawthorn centre which is located next door to AccessHC in Hawthorn. The Alcohol and Other Drug service provides specialist AOD assessment and treatment services to young people presenting to headspace Hawthorn with substance use concerns. In addition, we provide specialist youth education and harm minimisation programs to schools and community groups in the Inner East. Families The Alcohol and Other Drug service receives State and Federal funding to provide assessment, counselling and support services to individuals and families who are affected by someone s substance use, including targeted funding under the Victorian Ice Action Plan to support families affected by ice and other drugs. Forensic populations: The Alcohol and Other Drug service at AccessHC provides specialist assessment and treatment services to forensic clients who are referred by ACSO Community Offender Assessment and Treatment Services (COATS), Corrections Victoria, Department of Justice, Child Protection and other agencies. Most of these clients are mandated to attend treatment as a condition of a criminal justice order. Position Purpose The Alcohol and Other Drugs (AOD) Clinician will work with adults (aged 16yrs and over) to provide assessment, therapeutic counselling, case management and care co-ordination. The position will work alongside the Intake and Assessment clinician and other AOD clinicians in our team, supported by the Clinical Lead and Manager.
4 WHAT DOES THE ROLE INVOLVE? There will be three components to the role: Conducting comprehensive AOD assessments, developing collaborative individual treatment plans and making referrals to support the client s recovery journey Providing therapeutic AOD counselling for clients using evidencebased treatment and a dual diagnosis framework A small component of case management and care co-ordination for clients with complex and multiple needs The AOD Clinician will work within a recovery-orientated model which uses a strengths-based, client-centred and family inclusive approach to substance use. The clinician will use a dual diagnosis framework and no wrong door approach in their assessment and treatment of people presenting to our service. Therapeutic interventions may include motivational interviewing, mindfulness, acceptance and commitment therapy (ACT), cognitive behavioural therapy (CBT) and relapse prevention. There may also be opportunities to co-facilitate therapeutic AOD and dual diagnosis groups within our service. WHO ARE THE CLIENTS? The AOD Clinician will work with adults, with most clients aged 25 years and over. The role includes work with voluntary and mandated (forensic) clients. Some clients may engage in individual therapeutic counselling, whilst others may require a combination of counselling and case management/care co-ordination. Our clients can also access a peer support group run by peer facilitators, brief interventions and therapeutic groups as appropriate. All clients who present to the Alcohol and Other Drug service at AccessHC are able to access a range of community-based health services at AccessHC, including GPs, nursing and dental services. WHERE IS THE ROLE BASED? This position will be based at Hawthorn and Doncaster East sites, with days and splits across the two locations to be negotiated. Although the role will primarily be clinic-based in the provision of therapeutic counselling, there is a small component of case management and care co-ordination work in this position. This may include assisting clients to engage with other services (eg. mental health services) and attending care team meetings with other professionals.
5 Position Functions Key areas of accountability Drug and Alcohol Assessment Services Conduct comprehensive and client-centred assessment (including risk assessment) of clients presenting to the drug and alcohol service, administering Department of Health and Human Services AOD assessment tools and other relevant assessment tools as appropriate Conduct assessments using a dual diagnosis framework, including screening for mental health concerns and provision of brief interventions where appropriate Develop collaborative Individual Recovery Plans and treatment plans with clients based on the comprehensive assessment Conduct assessment of forensic clients, including DDAL, CISP and CREDIT Bail assessments Complete assessment reports and documentation in a timely manner as required and maintain accurate and up-to-date client files Facilitate and conduct referrals to other health, mental health, AOD and welfare support services (including residential withdrawal, rehabilitation and supported accommodation) Work within a collaborative care-team approach and participate in multidisciplinary team and case management practices as appropriate Contribute to the support of clients who are waiting for service provision and assist in the management of waiting lists and service allocation Provide secondary consultation, advice and recommendations to internal and external stakeholders regarding drug and alcohol issues Drug and Alcohol Treatment Services Provide therapeutic counselling to voluntary and mandated clients using evidence-based approaches, such as Cognitive Behavioural Therapy (CBT), Acceptance and Commitment Therapy (ACT), motivational interviewing and mindfulness based therapies
6 Provide therapeutic interventions to adults presenting with substance misuse (with and without co-occurring mental ill health issues) within a harm minimisation and dual diagnosis framework; where appropriate this may include treatment of mild-moderate mental health concerns (such as anxiety) alongside treatment of substance misuse Facilitate therapeutic group programs and brief interventions as required; participate in the development of therapeutic and support groups to respond to the needs of clients Work collaboratively with clients, families and significant others to support treatment goals and recovery Provide brief intervention, crisis management and relapse prevention to clients as required Participate in case allocation, case review and discharge planning processes Provide a holistic and accessible service to adults who present with multiple and complex needs, including linkages with other support and treatment services Participate in case management and service coordination practices with other professionals, supporting the clients journey through relevant treatment services Documentation and administration Participate in clinical review, case conferencing, intake and allocation meetings within the Alcohol and Other Drug team as required Conduct reviews and discharge planning and liaise with the Manager/Clinical Lead as appropriate Contribute to the planning, development, delivery and evaluation of the AOD service model at AccessHC and ECADS Build on and maintain networks and referral pathways with relevant internal and external stakeholders (for example, General Practitioners, mental health services and other AOD providers) Work as part of a multi-disciplinary team to contribute to service and team development and the promotion of the consortium and its services Participate in other program development and project work as required Manage and meet individual performance targets as required for funding purposes; provide accurate and up to-date client records and targets each quarter
7 Represent the service as required in a professional, courteous and empathic manner Maintain high quality clinical case files and related data recording as per program requirements, including accurate and thorough documentation of clinical risk incidents Actively participate in individual and group clinical supervision with the Clinical Lead/Manager Integration with Access Health & Community services Actively participate and contribute to AccessHC service integration activities as requested by the Manager Participate in shared care plans in collaboration with internal services and external agencies as required Other responsibility areas (all staff) Quality and Risk Management Develop and maintain a sound knowledge of and commitment to AccessHC policies and procedures Occupational Health and Safety Ensure that work and services are provided in a safe manner at all times by regularly reviewing practices and environment and by participating in Health and Safety training as required Professional Development Actively participate in relevant continuing professional development as required Standard of Conduct To abide by the policies and procedures of AccessHC applicable to the maintenance of good order and conduct. Key Selection Criteria Relevant qualification in alcohol and other drugs (such as a Cert IV or Graduate Diploma in AOD), or demonstrated experience working in the AOD sector Recognised tertiary qualifications in psychology, social work, mental health nursing, counselling or related field Demonstrated skills and experience in AOD assessment and mental health screening M = D = Desirable
8 Demonstrated skills, training and experience in therapeutic counselling interventions for substance use and/or dual diagnosis Demonstrated service co-ordination practices with internal and external service providers and the ability to provide holistic care for clients presenting with a broad range of substance use and psychosocial needs Demonstrated high level of knowledge and understanding of the AOD treatment sector, the mental health sector and community support services The ability to maintain high level assessment documents, client records, reports and case notes Well-developed interpersonal and communication skills and the ability to work both autonomously and as part of a team Demonstrated experience and skill in the provision of risk assessment and management Understanding of harm minimisation and reduction principles and ability to work with a recovery-oriented model The ability to work respectfully and creatively with a culturally and linguistically diverse client population, including Indigenous Australians.
