Tapi, Gujarat, Pin No. KAPS/ MED/00100/2018/ S/ June 5 th, 2018 : Sub: Report on Eye Camp-2018 organized by Kakrapar Gujarat Site. 2018, ( ) 25-27, Under Corporate Social Responsibility (CSR) of Kakrapar Gujarat Site, the Eye Camp was organized by KAPS Hospital from 25 th to 27 th March, 2018 in association with Divyajyoti Trust s Tejas Eye Hospital, Mandvi. - 25, ,,,,,, 3 4,,, 3 4,, 3 4,, After wide coverage of publicity, pre-operative eye check-up camp was organized on 25 th March, 2018 from 10 AM onwards at KAPS Hospital. The inaugural function was presided by Shri Vineet Kumar, Site Director, Kakrapar Gujarat Site. Shri Nalinbhai Shah, Trustee of Divya Jyoti Trust,Mandavi invited as Guest of honour. Other dignitaries on dais were Dr. Uday Gajiwala, Tejas Eye Hospital, Shri. M.Venkatchalam, Station Director, KAPP-3&4, Shri Ranjay Sharan, Project Director, KAPP-3&4.Shri N.K.Mittal, Chief construction Engineer KAPP 3&4 Other invitees in the function were senior officers of Kakrapar Gujarat Site, union representatives, ladies club members and Sarpanch from surrounding villages. The function was inaugurated by lighting the lamp in traditional way followed by welcome speech by Dr. (Mrs.) Uma P.Gupta, MOIC, KAPS Hospital. Page 1 of 22

2 Shri Vineet Kumar narrated the details of CSR activities taken up by Kakrapar Gujarat Site for the benefit of people of surrounding villages. Shri Nalinbhai Shah had all praise for CSR works and expressed his gratitude for assisting Tejas Eye Hospital, Mandvi. 506 : 506 patients were screened by Ophthalmologist of Tejas Eye Hospital. Those who required correction of vision for refractive errors were given spectacles free of cost. Medicines were also distributed free of cost to the OPD patients , :,,,, On 26 th & 27 th March 2018, a total of 122 Patients were operated for cataract & other ophthalmic surgeries wherein a Intraocular Lens was implanted in cataract surgery to restore the vision of patients. All the patients & their relatives were served lunch on the day of the pre-operative eye check-up, whereas all patients and their relatives were served food free of cost on the day of operation & discharge. At the time of discharge each patient was given a blanket, Napkin and steel plate with bowl. All patients were satisfied and thankful for the eye care services provided to them with joy and satisfaction quite visible on their faces. The complete details of the Eye Camp is as follows-. There were no post-operative complications and all patients went home satisfactorily.
