3 Five Rivers Health Centers FRHC was formed by taking three (3) former hospital departments of Miami Valley and Good Samaritan Hospitals in May, 2011 Achieved FQHC Look-Alike status in 2012 and FQHC status in Nov FRHC is the only FQHC with over 137 residents largest # of residents for an FQHC in the nation

7 WHAT IS PRIMARY CARE? Primary care practices provide health promotion, disease prevention, health maintenance, counseling, patient education, diagnosis and treatment of acute and chronic illnesses in a variety of health care settings.

14 ASK THE PATIENT What Will YU Prevent TDAY? Question should be focused on the disease/condition being prevented NT the test patient needs to do Would you like to prevent colon cancer today? Instead of, you are due for colon cancer screening, do you want referral for a colonoscopy or test your stool for blood today? Would you like to prevent a heart attack, stroke or kidney disease today? Versus your BP is high and needs to be treated Would you like to prevent cervical cancer today? Versus you are due for your pap test. FCUS N THE TANGIBLE AND DABLE!
