22 Future aims for the 8 Pillars model Test sites result in effective service model Dementia Practice Coordinator role is fully tested We are able to roll out 8 Pillars across Scotland 8 Pillars model is accepted as follow-on from post diagnostic support and self management Alzheimer Scotland developing report on using 8 Pillars principles to support people with advanced dementia/at end of life 8 Pillars model supports systemic integration of health and social care to create better outcomes for people.
